Différences ethniques et saisonnières du statut en vitamine D chez les étudiants au Québec : une étude de cohorte

Une faible exposition au soleil est un des facteurs importants de la déficience en vitamine D dont la prévalence est élevée dans les pays à hautes latitudes et chez les personnes de race noire. Les concentrations sériques de vitamine D et les facteurs pouvant influencer celles-ci ont été mesurés à trois reprises (automne 2010, hiver et été 2011) dans un échantillon de sujets volontaires résidant dans la ville de Québec. Soixante-cinq sujets dont 29 africains et 36 québécois ont complété l’étude. Le statut de vitamine D a varié au cours des saisons. Il était particulièrement sous-optimal chez les sujets africains. Les concentrations de vitamine D étaient significativement associées à la race, à l'indice de masse corporelle (IMC) et aux saisons. Le faible statut en vitamine D des sujets résidents à Québec et particulièrement celui des ressortissants africains est un problème de santé publique préoccupant qui devrait être exploré davantage. / Lack of sun exposure is a major risk factor of vitamin D deficiency, which is more pronounced in countries of high latitudes and in black people. Serum concentrations of vitamin D, intrinsic and extrinsic factors, which may influence vitamin D status, were measured 3 times (fall 2010, winter and summer 2011) in a sample of volunteer students living in Quebec City. A total of 65 participants including 29 Africans students and 36 Quebecers students completed the study. Vitamin D concentrations changed over the seasons. They are particularly low among African participants. Vitamin D concentrations were significantly correlated with race, body mass index (BMI), and seasons. The low vitamin D status of students in Quebec City and particularly among Africans students is a public health concern that should be explored further.

Nous avons mené une étude transversale afin de caractériser biologiquement le statut de la vitamine D et ses déterminants chez les Cris de la Baie James. Dans un échantillon aléatoire de 944 participants âgés de 15 ans et plus provenant de la Nituuchischaayhitaau Aschii: A Multi-Community Environment and Health Longitudinal Study in Iiyiyiun Aschii, une grande proportion (49%) des participants avaient des niveaux insuffisants en vitamine D selon le seuil récemment établi. La prévalence d’insuffisance en vitamine D était nettement plus marquée chez les femmes et chez les participants âgés de 15 à 39 ans. Dans un deuxième échantillon de 52 enfants âgés de huit à 14 ans, cette proportion était de 43%. Notre étude souligne l’importance d’informer les professionnels de la santé en regard aux risques associés à des niveaux bas en vitamine D et aux interventions pouvant les prévenir. / Aboriginal peoples affected by a nutrition transition and living at high latitudes are among the groups most at risk of vitamin D insufficiency. We conducted a cross-sectional study to examine the vitamin D status and associated factors among James Bay Cree. Data from 944 participants aged 15 years and above are from the Nituuchischaayihtitaau Aschii: A Multi-Community Environment and Health Longitudinal Study in Eeyou Istchee. According to the new reference level recently established, the vitamin D status in Cree was found to be suboptimal with nearly half (49%) of the sample population having inadequate concentrations for optimum bone health. We found that women and participants aged less than 40 years were more at risk of insufficiency. In a small sample of Cree children aged eight to 14 years, 43% of participants had concentrations inadequate for bone. Our findings support that it is essential that health professionals be informed in order to provide the necessary dietary counselling or other interventions at the population level to prevent the nutrition transition.
