Apesar de 95% das mulheres serem capazes de manter a tolerância normal de carboidratos durante a gestação, entre 1 e 20% delas desenvolve o DMG ou hiperglicemia leve. Duas das principais contribuintes para a resistência insulínica incluem o aumento da adiposidade materna neste período e a dessensibilização à sua ação, por efeitos dos hormônios produzidos pela placenta. O DMG, assim como as outras formas de diabetes, pode trazer complicações e está associado à maior ocorrência de disfunção da musculatura do assoalho pélvico (AP) e, consequentemente, com a incontinência urinária (IU). Apesar da associação entre DM e IU apontada pela literatura, pouco se sabe sobre os mecanismos que a desencadeiam no ambiente hiperglicêmico. Alguns estudos sugerem que as complicações microvasculares da doença possam estar entre os fatores preditores da IU. Conhecer os aspectos fisiológicos que envolvem o DMG e as disfunções da musculatura do AP e a IU, podem fornecer perspectivas para o melhor entendimento destas patologias, bem como uma melhor diretriz para o tratamento destas / Abstract: Gestational diabetes mellitus (GDM) is a complication defined as any degree of glucose intolerance with onset or first recognition during pregnancy. Although 95% of women are able to keep normal carbohydrate tolerance, 1-20% will develop GDM or mild hyperglycemia during pregnancy. Increased maternal adiposity and insulin-desensitizing effects of hormonal products of the placenta are major contributors to insulin resistance. GDM, as well as other diabetes forms, may cause complications and is associated with a higher incidence of pelvic floor (PF) muscle dysfunction and, in consequence, urinary incontinence (UI). Although the association between DM and UI has been pointed out in the literature, little is known about triggering mechanisms in an hyperglycemic environment. Some studies suggest that diabetes-induced microvascular complications may be a predictive factor of UI.
