Loading and velocity generation in the high performance tennis serve

[Truncated abstract] Shoulder injuries rank among the most prevalent and debilitating sustained by professional tennis players. The loads, or magnitude, location, direction, duration, frequency, variability and rate of force application, endured by tissues of the shoulder during stroke production, and more particularly the serve, are commonly implicated in shoulder joint injury (Chandler et al., 1992; McCann and Bigliani, 1994; Kibler, 1995). Indeed, past evidence points to these loads increasing along with serve velocity, as well as with varied segment use (Elliott et al., 2003). This dissertation therefore aimed to quantify hypothesised relationships between certain serve types and techniques, and shoulder joint loading among high performance able-bodied and wheelchair players. . . Of final note is that prospective 3D biomechanical examinations of shoulder joint motion in the tennis serve should consider placement of humeral triads distal to the biceps and/or triceps muscle belly. In comparison to markers placed at the mid-point of the humerus (i.e. as used in this thesis), these more distal triad positions appear to alleviate the spurious effects of soft tissue artefact thereby enhancing the accuracy of estimated long-axis rotation of the upper arm. Although the current representation of 3D humeral motion did not confound the comparisons made between serve types or techniques, it is likely that upper arm triads located just above the epicondyles of the humerus could have offered more insightful absolute comparisons to the literature. Further, the elaboration of a joint coordinate system at the shoulder to provide for the more meaningful and functional expression and interpretation of shoulder joint kinetic and kinematic data should also be central to all future, related investigative efforts.

The prevalence and pattern of myofascial trigger points in the shoulder girdles of swimmers as compared to non-swimmers in the greater Durban area.

Dissertation submitted in partial compliance with the requirements for the Master’s Degree in Technology: Chiropractic Durban University of Technology, 2013. / Objectives: Myofascial pain dysfunction is a common musculoskeletal disorder, known to affect athletes. This research aimed to create a map of myofascial trigger points (MFTPs), to ascertain sport specific combinations. Design and Setting: This IRB approved study was a cross-sectional, observational study. Participants: Forty swimmers and forty non-swimmers (soccer players). Measurements : All participants underwent one assessment, non-intervention session where primary measures included: shoulder disability index (SDI), myofascial diagnostic scale (MDS), algometer and numerical pain rating scale (NRS). Manual palpation, the MDS and an algometer assessed MFTPs and the SDI overall function. SPSS version 20 (IBM) using Pearson’s chi square tests / Fisher’s exact tests compared MFTP locations between the groups, and non-parametric Mann-Whitney tests compared continuous measures (due to significant non-normal distribution), with a p-value <0.05 level of significance. Results: MFTP presence is very uncommon in swimmers, with associated pain and loss of function being very low on average. No evidence was found that swimmers were affected more than non swimmers by MFTPs related pain or loss of function, but has indicated that algometer measurements for infraspinatus MFTP 1, were significantly higher (p<0.027) (showing decreased tenderness) than the values in non swimmers. Conclusions: These results contradict the literature which suggests that unique activity specific patterns of MFTPs exist. This may be as a result of underlying systemic causes of MFTPs that obscured the pattern in this study. It is therefore suggested that larger trials with more participants per group be done in order to verify the results of this study.

Job coach model for occupational shoulder soft tissue injuries rehabilitation. / CUHK electronic theses & dissertations collection

A "Job Coach" model was developed based on sports medicine and rehabilitation principles for athletes, and individual placement and supported employment for people with psychiatric disabilities. This is a biopsychosocial model, emphasising workplace-based intervention. / Background. Occupational musculoskeletal soft tissue injuries represent a major source of work disability. There has been a gradual rise in the occurrence of occupational musculoskeletal injuries of the upper extremity, including both acute injuries and more chronic health problems. Return to work following an occupational injury is a multifactoral process, although traditional clinic-based rehabilitation programmes do not appreciate the importance of contextual factors. / Conclusion. Workplace work hardening programmes are a further development of work rehabilitation programmes. The therapeutic use of actual work facilities and the work environment can effectively facilitate the successful return to work process of the injured worker. More importantly, many of the psychosocial problems associated with separation from the work routine, peer group and/or the employer are minimised by the presence of the Job Coach. The results of this study confirm that workplace-based rehabilitation intervention is more effective than conventional clinic-based rehabilitation programmes in terms of prevention of further work disability, improvement in functional capabilities and decrease in perceived pain and disability. / Keywords. Job coach, workplace-based rehabilitation, rotator cuff injury, work disability, return to work intervention. / Methodology. A randomised controlled trial was conducted on 94 workers recruited from Workers' Compensation insurers. These workers had all sustained occupational rotator cuff injury and had lost more than 90 workdays. The workers were randomly assigned into control or experimental groups. The control group received a traditional work hardening programme and the experimental group received a workplace work hardening programme using the Job Coach model. The return to work outcomes of the two groups were compared. Areas of comparison included return to work rate after training, job retention ability and impact on earning capacity. Other outcome measures included change in active range of motion of the shoulder joint, eight basic functional capacities and the worker's perception of shoulder pain and disability based on the Shoulder Pain and Disability Index (SPADI). / Results. After one-month of the training programme, a higher return to work rate was obtained in the experimental group compared to the control group (71.4% against 37%, chi2=11.095, p=0.001). For job retention ability, 93.5% of the workers in the experimental group were still at work compared to 72.9% of workers in the control group (chi 2=7.031, p=0.008). No obvious salary change was noted between the two groups. / The SPADI was statistically significantly lower in the experimental group than in the control group (p=0.032), meaning that workers in the experimental group had fewer shoulder problems after training.
