Tumores de glandulas salivares menores intra-orais : analise multivariada de fatores prognosticos

C. Camargo, referentes ao período de 1954 a 1993. 128 (65,3%) eram malignos e 68 (34,7%) benignos. O palato foi o principal local de ocorrência, correspondendo a 65% dos casos. Adenoma pleomórfico foi o tumor benigno predominante, e o carcinoma mucoepidermóide o maligno mais comum. A principal modal.idade terapêutica foi a cirurgia. Recorrência local foi observada em 2 pacientes com adenoma pleomórfico e em 8 pacientes com tumor maligno. Observou-se 9 casos de metástase, sendo 4 linfonodal e 5 à distância. Quarenta e seis (76,7%) pacientes com tumor benigno e 47 (44,3%) com tumor maligno estavam vivos sem evidência de doença após o 5 anos de seguimento. As variáveis idade, estádio T, está~io N, comprometimento de linfonodos, comprometimento de pele, comprometimento ósseo e tipo de tratamento modificaram a sobrevida global. Para sobrevida livre de doença as variáveis mais importantes foram estádio N, tipo histológico, comprometimento de linfonodos e comprometimento ósseo. A análise multivariada mostrou que os fatores mais importantes para risco de óbito foram estádio N, tipo histológico, sexo e tipo de cirurgia. Para o risco de recorrência os fatores mais importantes, através da análise multivariada, foram estádio N, tipo histológico e comprometimento ósseo / Abstract: One hundred and ninety six cases of intraoral sali.vary gland tumors ftom the files of the A. C. Camargo Hospital, between 1954 and 1993, were studied. 128 (65,3%) were malignant and 68 (34,7%) benign tumors. 65% of the cases occured in the palate. Pleomorphic adenoma was the most cmmnon benign tumor, whereas mucoepidennoid carcinoma was the most common malignant tumor. Surgery was the main treatqlent. Local recurrence was observed in 2 patients with pleomorphic adenoma and in 8 with malignant tumors. Nodal and distant metastasis occured in 4 and 5 cases respectively. FoFty six (76,7%) patients with benign tumors and 47 (44,3%) patients with, malignant tumors were alive without disease .after follow-up of 5 years. Age, stages T. and N, nodal, skin. and bone involvements and type of treatment modified the overall survival. For ftee disease survival the most importants variables were stage N, histological type, nodal involvement and bone involvement. The multivariate analysis showed that the most importants factors to death risk were stage N, histological type, gender and type of surgery.
