Participatory mapping as an approach for health services co-planning : finding the local voice in the rural context

A key factor to the Scottish Governments' public policy and public service reconfiguration is collaborative working between service providers and service users in a framework of co-production. A second key factor in this reform is a place-based approach. Despite this rhetoric of co-production of health services having been used for some time in health policy, and considering the numerous interpretations of the meaning of both co–production and 'place' in the academic and professional literature, how rural dwellers experience rurality in terms of places and space, and how this may in turn affect health and interactions with co-production, remains underrepresented in health policy planning. In light of changes in health service provision, rural health care poses itself as a potentially emotive and sometimes volatile topic. A co-productive approach may encourage understanding, acceptance and better usage of health services and neighbourhood resources, by the residents registered as patients with local medical practices. This doctoral research study considered the use of participatory mapping techniques to generate, gather and capture the local voice of residents from two rural Scottish communities, regarding the self-perception of their health in relation to the place they call home. Through a participatory action research approach, using iterative co-design, residents were asked how their environment impacted on their health, and in particular their cardiovascular health. Qualitative data were collected through participatory mapping techniques and co- analysed using a thematic analysis process. The application of the concept of therapeutic landscapes revealed the importance of the 'sense of place' and its impact on health, along with the physical, social and cultural environmental aspects traditionally considered in public health disciplines. Results were digitised using geographic information systems (GIS) to illustrate the interactions between place, people and health, through a relational lens. This research demonstrates a working example of how, drawing from the discipline of health geography, a place based approach can make an important contribution to rural health service co- planning within a co-production framework.

Mapping the uncertainties in internet-based clinical trials : a systematic review and qualitative study

This thesis maps the growth of the use of internet technologies in randomised controlled trials in health care and public health, and explores the methodological and ethical issues that arise from their use from the perspective of researchers and participants. Online clinical trials are growing in number, and claim to offer benefits for researchers and participants, providing solutions to some of the inherent problems associated with traditional trials. However, little is known about how many internet-based trials have been conducted, what methodological research has been undertaken, or what impact the new technologies might have on researcher or participant experience. The thesis followed a step-by-step approach, using information science, research synthesis, and qualitative methods. The creation of a database of internet-based clinical trials established that they have grown rapidly in number, use internet technologies primarily to deliver an intervention, predominantly in behavioural, mental health, or life-style public health settings. A two-stage systematic review, comprising a descriptive map and a qualitative synthesis, established what is known about the methods, conduct or participant experience in internet-based trials. A qualitative primary study was then carried out, based on the findings of the review, to further explore the views, attitudes and experiences of researchers, participants and the public, into the motivations, benefits or barriers to taking part in internet-based clinical trials. Themes emerging from the research suggest complex interactions between design and technology, particularly in the area of participant characteristics and choice; convenience versus intrusion; impact of time and place; the pace of change and impact of societal changes in the use of technology. A range of ethical considerations emerged, including the nature of informed consent, ethical approval, and the need for a systematic approach to patient and public involvement. Recommendations are made to help inform and improve research practice in the digital age.

Aprendizagem criativa é um conceito em construção (CRAFT; CREMIN; BURNARD, 2008), sendo que nesta expressão aprendizagem indica que o foco está centrado no processo de aprendizagem da criança, e criatividade refere-se ao potencial criativo a ser desenvolvido dentro de um campo de conhecimento específico, como a música. Do ponto de vista sociocultural, esse conceito considera a interação de fatores culturais e sociais, como o contexto e a perspectiva da criança, o processo colaborativo e não competitivo, a análise e reflexão dos trabalhos como ponto fundamental para a aprendizagem colaborativa e a interação entre as crianças e com o professor. Tal referencial envolve, portanto, o papel do professor na promoção da aprendizagem criativa, a agência das crianças no processo de aprendizagem, e a prática reflexiva tanto dos professores quanto das crianças. A metodologia de pesquisa de natureza qualitativa integrando elementos da pesquisa participante e pesquisa-ação foi adotada por possibilitar a intervenção do pesquisador no contexto e a inclusão dos colaboradores da pesquisa nos processos de construção de estratégias metodológicas e reflexão sobre a ação pedagógica, questões importantes para que atingíssemos os objetivos da pesquisa. A produção de dados foi realizada em duas etapas: (1) curso de formação para professores de música visando introduzir o referencial de aprendizagem criativa e discutir aspectos sobre a atuação do professor; (2) acompanhamento da ação pedagógica de duas professoras de música participantes do curso e atuantes na Educação Infantil, através de observação de aulas, reuniões para planejamento e reflexão. Dessa forma foi possível construir um processo de formação, ação e reflexão, e um espaço para manifestação das expectativas e dificuldades dos professores. Os resultados indicam que a abordagem de aprendizagem criativa influenciou as concepções dos professores refletindo em mudanças no planejamento e ação docente, tais como: menos estruturação e interferência dos professores e mais liberdade para as crianças; realização de trabalhos de composição em pequenos grupos; reavaliação de uma prática criativa para uma prática que promove a criatividade; documentação das atividades e produções das crianças, procurando dar voz às crianças, estimular e mediar a aprendizagem. Estudos futuros poderiam ser realizados com o objetivo de envolver outros profissionais da Educação Infantil e evidenciar a perspectiva das crianças nesse contexto sob a ótica da aprendizagem criativa.
