Healthcare Professionals as Study Participants: A Scoping Review

Class of 2017 Abstract / Objectives: To conduct a scoping review of studies involving healthcare professionals as study subjects and to describe the methods used, identify the topics researched, and describe the rationale and limitations of using healthcare professionals as subjects. Methods: The study was a scoping review of research utilizing health professionals as study subjects. A comprehensive literature search was conducted in several databases. Two investigators independently screened studies, collected data, and met to resolve discrepancies. Results: Sixty-five studies met the eligibility criteria. Forty-six percent of the studies evaluated cardiovascular events, 25% evaluated cancer, 9% examined ophthalmic events, 5% examined cognitive issues, and 17% miscellaneous topics. Of the 65 studies, 88% were prospective cohort studies. Questionnaires were utilized as the data collection method in 59 studies (91%). Physicians were the primary study subject in 30 studies (46%) and nurses in 20 studies (31%). No study included in the sample identified pharmacists as study subjects. A total of 41 studies (63%) did not list rationales or limitations to utilizing health professionals as subjects. Of the 24 studies that did discuss rationales and limitations, the most frequently cited advantage was reliable self-reporting (38%). The most common limitation to generalizability was high socioeconomic status. Conclusions: Questionnaires were the most common method used to collect data. Physicians were the most often studied health professional. High reliability of data reporting was a common rationale in using health professionals as subjects. The lack of studies utilizing pharmacists as subjects demonstrates an opportunity that should be further evaluated.

The role of mechanical loading in osteoarthritis of the knee

Medial osteoarthritis (OA) and lateral OA have distinct characteristic cartilage lesion locations and knee flexion angles associated with lesion development. These types of OA are suggested to be caused by loading when the knee is in extension and mid-range flexion, respectively. This project used subject-specific finite element (FE) models to investigate contact conditions within the extended and flexed knee. A method of creating subject-specific FE models by combining geometry (derived from magnetic resonance imaging scans) and load cases (calculated from motion analysis data) collected from the same subject was developed. This model creation method was validated by comparing experimentally-measured pressure data to contact data calculated by FE models. Models of normal knees in three subjects were created first. Models with larger subject-specific loads had larger displacements and higher stresses and contact pressures. Contact occurred over most of the articulating cartilage surfaces, both in areas of typical cartilage lesions and outside areas of typical cartilage lesions. Parameters in the normal models were then altered to reflect three mechanical changes hypothesized to lead to OA: increased loading, globally decreased cartilage stiffness, and locally decreased cartilage stiffness. Increased loading led to increased displacements, stresses, and contact pressures. Contact shifted anteriorly in the extended knee models to locations of typical medial OA cartilage lesions; contact remained stationary with elevated stress magnitudes in the flexed knee models. Globally decreasing cartilage stiffness had limited effects on contact results. Locally decreased cartilage stiffness led to locally increased displacement and strain and locally decreased stress and contact pressure. Contact again shifted anteriorly in the extended knee models. Potential mechanisms of OA initiation were then proposed. Increased weight or locally decreased cartilage stiffness increased strains within the cartilage. High strains can damage the cartilage matrix fibres, further decreasing cartilage stiffness and eventually leading to cartilage lesions and OA.

Ce mémoire a pour objectif de comprendre la situation de l’emploi et le salaire pour les diplômés universitaires en Chine à l’entrée du marché de travail pendant les dernières années. Après la revue de littérature, nous avons supposé trois hypothèses sur la compétence personnelle basée sur la théorie du capital humain. Dans cette recherche, les données retenues proviennent de deux sources : (1) les canaux officiels, y inclus les rapports diffusés et les statistiques publiées par le ministère de l’Éducation ou le Bureau national des Statistiques en Chine. (2) les canaux non gouvernementaux, soit les institutions d’enquêtes indépendantes et les organisations académiques en Chine. Nos résultats d’observation nous permettent de constater que les diplômés chinois sont exposés à une vive concurrence dans un marché où l’offre de travail y est excédentaire. Les caractéristiques personnelles, la qualité de l’enseignement, le niveau de scolarité et la discipline d’étude, peuvent affecter dans une certaine mesure l’emploi et le salaire de départ. Finalement, nous concluons ce mémoire avec des propositions de pistes de solution aux caractéristiques chinoises qui visent à réduire la difficulté d’emplois des diplômés. / The objective of current study is to understand the situation of employment and the entrance level salary for graduates in China of recent years. According to the literature review, we assumed three hypotheses regarding individual competence based on the theory of human capital. The data in this research came from two sources: (1) the official channels, including reports and statistics published by the Ministry of Education or the National Statistics Bureau in China. (2) non-governmental channels, including investigation and research work by the institutions independent in China. Our analysis showed that Chinese graduates were exposed in a oversupply labor market. However, some personal character in academic background, such as the teaching quality, education level and study subject, may have an impact on employment and entrance level salary to some degree. In the end we draw a conclusion with Chinese-style solution, which aims at reducing the problem of unemployment.
