Fistulização temporária do ceco em equinos : estudo experimental da técnica cirúrgica e da viabilidade desta via para a administração de fluído enteral /

Foz Filho / Resumo: Na prática cotidiana da medicina eqüina, são freqüentes as situações onde é necessária a reposição e ou manutenção do equilíbrio hidroeletrolítico com fluídos e eletrólitos, principalmente em afecções do trato digestório. O presente estudo teve como objetivo avaliar a técnica cirúrgica de fistulização temporária do ceco com a implantação de uma sonda nesta víscera e a utilização desta via para a administração de fluido enteral. Foram utilizados 6 eqüinos adultos entre 350 e 435 kg, machos e fêmeas, que foram submetidos a dois períodos de jejum - hídrico e alimentar - um período sem hidratação e outro período com a hidratação enteral por via intracecal. A solução hidratante foi constituída de 5,7 g de Cloreto de Sódio, 3,78 g de Bicarbonato de Sódio, e 0,37 g de Cloreto de Potássio por litro de água administrado seis vezes ao dia, na base de 50 ml/kg de peso. Ao final do experimento pudemos constatar que a técnica cirúrgica de implantação da sonda é viável e de baixo custo, podendo ser considerada uma opção viável de terapia hídrica, eletrolítica e de nutrição enteral em eqüinos, nas situações em que as vias tradicionais - intravenosa e nasogástrica - se apresentam indisponíveis ou a terapia tradicional se mostra economicamente inviável. / Abstract: In daily equine medical practice is often necessary to re-establish or maintain hemodynamic balance with fluids and electrolytes, mainly due to digestive tract diseases. The objective of this study is to evaluate therapeutic caecal fistulization surgery, with a catheter placement and the main trans and post-operative complications. Six adult horses of both sexes, weighting between 350 and 435kg were used; they were submitted to two periods of total fast - hydric and nutritional - one period without hydration and the other with enteral hydration by intracaecal route. The hydrating solution consisted of 5.7g sodium chloride, 3.78g sodium bicarbonate, and 0.37g potassium chloride per litre of water. We could observe the horse's capacity to tolerate long periods without food and water, and that surgical catheter implant is a viable and low cost option for hydration and electrolyte therapy, and enteral nutrition in horses where traditional routes - intravenous and nasogastric - are not available or traditional therapy is not economically viable. / Mestre

When implanted in animal tissues, the BMPs link to specific receptors located in the surface of the osteoinduced cells, they activate the SMAD cytoplasmatic proteins (mothers against decapentaplegic proteins) and, in the nucleus of the receiver cell, they begin to regulate the transcription of genes related with calcification and bone formation. In the present study, aspects of clinical-surgical, macroscopic and radioscopic of the bone repair were evaluated after the implantation of the BMPs in different concentrations, carried by autogenous bone graft, in defects created on skulls of 20 adult-young female rabbits, randomizedly separated on five experimental groups that correspond to five periods of observations (7, 15, 21, 35 and 60 days). After the exposure of bones skull, six bone defects were performed on the fronto-parietal region of each animal. The defect I wasn´t filled, the II was completed filled with approximately 3,0 mg of autogenous bone graft and the defects III, IV, V and VI were filled with autogenous bone graft associed with 0,5; 1,0; 2,0 and 5,0 mg of BMPs respectively. There wasn´t clinical complications after the surgery. In the macroscopic evaluations, post-mortem, it was observed that independently of the treatment period of the defects, the bony filling began starting from the borders to the center and from the bottom to the surface of the flaws. The bony filling of defect I was the smallest found when compared with the others defects, in all the observation periods. It was also verified that until 2,0 mg the bigger the concentration of BMPs used, better was bone cover. The radiographic assessment did not permit the classification of the grade of bone filling and through the statistic analyses, it was comproved that there are significant differences between the treatments. It was verified microscopically in the first evaluations, at the seventh day, that the bone growth started from the borders and from the bottom of the lesion, with mobilization and differentiation of cells deriving from the periosteum and of the meninges, respectively, and in the subsequent evaluations, the osteoblastic activity also derived from "ossification islands" to ossification centers, located in the center of the flaw. The trabecular formation increased proportionally with the concentration of BMPs used, and the apposition and bone organization increased proportionally with the time of observation. The presence of cartilaginous tissue was verified in almost all the flaws. The best relation of quantities of autogenous bone graft and BMPS were 3,0 mg and 2,0 mg, respectively, and the association of this protein with this graft contributed to the formation of new bone tissue, promoting larger mobilization, differentiation and cellular organization. It also abbreviated the time of bone formation, suggesting an endochondral ossification process. / As proteínas morfogenéticas ósseas (BMPs) são promotores de crescimento capazes de induzir a formação de osso e cartilagem, promovendo a quimiotaxia e diferenciação de células mesenquimais para o local onde foram implantadas, mas para uma efetiva osteocondução necessitam de uma substância carreadora. Quando implantadas em tecidos animais, as BMPs se ligam a receptores específicos existentes na superfície das células osteoinduzidas, ativam as proteínas citoplasmáticas SMAD (mothers against decapentaplegic proteins) e, no núcleo da célula receptora, passam a regular a transcrição de genes relacionados com calcificação e formação óssea. No presente estudo, foram avaliados os aspectos clínico-cirúrgicos, macroscópicos, radiográficos e microscópicos da reparação óssea, após implantação de BMPs em diferentes concentrações, carreadas por enxerto ósseo esponjoso, em falhas criadas no crânio de 20 coelhas adultas-jovens, separadas aleatoriamente em cinco grupos experimentais, que corresponderam a cinco períodos de observação (7, 15, 21, 35 e 60 dias). Após a exposição dos ossos do crânio, foram realizadas seis falhas ósseas na região fronto-parietal de cada animal. A falha I não foi preenchida, a II foi completamente preenchida com aproximadamente 3,0 mg de autoenxerto ósseo esponjoso e as falhas III, IV, V e VI foram preenchida com o autoenxerto associado a 0,5; 1,0; 2,0 e 5,0 mg de BMPs, respectivamente. Não foram observadas complicações clínicas após a cirurgia. Nas avaliações macroscópicas, postmortem, verificou-se que independentemente do período de tratamento das falhas, o preenchimento ósseo iniciava-se a partir das bordas para o centro e do fundo para a superfície das falhas. O preenchimento ósseo das falhas I foi o menor encontrado quando comparado com as demais falhas, em todos os períodos de observação. Constatou-se também que até os valores de 2,0 mg, quanto maior a concentração de BMPs utilizada, maior foi a cobertura óssea. A avaliação radiográfica não permitiu a classificação do grau de preenchimento ósseo e por meio de análises estatísticas, foi comprovado que há diferenças significativas entre os tratamentos. Microscopicamente verificou-se nas primeiras avaliações, aos sete dias, que o preenchimento ósseo iniciava-se a partir das bordas e do fundo da lesão, com mobilização e diferenciação de células provenientes do periósteo e das meninges, respectivamente e, nas avaliações subseqüentes, a atividade osteoblástica provinha também de ilhas de ossificação , semelhantes a centros de ossificação, localizadas no centro da falha. A formação trabecular aumentou proporcionalmente com a concentração utilizada de BMPs e a aposição e organização óssea aumentaram proporcionalmente com o tempo de observação. Em quase todas as falhas foi verificada a presença de tecido cartilaginoso. A melhor relação de quantidade de autoenxerto ósseo esponjoso e BMPs foi de 3,0 mg e 2,0 mg, respectivamente e a associação desta proteína com esse enxerto contribuiu para a formação de novo tecido ósseo, promovendo maior mobilização, diferenciação e organização celular e, abreviou o tempo de formação óssea, sugerindo processo de ossificação endocondral. Évaluation de la stabilisation de la rupture du ligament croisé crânial canin par la suture latérale : une étude de cinématique 3D in-vitro

L’impact de la correction chirurgicale de la rupture du ligament croisé crânial (LCC) sur la cinématique 3D du grasset a peu été étudiée chez le chien. Plusieurs techniques tentent de restaurer la fonction normale du grasset suite à la rupture du LCC, mais les succès demeurent mitigés comme en témoignent le retour à une fonction suboptimale et la progression de l’arthrose. L’incapacité de ces techniques à restaurer la cinématique 3D pourrait être un élément clé expliquant ces succès suboptimaux. L’objectif de ce projet de recherche était d’évaluer l’impact d’une des techniques fréquemment utilisées, la stabilisation par suture latérale (SSL), sur la cinématique 3D du grasset. Dix membres pelviens provenant de grands chiens (25-40kg) ont été utilisés avec un montage préalablement validé simulant le trot. Trois situations ont été comparées : i) grasset sain, ii) grasset instable par transsection du LCC, et iii) grasset stabilisé par SSL. Pour chaque situation, la cinématique 3D était enregistrée durant 5 cycles dans le montage. Les courbes ont été analysées en utilisant un test statistique Wilcoxon signed rank test. La SSL a restauré la cinématique du grasset intact sain sur l’entièreté de la phase d’appui pour la translation crânio-caudale, médio-latérale, la flexion et l’abduction. Elle a restauré la translation proximo-distale sur 90% de la phase d’appui. La rotation interne a été limitée par la SSL, mais n’a pas été restaurée. Ces résultats montrent que cette technique chirurgicale reproduit une cinématique 3D globalement semblable à celle du grasset sain. La progression de l’arthrose rapportée suite à la stabilisation du grasset par SSL pourrait donc ne pas résulter d’une cinématique 3D anormale. / The impact of surgical correction of cranial cruciate ligament rupture (CCLR) on 3D kinematics has not been thoroughly evaluated in dogs. The success of current techniques remains limited, as illustrated by suboptimal weightbearing and progression of osteoarthritis. The inability to restore the stifle’s 3D kinematics might be a key element in understanding these suboptimal outcomes. The objective of this study was to evaluate the impact of lateral suture stabilization (LSS) on the 3D kinematics of the canine stifle joint. We hypothesized that LSS would not restore 3D kinematics in our model. Ten pelvic limbs collected from euthanized large dogs (25-40 kg) were tested with a previously validated apparatus that simulates trot. Three experimental conditions were compared: (a) intact stifle; (b) unstable stifle following cranial cruciate ligament transection (CCLt) and (c) CCLt stabilized by LSS. Three-dimensional kinematics were collected through 5 gait cycles and curves were analyzed using a Wilcoxon signed-rank test. LSS restored baseline kinematics for the entire stance phase for cranial and lateromedial translation, sagittal flexion, and abduction. It restored distraction over 90% of the stance phase. Internal rotation was limited, but not restored. This in vitro study had limitations, as it used a simplified model of stifle motion and weight-bearing. The results of this study demonstrate that LSS can restore physiologic 3D kinematics largely comparable to those of healthy stifles. Suboptimal outcome in patients following CCLR stabilization by LSS may therefore result from causes other than immediate postoperative abnormal 3D kinematics.

The aim of this diploma thesis is the design of this closed area of the shelter dogs with veterinary facilities and administration shelter in a specific location in the outskirts of Bystřice pod Hostýnem, street Meziříčská. It is a building amenities. In addition to the project itself is part of the job specialization - health and technical installations and connections. The work was developed in response to a bachelor thesis. The intention is to create a functional device with everyday hour, which will be provided by 7 employees. Given that the region is not represented such a device, its implementation will meet the requirements and demands of citizens of the town but also surrounding communities. The capacity of the shelter is designed based on the client's requirements with the possibility of placing domestic animals in max. Of 50 dogs, 50 cats and 16 small animals. Unless capacity is full, the device can be used as an animal hotel. In its own proposal, the emphasis was on the overall architectural character and design solutions.
