Avaliação do atendimento inicial ao paciente submetido à trombólise endovenosa por AVC isquêmico em um hospital terciário

Intravenous thrombolysis with alteplase, alone or later endovascular thrombectomy, is an effective treatment for acute ischemic stroke, and should be administered within up to 4.5 hours and takes into account a number of factors, including the ¿weekend effect¿. OBJECTIVE: To evaluate the parameters of initial care for patients undergoing intravenous thrombolytic therapy treated at a tertiary center. METHODS: Retrospective cohort study, conducted at the General Hospital of Fortaleza (HGF), Ceará, Brazil. We analyzed all records of Emergency Nursing and the medical records of patients who underwent thrombolytic therapy from May 2015 to April 2016. Initially, all available data were cataloged on the form and, based on this initial strategy A review of all medical records of those who possibly underwent thrombolysis was performed. RESULTS: A total of 1361 files of patients with suspected stroke were analyzed, and it was designed to analyze 201 patients who underwent thrombolysis. The mean age was 66 years, the majority of males (58%), as major comorbidities such as hypertension (73%) and diabetes (29%), regarding the classification of BAMFORD, more common for a syndrome of total circulation above (48%). The time of onset of the clinical picture in turn with an average of 143 minutes, the time of installation to the computed tomography in the media of 15 minutes and the time of the needle had an average of 51 minutes. (P <0.04) and the time of the needle-holder was shorter without daytime (49 ± 18 minutes, p <0.04). The time of onset of the clinical condition was new at night time (133 ± 52 minutes, p <0.04). CONCLUSION: The times related to thrombolysis are within the international prayers, with influence of the period of the day on them. The "weekend effect" was not found. Key words: stroke; thrombolytic therapy; weekend effect. / INTRODUÇÃO: O acidente vascular cerebral é a terceira causa de morte e incapacidade nos países desenvolvidos, sendo um dos principais problemas de saúde em todo o mundo. A trombólise intravenosa com alteplase, sozinha ou seguida de trombectomia endovascular, é um tratamento eficaz para o acidente vascular encefálico isquêmico agudo, devendo ser administrada dentro de até 4,5 horas e leva em conta uma série de fatores, dentre eles o ¿efeito de final de semana¿. OBJETIVO: Avaliar os parâmetros do atendimento inicial a pacientes submetidos à terapia trombolítica endovenosa atendidos em um centro terciário. METODOLOGIA: Estudo do tipo coorte retrospectiva, realizado no Hospital Geral de Fortaleza (HGF), Ceará, Brasil. Foram analisadas todas as fichas de acolhimento da Enfermagem da Emergência e os prontuários dos pacientes que realizaram a terapia trombolítica no período de maio de 2015 a abril de 2016. Inicialmente, foram catalogados todos os dados presentes na ficha e, a partir dessa triagem inicial, foi realizado uma revisão de todos os prontuários daqueles que possivelmente realizaram a trombólise. RESULTADOS: Foram analisadas 1361 fichas de pacientes com suspeita de acidente vascular cerebral, sendo considerados para análise os 201 pacientes que realizaram trombólise. A média de idade foi de 66 anos, a maioria do sexo masculino (58%), as principais comorbidades foram hipertensão arterial (73%) e diabetes (29%), quanto à classificação do BAMFORD, a mais comum foi a Síndrome da circulação anterior total (48%). O tempo do início do quadro clinico à chegada teve média de 143 minutos, o tempo de chegada à tomografia computadorizada foi, em média, de 15 minutos e o tempo porta-agulha apresentou média de 51 minutos. O tempo do início do quadro clinico à chegada foi menor no período noturno (133±52 minutos, p<0,04) e o tempo porta-agulha foi menor no período diurno (49±18 minutos, p<0,04). CONCLUSÃO: Os tempos relacionados à trombólise estão dentro das recomendações internacionais, havendo influência do período do dia sobre os mesmos. Não foi encontrado o ¿efeito de final de semana¿. Trombólise em pacientes com acidente vascular cerebral isquêmico agudo : aspectos epidemiológicos e funcionais na admissão e alta hospitalar

During ischemic stroke reduction of cerebral blood flow is observed, which produces ischemic zone with adjacent area of dysfunctional neuronal tissue (penumbra) potentially recoverable with thrombolytic procedure until 4,5hours of beginning of ischemic stroke. Thrombolysis is dissolution of intravascular thrombus with intravenous infusion used in acute ischemic stroke to restore cerebral blood flow. OBJECTIVE: To evaluate epidemiological aspects, motor and cognitive of patients with acute ischemic stroke who subjected thrombolysis with alterplase® at admission and discharge from the hospital. METHOD: Observational, longitudinal, analytical and quantitative realized in public united of stroke in January/August of 2012. Thrombolized patients were evaluated using neurological scales (Glasgow, Mini Mental MEEM, Modified Rankin Scale MRS and the National Institute of Health Stroke Scale HIHSS) during admission and discharge from the hospital. RESULTS: In 38 patients the age (27-28). Males (60,5%), low and middle education (71,1%). Clinical and functional data: numbness or weakness in hemi body (97,4%), difficult walking (97,4%) and speaking (92,1%) lip deviation (71,1%), aphasia (42,1%).Total anterior circulation syndrome (39,5%) and middle cerebral artery involvement (89,5%). Those patients had aphasia obtained OR=7,8 to death. Mortality (24%) and these, 78% had 70 years old or older. Average of variable in time for minutes: time window (120), needle holder (75,55) were delayed compared to validated protocols in the literature, however the image port (24,45) was the faster than average of protocols SITS-MOST. At time more than 90 minutes the needle holder associated with MEEM, observed changing cognitive (p=0,02). The remaining variables of time there wasn´t association with neurological scales. Evolution in NIHSS scale in admission and discharge (p<0.01). There was improvement in functional disability in Rankin scale admission/discharge and after 90 days of thrombolysis (p<0,01). The Mini Mental result was insignificant comparing admission and discharge. The result was repeated in the Glasgow scale. Patients were evaluated by a multidisciplinary team including doctors, nurses, physiotherapists, speech therapists and occupational therapists. CONCLUSION: The thrombolysis confirmed be effective in improve the neurological severity score and to reduce the functional disability in patients with acute ischemic stroke. The time spent by patients until 3h58 minutes to arrive the hospital from the onset of symptoms of ischemic stroke was the criterion for realizing thrombolysis. The evaluation using NIHSS, Glasgow, MEEM e MRS scales appeared relevant to recognize deficits, influencing rehabilitation treatment. The precocious full time assistance offer by multidisciplinary team contributed to a favorable functional prognosis of these patients. / INTRODUÇÃO O acidente vascular cerebral isquêmico (AVCI) é causa de incapacidade e morte de pessoas de todas as raças e etnias. Durante o AVCI observa-se redução do fluxo sanguíneo cerebral, que produz uma zona isquêmica com área adjacente de tecido neuronal disfuncional, chamada penumbra, potencialmente recuperável se administrada a trombólise dentro do tempo de até 4h30min do início do AVCI. Trombólise é a dissolução de um trombo intravascular por meio de medicamento endovenoso, utilizada na fase aguda do AVCI para restaurar o fluxo sanguíneo cerebral. OBJETIVO Avaliar aspectos epidemiológicos, motores e cognitivos de pacientes com AVC agudo, submetidos à trombólise com alteplase® na admissão e alta hospitalar. MÉTODO Observacional, longitudinal, analítico e quantitativo, realizado em Unidade de AVC pública, período janeiro/agosto de 2012. Pacientes trombolisados foram avaliados mediante utilização de escalas neurológias (Glasgow, Mini Mental MEEM, Rankin Modificada - ERM e da National Institute of Health Stroke Scale - HIHSS) durante admissão e alta hospitalar. RESULTADOS Nos 38 pacientes a idade variou de 27 a 87 anos; prevaleceram: sexo masculino (60,5%), baixa e média escolaridade (71,1%). Dados clínicos e funcionais: dormência ou fraqueza em hemicorpo (97,4%), dificuldades para andar (97,4%) e falar (92,1%), desvio labial (71,1%), afasia (42,1%). Síndrome da circulação anterior total (39,5%) e acometimento da artéria cerebral média (89,5%). Os que apresentaram afasia tiveram OR=7,8 para óbito. Mortalidade em 24%, destes, 78% tinha 70 anos ou mais. Médias das variáveis de tempo em minutos: janela de tempo (120), porta agulha (75,55), atrasadas em relação aos protocolos validados na literatura, entretanto o tempo porta- imagem (24,45) foi mais rápido se comparado à média do protocolo SITS-MOST. Observaram-se alterações cognitivas (p=0,02) no tempo porta-agulha superior a 90 minutos, associado ao MEEM,. Nas demais variáveis de tempo, não houve associação com as escalas neurológicas. Houve evolução neurológica (NIHSS) significante entre admissão e alta hospitalar (p<0,01), melhora da incapacidade funcional na escala Rankin admissão/alta e após o 90º dia da trombólise (p<0,01). Resultado insignificante no Mini Mental, comparando admissão e alta. O resultado se repetiu na escala de Glasgow. Pacientes foram avaliados pela equipe multidisciplinar: médicos, enfermeiro, fisioterapeutas, fonoaudiólogos e terapeutas ocupacionais. CONCLUSÃO. A trombólise comprovou ser eficaz em melhorar o nível de gravidade neurológica e reduzir incapacidades funcionais de pacientes com AVC isquêmico agudo. O tempo de até 3h58minutos gasto pelos pacientes para chegarem ao hospital a partir do início dos sintomas do AVCI constituiu critério para realização da trombólise. A avaliação pelas escalas NIHSS, Glasgow, MEEM e ERM mostrou-se relevante para reconhecer os défices, influenciando na escolha do tratamento de reabilitação. A assistência integral precoce dada pela equipe multidisciplinar no tratamento clínico e de reabilitação contribuiu para um prognóstico funcional favorável desses pacientes.

O impacto do uso do DTC na fase aguda do AVCI bem como suas relações com exames de neuroimagem permanecem em investigação e constituem assunto de grande relevância. OBJETIVO Este estudo pretende avaliar a correlação clínica das características hemodinâmicas ao DTC e o subtipo de AVCI segundo a classificação de Bamford; também tem objetivo de verificar a correlação entre a extensão / volume da lesão isquêmica com o perfil hemodinâmico do território arterial envolvido. OBJECTIVE This study intends to evaluate the clinical correlation of the hemodynamic characteristics to the DTC and the subtype of AVCI according to the classification of Bamford; also aims to verify the correlation between the extent / volume of the ischemic lesion and the hemodynamic profile of the arterial territory involved. As características clínicas, hemodinâmicas e o terrítorio arterial comprometido foram analisados. Os eventos vasculares foram categorizados segundo a classificação clínica de Bamford para AVC agudo e o desfecho clínico com a escala de Rankim modificada (mRs). Os exames de tomografia computadorizada (TC) obtidos durante a fase aguda foram analisados de forma qualitativa e quantitativa. A volumetria semi-automática da área isquêmica foi realizada. Em seguida, as imagens segmentadas e as TCs foram normalizadas. A normalização e segmentação da substância branca e cinzenta foi realizada de forma automática utilizando a metodologia de morfometria baseada em voxel. Duas formas de análise foram realizadas: a correlação entre as lesões e os resultados dos exames de DTC; a análise de correlação entre mapas de substância branca e cinzenta e o DTC. RESULTADOS: Foram avaliados 27 pacientes, 60% sexo masculino, média de idade 64.3 ± 12 anos, média NIHSS entrada 11,2 ± 8 (1-23), sendo que 10 (37%) receberam trombólise com alteplase, com sonotrombólise em 3 deles e 2 com trombectomia com solitaire. A janela acústica foi desfavorável para realização de DTC em 5 (18%). Dos exames realizados, 14 (51%) estavam alterados. As alterações hemodinâmicas no DTC foram observadas em todos os pacientes com a síndrome de circulação anterior total (TACS), em 45% dos com a síndrome de circulação anterior parcial (PACS) e em 75% dos com a síndrome posterior (POCS). A investigação por neuroimagem revelou que a velocidade média e o índice de pulsatilidade (IP) da artéria cerebral média podem estar correlacionados com lesões em diferentes áreas cerebrais. O IP foi o parâmetro hemodinâmico que mais se correlacionou com as alterações na neuroimagem. CONCLUSÃO Na correlação clínica com alterações hemodinâmicas no DTC, o subtipo segundo a classificação de Bamford de AVCI que mais se correlacionou foi a Síndrome de Circulação Anterior Total (TACS) (100%) seguido pela Síndrome de Circulação Anterior Parcial (PACS) (45%) . O índice de pulsatilidade foi o parâmetro hemodinâmico que mais se correlacionou com as lesões na neuroimagem. RESULTS: Twenty-seven patients were evaluated, 60% male, mean age 64.3 ± 12 years, mean NIHSS admission 11.2 ± 8 (1-23), 10 (37%) receiving intravenous thrombolysis, sonothrombolysis in 3 and 2 with Solitaire Stent Thrombectomy. The acoustic window was unfavorable for performing TCD in 5 (18%). Of the exams performed, 14 (51%) were altered. Hemodynamic changes in TCD were observed in all patients with Total Anterior Circulation Syndrome (TACS), in 45% of with partial anterior circulation syndrome (PACS), and in 75% of with posterior syndrome (POCS). Neuroimaging research has revealed that the mean velocity and pulsatility index (PI) of the middle cerebral artery may be correlated with lesions in different brain areas. The PI was the hemodynamic parameter that most correlated with changes in neuroimaging. CONCLUSION In the clinical correlation with hemodynamic changes in the TCD, the subtype according to the classification of Bamford that most correlated was the Total Anterior Circulation Syndrome (TACS) (100%) followed by the Partial Anterior Circulation Syndrome (PACS) (45%). The pulsatility index was the hemodynamic parameter that most correlated with neuroimaging lesions. The behavior of this parameter varied according to the affected region.
