Estudo das talassemias do tipo alfa em dois estados das regiões nordeste e sudeste do Brasil /

No Brasil, as talassemias alfa tem sido observadas em alta freqüência nos grupos analisados. A expressão fenotípica variável desta alteração dificulta seu diagnóstico laboratorial e clínico. Em virtude da dificuldade diagnóstica e da elevada freqüência das talassemias alfa; analisou-se amostras de indivíduos adultos de dois estados do Brasil, com o objetivo de caracteriza-as laboratorialmente por procedimentos de rotina, incluindo HPLC, e biologia molecular. Todas a amostras apresentaram resultados laboratoriais sugestivos de talassemia alfa, sendo identificados molecularmente 50 (14%) heterozigotos e 11 (3%) homozigotos para a deleção -α3.7. Αs freqüências genotípicas não apresentaram diferencias nos grupos analisados. Não observou-se correlação entre os resultados obtidos na triagem e os da análise molecular. O nível médio da Hb A2 foi significativamente menor no grupo homozigoto quando comparado aos grupos normal e heterozigoto. Porem, não foram observadas diferenças nos níveis de Hb F entre os grupos genotípicos. Os valores médios de HCM e VCM foram significativamente diferentes entre os grupos genótipos. Os resultadas deste trabalho confirmam a freqüência da talassemia alfa pela deleção -α3.7 e, evidenciam a dificuldade diagnostica desta alteração. A eletroforese em acetato de celulose e, a pesquisa de corpúsculo de inclusão de Hb H, não foram métodos eficientes na identificação das formas leves de talassemias alfa. No entanto, as diferencias observadas para os níveis de Hb A2,VCM e HCM, podem auxiliar o diagnostico desta alteração. / Abstract: The alpha talassemias consist one of the most prevalent disturbances of hemoglobins in the world. In Brazil, the alpha talassemias have been observed in high frequency in the analyzed groups. The variable phenotypic expression of this alteration difficult its laboratorial and clinical diagnosis. Because of the diagnostic difficulty and of the high frequency of the alpha talassemias, it was analyzed samples of adult individuals of two states of Brazil, with the goal to laboratory caracterize them for routine procedures, including HPLC, and molecular biology. The samples presented suggestive laboratory results of alpha talassemia, being identified molecularly 50 (14%) heterozygotes and 11 (3%) homozygotes for the deletion -α3.7. The genotypic frequencies didn't present differences in the analyzed groups. Correlation was not observed among the results obtained in the routine screening and in the molecular analysis. Hb A2 medium level was significantly smaller in the homozygote group when compared to the normal and heterozygotes groups. However, differences were not observed in Hb F levels among the genotypic groups. The medium values of HCM and VCM were significantly different among the genotypes groups. The results of this work confirm the high frequency of the alpha talassemie for the deletion -α3.7 and, make evidence the diagnoses difficulty of this alteration. The eletroforese in cellulose acetate and, the Hb H inclusion body study, were not efficient methods in the identification of the light forms of alpha talassemies. However, the observed difference for Hb A2, VCM and HCM levels can aid the diagnose of this alteration. / Mestre
