Posouzení vlivu konstituční hypermobility na posturální stabilitu / Assessing the influence of constitutional hypermobility on postural stability

The thesis "Judging the influence of constitutional hypermobility on postural stability" focuses on the problem of constitutional hypermobility (CH) and the influence of joint instability on stability mechanisms. The aim of the research was to find out whether there is a difference in stability mechanisms with tested subjects with CH and without it. Furthermore, the research focuses on the influence of mobility techniques on postural stability by the means of chosen tests carried out on Balance Master® System platform. The tested sample includes 14 women aged from 20 to 28, 7 women with constitutional hypermobility and 7 women as a control group. 6 tests were chosen for the measurement on Balance Master® platform. 2 static tests: modified Clinical Test of Sensory Interaction on Balance (mCTSIB), Unilateral Stance (US), and 4 dynamic tests: Limits of Stability (LOS), Rhytmic weight shift (RWS), Step Up Over (SUO) a Forward Lunge (FL). We compared the medians of output values of individual tests. It was assumed that subjects with KH would have significantly worse test results than the control group. Furthermore, it was supposed that the mobilization processes cause stability disturbance on the basis of cancellation of compensational , thus protective block in comparison with KS. Abstract Severe hearing loss is standardly treated by cochlear implantation. Considering the anatomical proximity and interconnection to the vestibulocochlear nerve, it is possible to influence vestibular functions. The aim of this diploma thesis is to evaluate the effect of unilateral cochlear implantation on postural stability and vestibular function in adult patients with severe hearing loss caused postlingually. Eleven patients aged 28-72 were examined the day before surgery, the day after surgery, and on average 25 days after surgery. The evaluation consisted of short-form Dizziness Handicap Inventory (DHI), Timed Up and Go testing (TUG), static stabilometry and the perception of the subjective visual vertical (SVV) static and dynamic. Significant deterioration of TUG and SVV was recorded particularly immediately after surgery. In dynamic SVV, especially, when the field of view rotated to the left. Title: Strategy of postural stabilisation using unstable surface and water barrel. Objectives: The aim of this study is to investigate the level of muscle actiavtion of choosen muscles during lunge on unstable surface or with using water barrel. Investigation of the postural strategy used during lunge orn unstable surface or with the water barrel and creation of methodology for measurment and data analysis. Methods: Into this pilot study, there were picked 5 people (athletes). Data for outcomes where used from 3 athletes. The measurement of level of muscle activation were done by surface electromyography over gluteus medius muscle and musculi multifidii bilaterally.For data procession was used software Origin 2012 Postural stability was measured through force plates by Kistler and gained data were procesed by using software programmes Bioware, MS Excel and Matlab. For data analysis from EMG measurement was used simple comparasion of outcomes. Stabilometry outcomes were analysed by statistical methode t-test. Results: The results indicate greater level of muscle activation during lunge with aquabag than lunge on unstable surface for most of the measured muscles. Influence de l’avancement de la mandibule sur la posture générale : étude stabilométrique et compléments électromyographiques

Les orthèses de propulsion mandibulaire sont fréquemment utilisées en orthopédie-dento-faciale pour stimuler le développement de ce maxillaire. Le déplacement antérieur de son centre de gravité et les tensions musculaires et viscérales (appareil aéro-digestif) qui apparaissent alors sont susceptibles d’influer sur la posture du sujet debout. Pour le déterminer, une étude stabilométrique a été menée. Elle permet de mettre en évidence les variations de position de la projection au sol du centre de gravité du corps (PGv). Les effets du port de trois orthèses imposant une quantité de propulsion différente ont été analysés, comparés entre eux et à la situation sans orthèse portée. Ces résultats ont été complétés par ceux issus d’une étude électromyographique, menée pour détecter les muscles mis en jeu dans le contrôle de l'équilibre. L’ensemble de ces expérimentations montre principalement une perte du contrôle postural (augmentation des oscillations de PGv) et un repositionnement plus postérieur du centre de gravité corporel lors d’avancement mandibulaire important, et ce d’autant plus que l’orthèse portée provoque peu de désocclusion inter-maxillaire. Une activité accrue des muscles fléchisseurs dorsaux du pied, de la hanche, du cou et extenseurs du rachis lombaire accompagne ces variations de position de PGv. Une hypothèse d'adaptation posturale en réaction à l'avancement de la mandibule est formulée ainsi qu’une critique et une évolution du protocole d’étude. / Mandibular propulsion splints are frequently used in dento-facial orthopedics so as to stimulate the development of this maxilla. Consequently, we can notice that its centre of gravity moves forward but also that visceral (the aerodigestive apparatus) and muscle tensions appear. These elements are likely to have an influence on the position of a person standing. A stabilometric study was carried out to highlight the changes in the projection onto the ground of the body centre of gravity (PGv). We analysed and compared the effects of the wearing of three different splints (each of which required a different level of propulsion). But a comparison between these results and the normal situation (no splint worn) was also drawn. Moreover, an electromyographic study was carried out to find out which muscles were stimulated in the balance control. Its results were then added to those of the stabilometric study. These experiments mainly show that we lose control of the postural equilibrium (increase of the PGv oscillations). It also shows that when the mandible is significantly moved forward, the body center of gravity goes back into a more posterior position. This is all the more true as the splint worn does not cause much inter-maxillar disocclusion. The back flexor muscle of the foot, of the hip, of the neck and the extensor muscle of the lumbar rachis are greatly stimulated in the variations of the PGv. A postural adaptation to the mandibular advancement is hypothetically expressed, together with a criticism and an evolution of the study procedure.

Title: Postural stabilization and balance - theoretical background and definitions (literature review) Objectives: The aim of this theses is to investigate the present situation of theoretical background of postural stabilization and balance and find out the differences between both of these phenomenons. The theses should summarize the current literature including the newest studies. Furthermore, it discusses the most used computerized measurements methods for assessing balance and postural control. Methods: The theses has character of literature review. It is divided into several parts. The beginning introduces the theoretical background. The descriptive and analytical chapter discuss the most used computerized measurement methods for assessing balance and postural control. The discussion summarizes the current situation and results of the theses. Results: The total amount of the 104 studies was found, but only 12 of them met the criteria of the theses. Currently the most used methods for assessing balance and postural control are the computerized dynamic posturography and the visual feedback posturography.
