Transformation of nursing colleges in Gauteng

M.Cur. / Following the democratic elections that took place in 1994, South Africa is undergoing transformation. This transformation was initiated by the external and internal driving forces, of which, the political forces are the most significant. This political transformation led to the promulgation of the Constitution of the country, which impacted on the transformation of the health system, to ensure accessibility and equity. The promulgation of the Constitution was followed by many legislative and policy changes, such as a new Higher Education Act. The South African transition from apartheid to democracy, necessitated that all the existing policies, institutions and practices should be transformed and revised, according to the demands of the new era. With the new political dispensation of the country, many regional, national and local policies have changed. These changes have had an inevitable impact on higher education, which in turn impacts on nursing education. The change in the higher education system required the restructuring and rationalisation of nursing colleges in all the provinces. The need for the transformation of nursing colleges is unavoidable because of the development of the new legislation to readdress the imbalances of the past. Following the new political dispensation, it was however necessary to transform these nursing colleges through rationalisation/integration. The focus of this study is on the transformation of nursing colleges in Gauteng, which poses many challenges to the directors in terms of the management of transformation. The transformation process impacts directly on the directors of nursing colleges. This study is based on the transformation management of selected nursing colleges in Gauteng. The study is conducted in two phases. Phase one explores and describes the experiences and perceptions of directors regarding the transformation of nursing colleges in Gauteng. Phase two describes the formulation of transformation management standards which consists of two phases of standards formulation, namely, the development phase and the quantifying phase.

M.A. / A research study is undertaken with the aim of establishing what the attitudes and perceptions of supervisors and supervisees are regarding the rendering of cultural-sensitive supervision in social work. The aim of the study is to provide supervisors in social work with recommendations regarding the rendering of cultural-sensitive supervision. The amalgamation of organisations rendering social services has brought about multi-cultural and cross-cultural supervisional relationships that could be potentially problematic. The research process followed is the process identified by Mindel and McDonald in Grinnell (1993). The research process is initiated with a literature study on the concepts of supervision and cultural diversity. The research is limited to the Gauteng area and the study did not take place on a large scale due to the qualitative design utilised. An in-depth study is mader rather than a statistical analysis. Data collection takes place by means of semi-structured interviews with social work supervisors and self-administered questionnaires, completed by both supervisors and supervisees. The target group of the study is supervisors and supervisees employed by organisations which already render cross-cultural services. Results of the study indicate that supervisors and supervises generally don't experience problems in the rendering and receiving of cultural-sensitive supervision. Cultural sensitivity is perceived as being the acceptance of cultural factors such as background and values, as well as having a working knowledge of the cultural background, values attitudes etc. of the supervisees. Communication barriers seem to be the problem experienced by the majority of the participants of the study. It is recommended that supervisors receive further training in cultural-sensitive supervision and that cultural-sensitive supervision needs to focus on cultural values rather than biographical aspects such as race. Anthropology included in social work training, could address the need to enhance the knowledge base of social workers and supervisors, to the benefit of supervisor-supervisees relationships and thus the client system. Results and conclusions drawn from this study can not be generalised to the broader population of social workers and supervisors. Further research is needed to determine whether the perceptions and attitudes established in this study are the general perceptions and attitudes of social workers in South Africa.

The experience of biological fathers of their partner's termination of pregnancy

Nobody denies that termination of pregnancy has an effect on women, but very few people realise that termination of pregnancy also has an impact on men. The study tells the story of a few adult biological fathers who accompanied their partners to the various identified clinics in Gauteng for a termination of pregnancy. The goal of this study was to: Explore and describe how single adult biological fathers experienced the termination of pregnancy his partner had. Describe guidelines for the advanced psychiatric nurse practitioner to support adult biological fathers to mobilise their resources and promote their mental health as an integral part of health. The paradigmatic perspective of this study was guided by the Theory for Health Promotion in Nursing (Rand Afrikaans University, Department of Nursing Science, 1999:2-15), which reflects the focus on the whole person. A functional approach was followed (Botes, 1991 :2) based on the model for nursing research developed by Botes (1995:6). The design of the study utilised is a qualitative, descriptive, exploratory and contextual design (Mouton & Marais, 1994:43-44,51) using in-depth, semi-structured, phenomenological interviews as a method of data collection. Field notes were also taken after the completion of each interview, while prior to this permission was obtained from the various identified private clinics in Gauteng where this study was conducted, as well as from the adult biological fathers who voluntarily participated in this study. The results of this study were tabulated according to categories and themes. The first theme identified the powerlessness the adult biological fathers experienced related to the inability to have a choice in the process of the termination of pregnancy. The second theme told of the emotional turmoil experienced by the adult biological fathers related to the impact of the decision on interpersonal and intra-personal relationships. The third theme identified the psychological defence mechanisms the adult biological fathers used as a way of dealing with the stressful effects of the termination of pregnancy.

M.A. / Literature studies have shown that the organization, as well as the individual in the organization is directly affected by violence. The reasons and characteristics of violence have an emotional impact on the employee, and requires of him to work through the impact of incidents in his own time. The result of this process is delayed psychological growth, less creativity which eventually has an impact on productivity. Workmotivation is the force behind reaching goals individually as well as for the organization. External factors which includes violence, have an influence on the employee's ability to feel satisfied in his work situation. The goal of this research study is to determine if Eskom employees that were exposed to incidents of violence have a workmotivation profile that can be identified. A sample was selected with available statistics in Eskom, as well as the snowball method. The sample included 220 respondents. The respondents completed a unstuctured questionnaire, where they had to identify incidents in their work situation that made them feel good / happy, and incidents that made them feel bad / unhappy. The questionnaire was developed by Frederick Herzberg (1968) as part of his motivation hygiene theory where factors of worksatisfaction and rkdissatisfaction were identified. The results were computerized, and quantified by means of descriptive statistics. A workmotivation profile was constructed from the data. The study confirmed that a workmotivation profile of employees that have been subjected to incidents of violence can be identified. Determinants of worksatisfaction and work dissatisfaction were identified as responsibility, supervision, content of work, recognition, promotion, circumstances at work, personal circumstances and violence. On this profile violence received the highest percentage as a determinant of workdissatisfaction.

Transformation of nursing services in selected Gauteng hospitals

M.Cur. / It is explicit that the change in the South African government since 1994 accompanied by the transformation of nursing services with its new policies and practices is inevitable, and poses challenges for the nursing service managers in Gauteng hospitals. The transformation of nursing services can be a difficult process creating a period of fear, uncertainty, stress and role ambiguity for the nursing service managers. The transformation has been so extensive that the nursing service manager's role responsibilities have changed to such an extent that they are now required to manage grossly reduced levels of resources, multicultural societies with different values, beliefs and attitudes and increased numbers of patients, resulting in overcrowded hospitals. The transformation of nursing services demands the demonstration of appropriate nursing service management. The abilities of the nursing service managers are critical to the effectiveness of the future. Most nursing service managers are unprepared for their new roles and the present concern is for the quality of the management of the transformation process. No written standards for transformation management in a nursing service are available in Gauteng public hospitals. Consequently, nursing service managers find themselves making major decisions about nursing service transformation with little information about the effectiveness of alternative approaches. The experiences and perceptions of the nursing service managers regarding transformation of nursing services in selected Gauteng hospitals are also not known. Understanding the nursing service managers' experiences and perceptions will provide a basis for the formulation of standards for transformation management in a nursing service.

D.Cur. / There is currently lack of mobilization of the advanced psychiatric nurse as a resource person in the mental health services of the Gauteng Province. This lack of mobilization of the advanced psychiatric nurse as a resource person in the mental health services, is associated with her supervisors who are the psychiatric nurse managers. The purpose of this research was to develop and describe a model that could be implemented to guide the mobilization of the advanced psychiatric nurse as a resource person in the mental health services in order to facilitate her mental health. The research also focused on developing guidelines for the implementation of the model for the mobilization of the advanced psychiatric nurse as a resource person. The model: "Mobilization of the advanced psychiatric nurse as a resource person: an interactive process", together with its operational guidelines was developed by using a theory generative design, that is, qualitative, explorative, descriptive and contextual in nature. This model was developed according to Chinn and Kramer's (1995) approach to theory generation, namely: identification of the central concepts for the model by conducting a field study to explore and describe the views of the advanced psychiatric nurse and those of the psychiatric nurse managers with regard to the mobilization of the advanced psychiatric nurse as a resource person in the mental health services of Gauteng; analysing the data gathered through focus groups interviews from the sample of the advanced psychiatric nurses and form the sample of psychiatric nurse managers using Strauss and Corbin's (1990) open, axial, and selective coding approach to guide data analysis; analysing the data by identifying, defining, classifying the concepts and placing them into relationship with each other to form relationship statements as the conceptual framework for the model; describing the model using strategies proposed by Chinn and Kramer (1995) and then subjecting the model to evaluation by experts in theory generation; describing the guidelines for the implementation of the model in the clinical setting.

A strategy to enhance positive working relationships among the role players within the managed healthcare context in Gauteng

M.Cur. / Managed healthcare was introduced in South Africa largely in order to control the rampant increase of medical costs, more especially in the private healthcare industry. Managed healthcare is a system of healthcare delivery, which ensures that cost effective quality care is provided to patients without jeopardising the health of the patient. Positive working relationships are necessary among the role players in the provision of cost effective quality care within the managed healthcare context. Role players experience problems in the delivery of healthcare, affecting their working relationships, which in turn affects the quality of care provided to patients. Because managed healthcare is a new concept in South Africa, little is known about the problems. As a result, there are no guidelines for the formulation of a strategy to enhance positive working relationships among the role players within the context of managed healthcare. Understanding the problems experienced by the role players will provide guidelines to the researcher to formulate a strategy to enhance positive working relationships, hence the reseacher sought to explore and describe these problems in the study. The purpose of the study is to formulate a strategy to enhance positive working relationships among the role players within the managed healthcare context in Gauteng. The specific objective is to explore and describe the problems experienced by the role players, as well as the possible solutions within the managed healthcare context in Gauteng. The relevant research questions are: * What are the problems experienced by the role players within the managed healthcare context in Gauteng, and what are the recommended solutions to counteract these problems? * What strategy can be formulated to enhance positive working relationships among the role players within the managed healthcare context in Gauteng? A qualitative, exploratory, descriptive and contextual design was followed to answer the research questions. Focus group interviews and a workshop were conducted to collect data and a content analysis was conducted as described by Tesch (1990). The results were analysed in two phases: the problems experienced and the suggested solutions to counteract these problems. A strategy to enhance positive working realtionships was suggested. The strategy, which is based on role player empowerment, staff development, staff recruitment and selection, use of advanced information technology and standardisation of methods across managed healthcare industry, was derived from a synthesis of solutions suggested by the participants. Guba and Lincoln's (1983:290) principles of trustworthiness were employed by the researcher and the ethical standards as set by DENOSA (1998) were adhered to by the researcher to facilitate the quality of the study. It was recommended that the strategy be implemented and evaluated for its effectiveness by evaluating the quality of working relationships among the role players and that ethical standards be formulated in managed healthcare.

The experience of midwives delivering the babies of HIV positive women

The midwives are the frontline health workers who have to care for these pregnant HIV positive women and therefore are at occupational risk of HIV infection because of their caring role. The experience of midwives regarding the delivery of the babies of HIV positive women was not well addressed before as limited studies have been undertaken about the experiences of midwives, therefore inspiring the researcher to undertake this study. The purpose of this study was to: • Explore and describe how midwives experienced the delivery of the babies of HIV positive women. • Describe the guidelines for health professionals to support midwives in order for them to render good midwifery care. Depression and career choice in matriculants in South Africa

M.A. / In the face of economic hardship, social, political and cultural change, matriculants in South Africa are confronted with increased pressure when exploring and deciding on their career paths. This task in itself is complex and is expected to culminate in a career decision upon leaving school, even when subject to these and other extrinsic limitations over which the individual has no control. For example, the serious lack of career guidance programs in the large majority of our schools (Mathabe & Temane, 1993). Regardless, school pupils in South Africa are expected to make their preliminary choice in Standard 7 when they choose their matric subjects. Three years later they are expected to leave school with a career decision. These same individuals are also struggling to meet the challenges of adolescence and are searching for an identity they can call their own. It is surmised that those pupils who have not yet made a career decision are more likely to experience feelings of depression than those with definite career plans. This study regarding the relationship between depressive symptomatology and career choice, had the following objectives: A comprehensive literature review focusing on depression, depressive symptomatology, the aetiology of depression, theory and research with reference to the adolescent in particular. A further literature review focusing on various theories of career choice and research with particular reference to career indecision. A quantitative and qualitative investigation of career choice and depressive symptomatology in matriculants in South Africa. The initial part of the study focuses on the theoretical review of depressive symptomatology and career choice, to firstly define the concepts as operationalised in this study and secondly to explore the theory and research to date with reference to the factors contributing to and maintaining feelings of depression and career indecision in adolescents. The researcher reviews and discusses phenominological differences and similarities between the adult and adolescent experience of depression, with particular reference to depressive symptomatology, the approach used in this research. Career choice is also defined and discussed with reference to the developmental stage of adolescence. Various theoretical approaches with regard to depression and career choice are reviewed and discussed. Identity development, and the related concepts of self esteem and self efficacy are discussed, as well as the role of cultural factors, with reference to both depressive symptomatology and career indecision Matriculants from four different regions in Gauteng took part in the study. The sample comprised pupils from both genders and various population groups. Each pupil filled in a questionnaire developed by the researcher, the Beck Depression Inventory (BDI) and the Career Decision Scale (CDS). The data collected from the self developed questionnaire provided insight into various factors of career choice such as realistic goals and dream goals, perceived restrictions and elements of self esteem. This data was described and interpreted in a qualitative analysis and from such the researcher inferred predominant themes and concepts with suggested avenues for further research. The quantitative data collected from the results of the BDI and CDS were statistically described and analyzed. From this data the statistically significant findings were presented and discussed with reference to theory and research. The researcher found that depressive symptomatology and career indecision are significantly related with the incidence of feelings of depression being related to career indecision and career indecision possibly giving rise to symptoms of depression. The study can be criticized for not including a measure of self esteem, a factor closely related to depressive symptomatology and career choice. A further criticism is that the most highly represented cultural groups were the English and Zulu speaking pupils, future studies should include pupils from all cultural groups. The qualitative analysis is subject to an interpretative bias, which may be subjective and influenced by the researcher's own views. The recommendations that were made as a result of this study are as follows: More research is needed into career choice and the incidence of depressive symptomatology with coenizance of levels of self esteem, a more representative sample and perceptions as regards career choice and limitations to certain vocations. The implied direction of the relation between career indecision and depressive symptomatology requires in depth research to further investigate the findings of this research. If career indecision does indeed give rise to feelings of depression, then programs that are being developed and those that are already in place should take cognizance of these results and include preventative measures in any future programs. The implied higher incidence of depressive symptomatology and career indecision in the black population requires further investigation and if the findings in this study are further supported, preventative action is desperately needed. Fifty percent of the pupils show signs of moderate to severe depression. Further research is needed to reaffirm these results and, if substantiated, urgent preventative measures should be taken.
