Simulation of Myocardium Motion and Blood Flow in the Heart with Fluid-Structure Interaction

The heart is a complex organ and much is still unknown about its mechanical function. In order to use simulations to study heart mechanics, fluid and solid components and their interaction should be incorporated into any numerical model. Many previous studies have focused on myocardium motion or blood flow separately, while neglecting their interaction. Previous fluid-structure interaction (FSI) simulations of heart mechanics have made simplifying assumptions about their solid models, which prevented them from accurately predicting the stress-stain behaviour of the myocardium. In this work, a numerical model of the canine left ventricle (LV) is presented, which serves to address the limitations of previous studies. A canine LV myocardium material model was developed for use in conjunction with a commercial finite element code. The material model was modified from its original form to make it suitable for use in simulations. Further, numerical constraints were imposed when calculating the material parameter values, to ensure that the model would be strictly convex. An initial geometry and non-zero stress state are required to start cardiac cycle simulations. These were generated by the static inflation of a passive LV model to an end-diastolic pressure. Comparisons with previous measurements verified that the calculated geometry was representative of end diastole. Stresses calculated at the specified end diastolic pressure showed complex spatial variations, illustrating the superiority of the present approach over a specification of an arbitrary stress distribution to an end-diastolic geometry. In the third part of this study, FSI simulations of the mechanics of the LV were performed over the cardiac cycle. Calculated LV cavity pressures agreed well with previous measurements during most of the cardiac cycle, but deviated from them during rapid filling, which resulted in non-physiological backflow. This study is the first one to present a detailed analysis of the temporal and spatial variations of the properties of both the solid and the fluid components of the canine LV. The observed development of non-uniform pressure distributions in the LV cavity confirms the advantage of performing FSI simulations rather than imposing a uniform fluid pressure on the inner surface of the myocardium during solid-only simulations.

Stented Artery Biomechanics: A Computational and In Vivo Analysis of Stent Design and Pathobiological Response

Vascular stents have become a standard for treating atherosclerosis due to distinct advantages in trauma and cost with other surgical techniques. Unfortunately, the therapy is hindered by the risk of a new blockage (termed restenosis) developing in the treated artery. Clinical studies have indicated that stent design is a major risk factor for restenosis, with failure rates varying from 20 to 40% for bare metal stents. Subsequently, there has been a significant effort devoted to reducing failure rates by covering stents in polymer coatings in which anti-proliferative drugs are embedded, however complications have arisen (e.g. incomplete endothelization, lack of success in peripheral arteries, lack of long-term follow-up studies) that have limited the success of this technology. It has been thought that restenosis is directly related to the mechanical conditions that vascular stents create. Moreover, it has been hypothesized that stents that induce higher non-physiologic stresses result in a more aggressive pathobiological response that can lead to restenosis development. In this study, a combination of computational modeling and in vivo analysis were conducted to investigate the artery stent-induced wall stresses, and subsequent biological inflammatory response. In particular, variations in stent design were investigated as a means of examining specific stent design criteria that minimize the mechanical impact of stenting. Collectively, these data indicate that stent designs that subject the artery wall to higher stress values result in significantly more neointimal tissue proliferation, therefore, confirming the aforementioned hypothesis. Moreover, this work provides valuable insight into the role that biomechanics can play in improving the success rate of this percutaneous therapy and overall patient care.

The loading and function of the mitral valve under normal, pathological and repair conditions: an in vitro study

Currently, mitral valve repair techniques have shown substandard mid-term and long term results. In order to improve the efficacy of these repair techniques, detailed knowledge of normal mitral valve function and the alterations to the valvular and subvalvular apparatus which occur under pathological conditions is required. Furthermore, current techniques may be optimized through a better understanding of the function and mechanics of the mitral valve after a particular repair. The experiments which comprise this study were designed using an in vitro approach since this technique has the clear advantage of isolating and independently controlling specific parameters that are of importance to valvular mechanics and function. The experiments were conducted in the Georgia Tech Left Heart Simulator using native porcine and human mitral valves. The first set of experiments measured the chordal force distribution and anterior leaflet strain of the mitral valve in its normal geometrical configuration. Subsequent experiments measure mitral regurgitation volume and chordal force distribution in conditions associated with ventricular dilation. The last set of experiments simulated two commonly used mitral repair techniques. For the Alfieri stitch experiments, the effects of mitral flow rate, transmitral pressure, and mitral annular area on valve stenosis, mitral regurgitation and Alfieri stitch force were evaluated. For annuloplasty, the effect of annular saddle curvature on anterior leaflet strain was quantified. In Conclusion, the normal geometry of the native mitral valve optimized its function and mechanics. Under pathological conditions associated with ventricular dilation, significant alterations to mitral valve function and mechanics were present. Although the studied repair techniques may have significantly restored valve function, severe alterations to the mechanics of the valve still persisted.
