The effectiveness of the referral system in primary health care in the West Rand region : a normative-ethical study with special emphasis on traditional healers

Thesis (M.Phil.)--Stellenbosch University, 2000. / ENGLISH ABSTRACT: The aim of this research is to identify the various levels of health care units, their relationships and the problems hindering an effective referral system. To achieve this goal, use is made of a case study of the West Rand area in Gauteng. The standpoint is that, to achieve Primary Health for all South Africans referral systems within health care units and levels must be reciprocal. It is argued that for Primary Health Care to be successful, it must satisfy the goal of affordability appropriateness and accessibility. Results from the research revealed that four health care levels, namely traditional healers, health NGOs, Clinics, and Hospitals. Though there is some degree of referral in the study area, it was observed that referrals in the study area were not reciprocal. Amongst the major problems identified as hindering an effective referral system in the study area, include lack of cooperation between health institutions, poor health infrastructure and communication network as well as lack of other health paraphernalia. The research also found that government policy towards some of the health institutions (Traditional healers) contributes to the inefficiency of proper referrals in the study area. / AFRIKAANSE OPSOMMING: Die oogmerk van hierdie ondersoek is om die verskillende vlakke van gesondheidsorgeenhede, hulonderlinge verbande en die probleme wat doeltreffende verwysings in die wiele ry, te identifiseer. Dit word gedoen aan die hand van 'n gevallestudie van die Wes-Randarea in Gauteng. Die uitgangspunt is dat doeltreffende Primêre Gesondheid vir alle Suid- Afrikaners afhang van resiprokale verwysingsisteme tussen gesondheidsorgeenhede en -vlakke. Suksesvolle Primêre Gesondheidsorg vereis bekostigbaarheid, toepaslikheid en toeganklikheid. Die ondersoek het vier gesondheidsorgvlakke aan die lig gebring: tradisionele genesers, gesondheids-nie-regerings-organisasies, klinieke en hospitale. Hoewel daar 'n mate van onderfinge verwysing in die studie-area bestaan, was dit nie wederkerig nie. Onder die vernaamste struikelblokke vir 'n doeltreffende verwysingsisteem tel swak samewerking tussen gesondheidsinstellings, gebrekkige gesondheidsinfrastruktuur en kommunikasienetwerk, en 'n skaarste aan ander gesondheidsmiddelle. Die ondersoek het ook bevind dat regeringsbeleid aangaande sommige van die gesondheidsinstellings (tradisionele genesers) bydra tot die ondoeltreffendheid van verwysings in die studie-area.

Knowledge and use of traditional medicinal plants by the Setswana-speaking community of Kimberley, Northern Cape of South Africa

Thesis (MSc)--University of Stellenbosch, 2007. / ENGLISH ABSTRACT: The majority of South Africans still depend on the use of traditional remedies, as these are sometimes the only types of health care systems available, especially within rural communities. South Africa comprises approximately 400 000 traditional healers and an estimated 60 to 80% of individuals consulting such traditional healers. As a result, the over-harvesting of many traditional medicinal plants has become a threat to the country’s species diversity and has resulted in the scarcity of certain medicinal plant species. The non-sustainable use of traditional medicinal plants stems from their intense harvesting from the wild to supply the high demands from urban and rural markets. As a result of the escalating population growth rate; high rural unemployment; and fundamental value attached to traditional medicinal plants (socio-economic factors), the national and regional trade of traditional medicines is currently higher than it has ever been. Another reason for the increased threat to traditional medicinal plants is the degradation and weakening of customary laws that have previously regulated such resources. This study focuses on the use of traditional medicinal plants by the Setswana-speaking community for self-medication and as a form of primary health care. Research was conducted in Kimberley, Northern Cape of South Africa and focuses on the issue of the sustainability of medicinal plant use in the area, specifically on use and users as well as the acquisition of material sold by a single trader and harvesting techniques. This is to determine whether harvesting of medicinal plants is a potential threat to plant communities in the area. To address the shortcomings of medicinal anthropology the study also investigates the impacts of relocation and resettlement of various communities in the area, on plant use, methods of collection, the sustainability of the natural resource, as well as the transmission of Setswana indigenous knowledge inter-generationally. most abundant under high disturbances. Certain species reacted positively to disturbance and were most abundant in disturbed habitats. These included Elephantorrhiza elephantina and a Helichrysum sp. To minimise destructive harvesting in the Kimberley area and to ensure the sustainable harvesting of plant material, it is important that local harvesters are educated on proper harvesting techniques and that local gatherers are educated on sustainability issues as well as other ecologically fundamental issues. / AFRIKAANSE OPSOMMING: Die meeste Suid-Afrikaners is steeds afhanklik van tradisionele geneesmiddels aangesien dit soms, veral in landelike gemeenskappe, die enigste beskikbare gesondheidsorg is. Suid-Afrika het sowat 400 000 tradisionele geneeshere wat deur ’n geraamde 60% tot 80% van individue geraadpleeg word. As gevolg hiervan hou die oorontginning van talle tradisionele medisinale-planthulpbronne ’n bedreiging vir die land se spesiediversiteit in en het dit reeds tot ’n skaarste aan sekere medisinale plante gelei. Tradisionele medisinale plante word tans nievolhoubaar aangewend aangesien dit op groot skaal in die veld geoes word om in die groot vraag van stedelike en landelike markte te voorsien. As gevolg van die stygende bevolkingsgroeikoers, hoë landelike werkloosheidsyfer en die grondliggende waarde wat aan tradisionele medisinale plante geheg word (sosio-ekonomiese faktore), is die nasionale en streekhandel in tradisionele geneesmiddels tans groter as ooit tevore. Pharmacological, phytochemical and safety evaluaton of commercial herbal preparations common in South Africa.

Herbal formulations claimed to cure several medical conditions including skin eruptions, chest pains, wounds, gout, menstrual pains, stress, nervous disorders, microbial and viral infections as well as stomach ailments have recently appeared as part of South African traditional medicine. The formulations consist of mixtures of extracts of different plant parts from several different plant species packaged in labelled bottles or boxes. The mixtures are available for sale in herbal shops and public places. While there has been widespread use of these herbal mixtures, there has been no scientific evidence to support their use. This project was aimed at documenting, validating claimed efficacy and testing the safety of fourteen unregistered commercial herbal preparations commonly sold in Pietermaritzburg. Heavy metals in South African medicinal plants.

Plants are able to take up and accumulate certain environmental contaminants such as heavy metals. When the plants are ingested by man, these contaminants are transferred along the food chain. Due to the poorly regulated medicinal plant trade in South Africa, many opportunities exist for heavy metal contamination of medicinal plants namely contaminated harvest sites as well as poor drying, processing, storage, transport and manufacturing conditions. The concentrations of five heavy metals (As, Cd, Co, Ni, Pb) and six microelements (B, Cu, Fe, Mn, Mo, Zn) were determined in some commonly used South African medicinal plants obtained from street markets. Elemental content was determined using inductively coupled plasma optical emission spectrophotometry (ICP-OES). Some of the medicinal plant samples investigated contained As and Cd at levels exceeding the World Health Organization limits of 1 and 0.3 mg kg-1 respectively. Lead and Ni were detected in all the samples. Elevated Fe and Mn levels were recorded in certain plant species. The results revealed multiple metal contamination in some medicinal plant parts sold in local markets and is thus grounds for concern. The effects of Cd application on growth parameters of some medicinal plant species belonging to the Hyacinthaceae (Albuca setosa, Eucomis autumnalis, Eucomis humilis, Merwilla plumbea) gave insight into heavy metal accumulation and distribution in these species. Application of Cd at 5 mg l-1 over a 12 week period reduced growth in A. setosa. The medicinally used A. setosa bulbs accumulated 37 mg kg-1 Cd after 12 weeks. Cadmium application at 2 mg l-1 over a six week period had no effect on growth parameters of E. autumnalis or E. humilis. However, a substantial difference in total Cd accumulation was detected in the plants (40.2 and 15.3 mg kg-1 respectively). Cadmium application at 2 mg l-1 significantly reduced the fresh weight of leaves, bulbs and roots of M. plumbea. Although most of the Cd was stored in the roots, the medicinally used bulbs accumulated up to 11.6 mg kg-1 when applied at 10 mg l-1. The antagonistic effect between Cd and Zn treatments and their effect on micronutrient distribution in M. plumbea were investigated. Five treatments were evaluated: (1) Hoagland’s nutrient solution (HS) (control) (2) HS + Cd 2 mg l-1 (single) (3) HS + Cd 2 mg l-1 + Zn 50 mg l-1 (combination) (4) HS + Cd 2 mg l-1 + Zn 100 mg l-1 (combination) (5) HS + Cd 2 mg l-1 + Zn 150 mg l-1 (combination). Cadmium readily accumulated in leaves, bulbs and roots of M. plumbea when supplied at 2 mg l-1. Zinc at 50 mg l-1 led to increased Cd accumulation. However, further increases in Zn concentration showed an antagonistic effect of Zn on Cd uptake and accumulation. Thus, increasing Zn levels in soils may be favourable for reducing toxic Cd accumulation in M. plumbea plants. Boron was not significantly affected by the addition of Cd to the media. However, with an increase in Zn, leaf B content increased while the B content in the bulbs and roots decreased. Copper and Mo levels were not significantly affected by treatments with Cd or Cd/Zn combinations. Compared to the control, Cd and Cd/Zn applications caused an increase in Mn content in leaves, bulbs and roots. Iron levels of M. plumbea were not significantly affected by Cd in the media. However, with an increase of Zn in the Cd-containing media, Fe content in the leaves, bulbs and roots increased. Tulbaghia violacea is one of the few medicinal plants that is also frequently used as a leafy vegetable. Application of Cd at 2 and 5 mg l-1 to T. violacea of varying sizes (small 8 - 10 g, medium 16 - 20 g, large 80 – 95 g) elicited a difference in growth response, Cd accumulation and micronutrient distribution. Leaf length and fresh weight of leaves of the medium-size plants decreased with application of Cd at 2 mg l-1 whilst 5 mg l-1 Cd significantly decreased the number of leaves in small-sized plants. Small plants accumulated more Cd in the leaves than medium- or large-sized plants. Application of Cd at 2 mg l-1 and 5 mg l-1 lowered the leaf Cu, Fe, Mo and Zn contents in small- and medium-size plants. This study indicated that T. violacea has the ability to accumulate Cd. In addition, plant size plays an important role with regards to Cd accumulation and elemental distribution. The effect of various nutrient applications (10%, 50% and 100% Hoagland’s nutrient solutions (HS); and HS deficient in N, P or K) on growth parameters and micronutrient distribution in Dioscorea dregeana were investigated. Irrigating plants with 50% HS resulted in better growth performance, whereas a deficiency of either N, P or K negatively affected seedling growth. Plants grown in 10% HS contained higher total B, Fe and Mo levels compared to seedlings grown in 50% and 100% HS. Compared to the control, P deficiency resulted in a Fe increase in the leaves, tuber and roots while a lack of P and K significantly increased total Mn content in D. dregeana. The effect of excess Zn (100, 200 and 300 mg l-1) on growth performance, chlorophyll content and microelemental distribution on Dioscorea sylvatica was investigated. Growth parameters showed a significant decrease when supplied with Zn at 100 mg l-1. Zinc phytotoxicity was evident by the reduction in chlorophyll content. Highest Zn concentrations were detected in the roots. Certain micronutrients appear to be redistributed due to Zn toxicity. The effect of microelements (Cu, Zn) and heavy metals (Cd, Pb, Hg) on germination and seedling development of Bowiea volubilis, Eucomis autumnalis and Merwilla plumbea was investigated. Copper and Zn applied at 1 mg l.1 significantly reduced the percentage germination of E. autumnalis. Low concentrations (. 1 mg l.1) of Cu and Zn negatively affected the root growth of all three species. Mercury concentrations of 0.5 and 1 mg l.1 significantly decreased the percentage germination of B. volubilis and E. autumnalis respectively. Cadmium and Hg at 2 mg l.1 showed a negative effect on the root growth of B. volubilis. Concentrations of 0.5 mg l.1 of all heavy metals tested significantly decreased shoot length of M. plumbea. The effect of Cd on biological activity (anti-inflammatory, antibacterial and antifungal) of medicinal plants with previously confirmed activity was evaluated. When supplied with Cd at 2 mg l-1, Eucomis humilis bulbous extracts showed lower anti-inflammatory activity than the control for both COX-1 and COX-2 activity. Eucomis autumnalis bulbous extracts had greater COX-1 activity compared to the control. However, Cd suppressed the activity of COX-2. Compared with non-Cd-treated Merwilla plumbea plants (control), those supplied with Cd at 10 mg l-1 showed increased antibacterial activity against Bacillus subtilis, Klebsiella pneumoniae and Staphylococcus aureus. However, no change in activity against Escherichia coli was observed. Cadmium accumulation in the bulbs had no effect on antifungal activity of Tulbaghia violacea. Thus, optimized agricultural practices are essential for quality control of cultivated medicinal plants. The studies presented in this thesis collectively answer several questions related to heavy metal involvement in South African medicinal plants. The influences of traditional medicine in relation to its various use by the African societies : a review of Zulu novels.

Traditional medicine, unlike western medicine, is not merely concerned with physical illness, but it is used for various purposes. For instance: It can be used for lkuthwala' (the process whereby a person consults a traditional doctor for the medicine that will make a person very rich). The practice of 'ukuthwala' has numerous disavantages, especially because of the price that is paid in return of the wealth accumulated. The price is usually a human sacrifice, depending on what version of Ukuthwala' a person has opted for. Traditional medicine can also be used for witchcraft (ukuthakatha),for protection against evil spirits (ukuqinisa) , for making someone love you, for job opportunities, and for inspiration of the army. It can be either used for good or evil purposes. The dissertation looks at both versions by strongly drawing examples from Zulu novels. Other issues raised in this study is the importance of religion and cosmology, culture, magic, as well as spiritual healing, in association with traditional medicine. The Africans believe in the spirit world. They believed that for people to communicate with 'Mvelinqangi ' (God) there should be 'amadlozi' (the ancestors), who should intercede with God on their behalf. Usually, they call a sangoma' (medium) or 'inyanga' (medicine-man) to perform the religious ritual, or he would just instruct the elder person in the family how to carry out the procedure of communicating with the ancestors (Canonici, 1996). Traditional healers are one among professionals that are viewed as the most significant people in African Society. They are accorded the highest esteem because of the role they play in saving people’s lives and promoting cultural aspects such as ukuhlonipha amasiko in isiZulu. It should be remembered that this word ukuhlonipha which means to respect, is used across cultures to maintain peace within family and society. It all depends on which culture promotes it more than others. It is sociolinguistic truism that ukuhlonipha is essentially linked to the ubuntu philosophy of African people. However, traditional healers more than other people follow ukuhlonipha culture in numerous ways, especially when they meet, being consulted and during traditional ceremonies. They serve and promote what in IsiZulu is known as ukuhlonipha, which to the uninitiated and sometimes even to the scholars of this philosophy is a mystery. It is for this reason and others that people view them as the soul of society. Development of conservation methods for gunnera perpensa l.: an overexploited medicinal plant in the Eastern Cape, South Africa

South Africa, many plants which are used in traditional medicines are collected from wild populations. The high demand for trade and use of these medicinal plants place an enormous pressure on their natural populations, especially because they are indiscriminately harvested. The most affected of these plant species are those harvested from their underground parts, among which is Gunnera perpensa L. Gunnera perpensa is of considerable ethnobotanical interest in traditional medicine because of its wide usage. The rhizomes are widely used and indiscriminately collected in large quantities from the wild to meet the ever increasing demand in traditional medicine markets. As a result, this valuable medicinal plant species is being endangered. According to the Red List of South African Plants, the conservation status of G. perpensa has been listed as ‘declining’. The ethnobotanical survey conducted as part of this research confirms the plant species as threatened. It is, therefore, important to develop propagation and conservation strategies for this medicinal plant. Clonal propagation of G. perpensa was conducted using varying lengths of the rhizome (1, 2, 3, 4 and 5 cm) segments as propagules. While regeneration was possible with all the rhizome lengths, most of the growth parameters were significantly higher in the 5 cm rhizomes than the other rhizome segments. The appropriate planting depth for the rhizomes was also determined and 4 or 5 cm planting depths were found appropriate. No significant difference was observed in the growth parameters amongst the planting depths; nevertheless, 4 cm depth gave higher growth and yield. The results of this study show that regenerating G. perpensa.

An assessment of African traditional medicines in pregnancy and on birth outcomes: pharmacists' perceptions of complementary medicines in pregnancy

Increasing numbers of medicines are being used by pregnant South African women in the public sector during pregnancy, for the treatment of different biomedical and supernatural disease states and conditions. The motivation for the research is to support the development of more local pregnancy registries in order to strengthen evidence for the safety and efficacy of medicines used in pregnancy. A mixed methods approach was used. Women in their ninth month of pregnancy in a public sector setting, and four community pharmacists were identified. The women who met the inclusion criteria were recruited. One in-depth semi-structured interview was conducted with each woman before giving birth and data on their pregnancy outcomes were collected after labour. Coincidentally, the mother of one of the participants was found to be a traditional healer. She was also interviewed on the topic. A structured questionnaire was administered to the pharmacists. Ten pregnant women between the ages of 19 to 39 who had used or were using a traditional medicine during the pregnancy were recruited. All the participants had had at least one antenatal check up during their pregnancy with one having attended five times. No abnormal results were reported from any of the check ups or tests done during the visits. All of them had been to school and had at least Standard 8/Grade 10 education. Ten babies were seen between one and four days postpartum and no birth defects were obvious or were reported for any of them. The traditional healer did not provide additional information to what the women had said and confirmed that some of the practices the women reported were known to her as traditional medicine practices. All four pharmacists indicated that they considered complementary and alternative medicines (CAMs) to be “somewhat effective” and sold them at their pharmacies although none of them were aware of whether or not they were registered with the MCC. None of the pharmacists appeared to have an in-depth knowledge of traditional, complementary and alternative medicines (TCAMs). All four pharmacists said that it is important to have a basic understanding of TCAMs before using them, although they did not agree on the reasons for this. All of them felt that pharmacists have a professional responsibility to provide information on TCAMs (especially herbal preparations) and two felt that providing this information is part of a medical doctors’ responsibility. No harm from taking TCAMs could be shown. However herbal medicines have numerous ingredients some of which are unknown and taking these medicines is risky. The pharmacists in this sample were unsure whether they were accessing unreliable CAM information. Reliable sources of information and reference materials on CAMs to assist pharmacists and other healthcare professionals are needed. The apparent widespread use of TCAM in pregnancy indicates a need for documentation about its efficacy and safety. The establishing of TCAM pregnancy registries should seriously be considered. Due to the increase in CAM use, CAM education during pharmacists’ training as well as continuing professional development (CPD) in CAM for pharmacists in practice should be encouraged.

The efficacy and toxicological effects of aloe ferox mill. used in the management of constipation in Nkonkobe municipality of the Eastern Cape province, South Africa

Herbal remedies are commonly used in developing countries for the treatment of various diseases, including constipation. The rationale for utilizing medicinal plants for the treatment of diseases rested largely on the belief that they are safe and free of side effects. However, there is limited scientific evidence on the safety and efficacy of these herbal medicines to back up their continued therapeutic application. Aloe ferox Mill. (Aspodelaceae), known as Cape aloe, locally called ikhala is a medicinal plant used by the people of the Eastern Cape Province for the treatment of gastrointestinal problems and constipation. The plant is a perennial shrub with thick succulent leaves bearing brown thorns on the margin and bright orange flowers arranged in oval lanceolate. It occurs in all weather in bush veld, road side, gardens and undisturbed places. According to the ethnomedicinal information, A. ferox is used as purgative. This research project was therefore designed to evaluate its ability in the treatment of constipation and to investigate its possible toxicological property. At the beginning of this programme, a survey of plants used for the treatment of constipation in Nkonkobe Municipality of the Eastern Cape Province was carried out using a questionnaire, which was administered to herbalists, traditional healers and rural dwellers. The study revealed 10 plant species from 8 families that are used for the treatment of constipation in the province. Four plants, Aloe ferox Mill, Boophone distischa L.f Herb, Alepidea amatybica Eckl and Artemisia afra Jacq, were repeatedly mentioned. Based on the frequency of usage, perceived efficacy and availability to the rural dwellers and the traditional healers, Aloe ferox was the most commonly used of the plants for the treatment of constipation. The plant was thus, chosen for the study. The invivo laxative effect of the aqueous extract of Aloe ferox in the treatment of loperamide-induced constipation in Wistar rats was investigated at varying concentrations. The leaf extract at all the dosages investigated (50, 100 and 200 mg/kg body weight) improved intestinal motility, increased fecal volume and normalized body weight in the constipated rats. This was an indication of its laxative properties. However, the laxative property of the herb at 200 mg/kg body weight of the extract showed best efficacy and compares favourably well with senokot, a standard laxative drug. These findings have therefore, lent scientific credence to the folkloric use of the herb by the people of the Eastern Cape of South Africa as a laxative agent. Toxicological evaluation of aqueous leaf extract of Aloe ferox in loperamide-induced constipation was studied at 50, 100, and 200 mg/kg body weight. The oral administration of the extracts did not show any significant effect on the liver and kidney body weight ratios as well as the kidney and liver function indices. The extracts, at all the dosages investigated, did not alter the levels of creatinine, uric acid, urea, calcium and potassium ions. Similarly, the levels of total protein, albumin, bilirubin and gamma glutamyl transferase (GGT) were not significantly different from the control. The plant extract appreciably normalized the elevated activities of alkaline phosphatase (ALP), alanine transaminase (ALT) and aspartate transaminase (AST) in the untreated constipated rats following treatment with the extract. The extract did not show a significant effect on the hematological parameters except for the increase in the lymphocyte count in the untreated constipated rats, which was attenuated after administering the herb. ThThe available evidence in this study suggests that A. ferox may be safe as an oral remedy for constipation. Generally, the effect of the extract compared favourably well with senokot, a recommended drug for the treatment of constipation. The antioxidant activities against 1, 1 diphenyl- 2 picrylhydrazl (DPPH), 2,2’ – azinobis [3- ethylbenzothiazoline -6- sulfonic acid] diammonium salt (ABTS), hydrogen peroxide (H2O2), Nitric oxide (NO), lipid peroxidation and the ferric reducing agents were investigated spectrophotometrically. Alkaloids, saponins, tannins, total phenols, flavonoids, flavonols and proanthocyanidin were also determined to assess their effects on the antioxidants activity of this plant. The phytochemical content of the ethanol and acetone extracts were consistently high compared to other solvents extracts. The level of tannins was not significant (P > 0.05) as compared with other solvent extracts. The free radical scavenging activity of the extracts was high even at lower concentrations (0.025 mg/ml) except in DPPH and lipid peroxidation. The ferric reducing potential of the extracts was concentration dependent and significantly different from Vitamin C and butylated hydroxytoluene (BHT) that were used as standard drugs. The present study showed a high level of scavenging activity of the leaf extracts of Aloe ferox in all the solvent extracts. Both ethanol and methanolic extract showed potent antioxidant activities than acetone and aqueous extracts. The study indicated that the leaf extracts of Aloe ferox might be a valuable source of natural antioxidant for both medicine and food industries. A. ferox leaf consists of the gel, latex and mesophyll layer; however, the main active constituents of the latex and the leaf exudate of Aloe ferox are anthraquinones which are believed to be responsible for the laxative property. The laxative compound in Aloe ferox leaf extract was isolated and characterized by extracting the plant material in methanol and extract suspended in distilled water. Partitioning was done with n-hexane, ethyl acetate and butanol respectively and was co-spotted with the over-the-counter (OTC) laxative drugs. This led to the successive column chromatography and thin layer chromatography (TLC) of the most active ethyl acetate fraction on silica gel with benzene/ethanol/ammonia hydroxide (BEA: 90:10:1), ethyl acetate/methanol/water (EMW: 40:5.4:5) and chloroform/ethyl acetate/formic acid (CEF: 50:40:10) as the mobile phase. The successive chromatograph and TLC afforded two compounds of Rf 0.420 (blue) and 0.831 (yellow) with the over-the-counter (OTC) drugs. These compounds were not totally elucidated due to their small quantity and instability. However, hydroxyl (OH) and carboxyl groups (COOH) was established as common to the extracted compounds, which might be responsible for the biological activity recorded for the plant extract.
