What Determines a Healthcare Professional¡¦s Intention to Use a Adverse Event Reporting System? An Empirical Evaluation of the Revised Technology Acceptance Model

Objective: Today, many healthcare organizations have implemented health care reporting systems in the hope of learning from experience to prevent or reduce adverse events, medical errors or accidents. However, most applications have failed or not been implemented as predicted. This study presents an extended technology acceptance model (TAM) that integrates subjective norm, trust, and management support into the TAM to investigate what determines healthcare professional reporting system acceptance. Design: The proposed model was empirically tested using data collected from a survey in the hospital environment. The structural equation modeling technique was used to evaluate the causal model and confirmatory factor analysis was performed to examine the reliability and validity of the measurement model. Measurements: Questionnaire administered items measuring the behavioral intention to use the reporting system and five hypothesized antecedents. Results: Our findings indicated that all variables significantly affected healthcare professionals¡¦ behavioral intention to use the reporting system. Among them, the subjective norm had the most significant influence. Conclusion: The proposed model provides a means to understand what factors determine healthcare professional¡¦s behavioral intention to use a reporting system and how this may affect future use. In addition, antecedents to the behavioral intent can be used to predict reporting system acceptance in advance of system development.

An Implementation of an Automatic Customer Power Service Event Reporting System

Customer side automations including automatic metering reading and load interruption are parts of an overall distribution automations (DA). Fault detection isolation and service restoration are considered the most beneficial functions among various DA functions. Due to the extensive areas covered by the distribution network, it is difficult to gather all real-time information from the distribution network. In order to aid in the monitoring of the power delivery quality and detecting fault, an automatic reporting system based on a digital signal processor, a public telephone system and a database system is presented in this thesis. In this system when the sensor detects a low voltage or lose of power, an alarm call will be placed via the telephone network. It will also report the energy usage of customer on a regular base. The control center master then organizes all incoming data into a useful information and displays the locations as they occur. This system will be useful to monitor power at various levels of the distribution circuits.

Innovative command and data handling will be incorporated at low cost and greater accessibility. In this paper we present the necessary communications and control architecture for the space segment and the ground segment of the nanosatellite.

Production Services Reporting System Analysis

The purpose of the analysis was to realise potential areas for improvement within the Production Services department. Sources of inefficiency within the Reporting System were identified to support recommendations for the rationalisation, streamlining and alignment of processes. Lean philosophies and Total Quality Management techniques were reviewed to gain an understanding of applicability to Production Services. Evaluation through Value Stream Mapping determined a reporting process efficiency of 71% and two supplementary waste types were recognised in addition to the standard seven wastes of lean processes. The root cause of inefficiency and waste was investigated further with the 5 Whys Technique. A benchmark was established through reviewing industry based literature and conducting comparative studies of both like and unlike departments. Participant and non-participant naturalistic observation techniques were employed to collect a balanced data set for value stream analysis. Variable environmental factors were recognised to moderate results from direct analysis of the reporting system. Dissimilar political, economic, sociocultural, technological and legal factors were considered during comparison and benchmark. The findings in this report provide evidence of issues that should be addressed for the continuous improvement of both the reporting system, and the department. These have been provided for the consideration of Shell Todd Oil Services Limited Production Services and Logistics Manager.

It is necessary to keep track of the budget expenses for the program, resources used and many other parameters involved in the program. There exist number of commodities and products in the process of educating people. Research is needed to determine which value-added products or processes are economically possible and what percentage of it is accepted by people. For these issues, it is very important to maintain this information in a database and generate reports accordingly. The aim of the project is to create a web interface for users to enter the program information regarding the various programs conducted by Family Nutrition Program. The various kinds of data include information about the budget for the program, information about the various collaborating agencies, various kinds of resources used, services provided, proposed equipment and travel funds etc. Users for this application are county agents who take the responsibility of conducting the program and managing their data. Creating a web interface provides a solution to facilitate the agents to manage their data more efficiently and to monitor their records on a day to day basis. It also aims for generating reports for Family Nutrition Program in order to keep a check over their advancements in the program. This project involves handling of various kinds of information such as FNP Proposals, Agent information, FNP Funds, Collaborating Agencies. Database maintenance is made simple thereby allowing the administrators to add as much as data possible and manage accordingly.

A study on Taiwan Legal System Regarding Dropout Counseling Management for Junior-High Stadents

Education is a long-term effort. The children and teenagers are the pillar-to-be in our nation. Moreover, the popularization of national education can contribute to the enhancement of national competitiveness. Thus, the Article 21 of the Constitution of Taiwan regulates that the people shall have the right and duty to receive national education, which means anyone shall have the right to receive national education and the duty to receive national education as long as he or she is a citizen of Taiwan. Furthermore, to practice education drastically, the nation promulgates the Compulsory School Attendance Act. It allows local governments to establish ¡§committee of compulsory school attendance¡¨ to manage the dropouts from elementary and junior high schools, targeting ¡§zero dropout.¡¨ However, the good faith of the regulations can not be implemented well for every citizen in terms of ¡§the right and duty to receive education,¡¨ especially for the students who drop out of school. It is affected by complicated factors, such as personal value, society, economy, peers, indigenous people and new immigrants. Dropping out has concerned the field of education the most, and it has also brought related problems to the society and public order. Therefore, it is necessary to research and to discuss how to organize an effective legal system of dropout counseling management. Based on the 5 major frameworks of administration law, including administrative principles, administrative organization, administrative authority, administrative remedy and administrative supervision, this study aims at examining the legal system of junior high dropout counseling management to redress the deficiency of current law with researching by reference, historical analysis, comparative analysis and induction analysis in different aspects. Meanwhile, by learning from advanced countries, such as the U.S., Japan and Korea, some specific manners and suggestions, such as establishing dedicated organizations or offices, paying attention to secondary education and reinforcing the function of halfway schools, are expected to be provided for the government, schools, private organizations and future researchers. Through the above-mentioned manners, the situation of dropouts can be redressed and corrected. Moreover, the spirit of the Constitution, namely, the right and duty to receive national education for all citizens, can be fulfilled.

In 2011 South Africa's main electricity supplier, Eskom, experienced a peak electricity demand of 89% of their total installed generation capacity. The high utilisation rate makes it difficult to perform essential maintenance on the system. Eskom implements Demand-Side Management (DSM) projects in various industries, in order to reduce the demand and to ensure sustainable electricity supply. The mining sector consumes 14.5% of the total amount of electricity generated by Eskom. Mine compressed air systems can consume as much as 40% of a mine's total electricity requirements. This makes mine compressed air systems an ideal target for DSM. Electricity load seems to be reduced, but many DSM savings are not sustained throughout the project lifetime. An existing project feedback method of a specific Energy Services Company (ESCo) includes the manual collection of data from the mines and manual generation of reports. These reports show energy savings of the DSM projects to help the ESCo and their clients to improve and sustain the performance of the projects. A great amount of man-hours is used which results in large time delays in the feedback-loop. In order to address this, the need for a new automatic feedback reporting system was identified. This study mainly focusses on the development and implementation of a new method to monitor DSM savings on mine compressed air systems. It includes the reliable collection of data from mines, processing and storing of the data in a central database and generating savings reports. This is done automatically on a daily basis. In order to complete the feedback-loop, the reports are verified and emailed to clients and ESCo personnel on a daily basis. The new reporting system is implemented at a number of mines. Four of these project implementations are used as case studies to measure and interpret the effectiveness and value of this system. It saves a significant amount of man-hours and proves to be of great value in the sustainability of DSM project savings. Projeto, implantação e avaliação de sistemas de relatos de incidentes : um estudo empírico em uma distribuidora de energia elétrica

Devido aos altos índices de acidentes do trabalho nas atividades de distribuição de energia elétrica, é imprescindível o empenho em aprimorar seus sistemas de gestão de segurança e saúde do trabalho. Nesse setor, bem como em outros, o desempenho em segurança é normalmente medido reativamente através de índices de acidentes, abordagem que vem sendo questionada pelos pesquisadores da área, que propõem que seja enfocada a variabilidade do trabalho real. Assim, emerge a premissa de que a gestão de segurança e saúde do trabalho deve estar imersa em uma cultura de informação, onde o sistema de relatos de incidentes se apresenta como ferramenta. É nesse contexto que é apresentado esse estudo realizado em uma concessionária de distribuição de energia elétrica no Rio Grande do Sul. A pesquisa tem por objetivo principal contribuir com a prevenção de acidentes sob uma perspectiva pró-ativa, através da proposição de diretrizes para o projeto, implantação e avaliação de um sistema de relatos de incidentes. A estratégia adotada para condução do estudo foi a pesquisa-ação, o que possibilitou aferir o processo envolvido na implementação do sistema. Durante a fase de projeto, que ocorreu de agosto a setembro de 2009, os pesquisadores em conjunto com membros da empresa determinaram as diretrizes fundamentais de operação do sistema, assim como adaptaram o formulário de relatos de acordo com a realidade da empresa estudada. Na última semana de setembro de 2009 teve início a etapa de implantação do sistema de relatos de incidentes, através de um estudo piloto com duas das sete equipes de eletricistas envolvidas no estudo, precedido por uma capacitação para o uso do sistema. Em meados de dezembro, o sistema de relatos foi estendido para todo o departamento envolvido no estudo e foi conduzido pela equipe de pesquisa até maio de 2010. A etapa de avaliação do sistema ocorreu através de entrevistas, em agosto de 2010 e março de 2011, com membros da empresa envolvidos no estudo, assim como por observações em campo durante toda a pesquisa. Durante a implantação do sistema de relatos de incidentes e a análise dos dados gerados, foi possível aprofundar a compreensão acerca da variabilidade do cenário em que os eletricistas desempenham suas tarefas, assim como identificar ações corretivas prioritárias. Como principais resultados podem ser citadas a prevalência de estruturas fora de padrão como agentes causadores dos incidentes e a influência do ambiente externo na atividade dos eletricistas. Por fim, foi analisado como um sistema de relatos de incidentes pode favorecer os princípios da engenharia de resiliência. / Due to high rates of occupational accidents in the electricity distribution´s activities, an effort is necessary to improve their safety and occupational health management. In this sector, as well as in others, the safety performance is normally measured by reactively accident rates, an approach that has been questioned by researchers, which propose to focus on the variability of the real work. Thus emerges the premise that the safety and occupational health management should be immersed in an information culture, where the incident reporting system is presented as a tool. This is the context of this study, realized in a company of electric energy distribution at Rio Grande do Sul. The research aims at contributing to the prevention of accidents with a proactive approach, by proposing guidelines for design, implementation and evaluation of an incident reports system. The strategy adopted for conducting the study was action research, enabling to assess the process involved in implementing the system. During the project, which occurred from August to September 2009, researchers and members of the company determined the basic orientation of the system operation, as well as adapted the reports form in accordance to the reality of the company. In the last week of September 2009 began the implantation phase of the incident report system, through a pilot study with two of the seven teams of electricians involved in the study, preceded by training. In December, the reporting system was extended to the entire department involved in the study and was lead by the research team until May 2010. The evaluation phase of the system occurred through interviews, in August 2010 and March 2011, with members of the company involved in the study, as well as field observations throughout the research. During the system´s implantation and analysis of data generated, it was possible to deepen the understanding of the variability of the scenario in which the electricians perform their tasks, as well as to identify priorities for corrective actions. Prevalence of non-standard structures as causative agents of the incidents and the influence of external environment on the activity of the electricians were the main results. Finally, it was analyzed how an incident reports system may favor the principles of resilience engineering.
