Revue de littérature et étude rétrospective portant sur le diagnostic, le traitement et le suivi de 36 cas confirmés d'actinomycose cervico-faciale

L'actinomycose cervico-faciale représente une infection endogène peu fréquente et mal connue. La révision de la littérature nous expose qu'il n'y a pas de consensus sur ses caractéristiques cliniques, diagnostiques et thérapeutiques. Nous avons révisé les dossiers de 36 patients porteurs d'actinomycose, traités à l'Hôpital de l'Enfant-Jésus de Québec. Les résultats d'une étude rétrospective nous ont permis de valider un protocole diagnostique et thérapeutique incluant la scintigraphic osseuse comme élément décisionnel et de conclure que toute infection maxillo-faciale apparaissant plus de 2 mois suivant un traumatisme chirurgical devrait soulever un diagnostic d'actinomycose

Nesse sentido, formulações nanotecnológicas foram desenvolvidas com o objetivo de carrear o fármaco imiquimode para um local específico – a mucosa vaginal – esperando gerar melhores desempenhos nesse tratamento quando comparados com a formulação comercial. Três nanoestruturas com morfologias distintas foram propostas visando potencializar o efeito do fármaco em células de câncer cervical (SiHa). As formulações desenvolvidas compreenderam: nanoemulsões (NEimiq), nanocápsulas poliméricas (NCimiq) e nanocápsulas poliméricas revestidas com quitosana (NCimiq-chit). Observou-se que nanocápsulas poliméricas produzidas com poli(ε-caprolactona) apresentaram efeito mais pronunciado frente às células SiHa. Para tanto, essas formulações (NCimiq e NCimiq-chit) foram incorporadas em hidrogéis de quitosana e de hidroxietilcelulose a fim de possibilitar uma melhor futura aplicação para o paciente. Estudos envolvendo mucosa vaginal suína demonstraram que ambas as formulações são mucoadesivas e permeiam a mucosa vaginal. Porém, a formulação produzida com hidrogel de quitosana (NCimiq) apresentou maior desempenho. Esta foi a formulação escolhida para dar continuidade aos estudos deste trabalho, sendo objeto de estudo posterior em cultura de células SiHa a fim de elucidar o mecanismo de ação da mesma. Esses estudos demonstraram que há uma ocorrência de processos combinados de diminuição da viabilidade celular de maneira tempo-dependente e que mecanismos como apoptose, autofagia e parada de ciclo celular estão presentes. Essa formulação (NCimiq) apresentou porcentagens de morte celular significativas, mesmo utilizando baixas concentrações do fármaco. The influence of the hormonal milieu on eicosanoid and cytokine production in tissues from the female reproductive tract

In the human uterus prostaglandins (PG) PGE2, PGD2, PGI2, PGF2α and Thromboxane A2 (TXA2), also termed prostanoids, are synthesised and deactivated to 15-keto PGE2, J2 metabolites, 6-keto-PGF1α, 15-keto PGF2α and TXB2 respectively. However, not all metabolites have been analysed simultaneously within the same tissue. The primary objective of this thesis was to determine full uterine prostanoid profiles in human non-pregnancy, pregnancy and parturition, to better understand these processes and find suitable tocolytic targets. In addition, ten cytokines in human cervico-vaginal fluid (CVF) were measured according to interval to labour to test their suitability as labour onset predictors, with a view to developing a test to determine women at risk of preterm labour. Prostanoid analysis was carried out in endometrium (n=9) and myometrium (n=15- 16) donated by non-pregnant women and lower segment myometrium obtained from pregnant women (before (n=14) and after labour onset (n=7)) by liquid chromatography coupled with electrospray ionisation mass spectrometry (LC/ESIMS/ MS). Cytokines produced by CVF collected from pregnant donors (20-41 weeks gestation, n=2-10) were investigated using Enzyme-Linked Immunosorbent Assay (ELISA) or Luminex®. Human endometrium produced greater concentrations of TXB2, PGE2 and PGF2α than myometrium in vitro (p<0.05). Fifteen prostanoids were detected in human myometrium. Production of 6-keto-PGF1α, PGE1 and PGF1α increased whilst 15- keto PGE2 and PGJ2 decreased at term pregnancy (37-41 weeks gestation) versus non-pregnancy (p<0.05). Myometrium from parturient donors synthesised TXB2 and PGE2 more abundantly than the non-labouring equivalent. Cytokine concentration was greatest in CVF sampled the week before labour, in particular Interleukin-6 (IL-6), Macrophage Inflammatory Protein-1α (MIP-1α) and Monocyte Chemotactic Protein-1 (MCP-1) (p<0.05). Endometrial TXB2, PGE2 and PGF2α could aid in proliferation of glandular epithelium prior to ovulation. Prostacyclin may facilitate prolongation of pregnancy to term and thromboxane could contribute to uterine stimulation during labour. Cervical dilation may be influenced by PGE2 in lower segment myometrium. MCP- 1, MIP-1α and IL-6 could mark a short interval to labour onset.

Tumor neuroendocrino en cérvix uterino: reporte de caso

Neuroendocrine tumors of the cervix are extremely rare. Women diagnosed with small cell neuroendocrine carcinoma of the cervix have a higher frequency of metastases in the lymph nodes, lymphovascular invasion, recurrence and worse prognosis compared to those with other types of cervical neoplasia. We report the case of a 58-year-old female, with a history of six years of postmenopausal irregular vaginal bleeding, in addition to symptoms related to chronic anemia. Gynecological examination showed a tumor of 4 cm that occupied the upper third of the vagina and protruded through the cervix initially diagnosed as an abortifacient myoma, and sent to histopathology study. 90% of the tumor was small cell neuroendocrine carcinoma grade III, and the remaining 10% was squamous cell carcinoma. The patient underwent into a radical hysterectomy plus bilateral salpingo-oophorectomy, and bilateral pelvic and para-aortic lymphadenectomy. Histopathologic examination of the surgical specimen found endometrium and myometrium compromised by malignancy. Parametrium, annexes and lymph nodes were free of neoplasia. At microscopy, the result was a grade III neuroendocrine carcinoma (small cell carcinoma, infiltrating), with extensive lymphovascular emboli. The immune-histochemical study showed synaptophysin positive in areas with neuroendocrine differentiation. / Los tumores neuroendocrinos de cuello uterino son extremadamente raros. Las mujeres con diagnóstico de carcinoma neuroendocrino de células pequeñas del cuello uterino tienen mayor frecuencia de metástasis en los ganglios linfáticos, invasión linfovascular, recurrencia y peor pronóstico en comparación con aquellos con otros tipos de neoplasias cervicales. Se presenta el caso de una mujer de 58 años, con un tiempo de enfermedad de seis años antes del ingreso, caracterizado por sangrado vaginal irregular posmenopáusica, además de sintomatología relacionada a anemia crónica. En el examen ginecológico, se evidenció tumoración de 4 cm que ocupaba tercio superior de vagina y protruía por el cérvix. Fue diagnosticado como mioma abortivo y enviada a estudio anatomopatológico. El resultado fue carcinoma neuroendocrino de células pequeñas grado III en el 90% y carcinoma epidermoide en el 10%. La paciente fue sometida a histerectomía radical más salpingo-ooferectomía bilateral y linfadenectomía pélvica bilateral y para-aortica. El estudio anatomopatológico de la pieza quirúrgica encontró endometrio y miometrio comprometido por neoplasia maligna. Parametrios, anexos y ganglios linfáticos se encontraron libres de neoplasia. A la microscopía el resultado fue carcinoma neuroendocrino grado III (carcinoma de células pequeñas, infiltrante), con extensa embolia linfovascular. El estudio de inmunohistoquímica arrojó sinaptofisina positivo en las áreas con diferenciación neuroendocrina.

Génotypage moléculaire des papillomavirus humains chez des femmes à risque de cancer du col de l'utérus : implication pour le dépistage et la prévention / Molecular genotyping of human papillomavirus in women at risk of cervical cancer : Implications for screening and prévention

A l'échelle mondiale, le cancer du col de l'utérus (CCU) constitue un problème majeur de santé publique touchant 528 000 femmes, il est responsable de 266 000 décès chaque année. Son taux d'incidence varie d'un pays à l'autre et reste remarquablement plus élève dans les pays en voie de | développement où l'accès aux soins, même primaires reste précaire. De nombreuses études I épidémiologiques ont démontré qu'environ 99,7 % des cancers du col de l'utérus sont associés à l'infection par des Papillomavirus Humains (HPV) oncogènes, en particulier les HPV16 et 18. Ces données ont permis le développement de vaccins prophylactiques dirigés contre ces deux génotypes viraux. Il s'agit d'un des! cancers les plus faciles à prévenir et à traiter a condition qu'il soit détecté suffisamment tôt et traité| correctement. L'objectif de ce travail de thèse était d'étudier deux populations de femmes à risque du CCU qui vivent sur deux continents différents (Afrique/Europe). Ces deux populations étaient référées auprès de médecins hospitaliers pour des problèmes gynécologiques. La majorité des femmes n'avaient pas été dépistées (femmes marocaines), ou avaient été sous-dépistées selon les recommandations de l'ANAES (femmes françaises de plus de 65 ans). Au cours de cette étude, les femmes ont eu un frottis cervico-utérin (FCU) pour une analyse cytologique et une recherche d'ADN d'HPV, soit par amplification de cible (PCR ; population marocaine), soit par amplification de signa! (test hc2 ; population française). Les échantillons; HPV positifs ont ensuite été génotypes, soit par génotypage complet (INNO-LiPA ; population marocaine),| soit par génotypage partiel (qPCR HPV16,18 et 45 ; population française). Les frottis étaient normaux dans la majorité des cas : chez 81,9% des femmes marocaines et 68,6% des femmes françaises. Les frottis de signification indéterminée (ASC-US) ont été diagnostiqués chez 15,8% des femmes françaises. Les frottis en faveur d'une lésion de bas grade (LGSIL) étaient observes chez 12,9% des femmes marocaines et 7,4% des femmes françaises. Les frottis en faveur d'une lésion de haut grade (HGSIL) étaient de 5,2% chez les femmes marocaines et 4% chez les femmes françaises. Les frottis évocateurs de cancer ont été décelés uniquement dans la population française (2,9%). La prévalence d'HPV était du même ordre de grandeur (23%) dans les 2 populations et augmentait avec la sévérité de la lésion cytologique pour atteindre 75% dans les frottis HGSIL chez les femmes marocaines et plus de 90% chez les femmes françaises. La quasi-totalité des échantillons en faveur d'un CCU étaient infectés par un HPV. Par ailleurs, dans les deux populations, le génotype prévalent dans tous les types de frottis était THPV16, génotype le plus cardnogène. Un tiers (36,2%) de la cohorte des femmes marocaines était infecté par le VIH, qui s'est avéré le plus important facteur prédictif de l'infection HPV, indépendamment des autres facteurs de risqueI étudiés (caractéristiques sociodémographiques, comportement sexuel, tabagisme...). En France, le dépistage du CCU est préconisé tous les trois ans chez les femmes de 25 à 65 ans. Toutefois, les femmes âgées de plus de 65 ans dont l'espérance de vie est élevée et qui n'ont pas été ou qui ont été mal dépistées pourraient bénéficier d'un test HPV dont la Valeur Prédictive Négative est très élevée. Au Maroc, en l'absence de dépistage organise, il est aussi souhaitable de proposer un test HPV dans le cadre du dépistage j du CCU. La vaccination anti-HPV pourrait aussi prévenir ce cancer. / Worldwide, cervical cancer (CC) is a major public health problem affecting 528 000 women and responsible for 266 000 deaths every year. These data allowed the development of prophylactic vaccines directed against these two viral genotypes. It is one of the easiest cancers to prevent by early detection of precancerous lesions. The objective of this PhD thesis work was to study two populations of women at risk of CC who live on two different continents (Africa / Europe). These two populations were referred to hospital doctors for gynecological problems. The majority of women were not screened (Moroccan women) or were under-screened according to ANAES recommendations (French women over 65 years). At entry in the study, women had a cervical smear for cytology and HPV DNA research, either by target amplification (PCR; Moroccan population) or signal amplification (hc2 test; French population). HPV positive samples were then genotyped, either with full genotyping (INNO-LiPA; Moroccan population) or with partial genotyping (qPCR targeting HPV16,18 and 45; French population). Smears were normal in most cases: in 81.9% of Moroccan women and 68.6 % of French women. Atypical Squamous Cells of Undetermined Significance (ASC-US) were diagnosed in 15.8% of French women. Smears in favour of low grade lesion (LGSIL) were observed in 12.9% of Moroccan women and 7.4 % of French women. Smears in favour of high grade lesion (HGSIL) were in 5.2 % of Moroccan women and 4% of French women. Smears suggestive of cancer were detected only in the French population (2.9%). The prevalence of HPV was of the same order of magnitude (23%) in both populations; and increased with the severity of cytological lesions to reach 75% in HGSIL smears among: Moroccan women and over 90% among French women. Almost ail samples with CC were infected with high risk HPV. Furthermore, in both populations, the most prevalent genotype in ail types of smear wasHPV16, known to be the most carcinogenic. One third (36.2%) of the cohort of Moroccan women was infected with HIV, which was the most powerful predictor of HPV infection, independent of other risk factors studied (sociodemographic characteristics, sexual behavior, smoking ...).In France, CC screening is recommended every three years for women 25 to 65 years. However, women over 65 whose life expectancy is high and who have not been or have been poorly screened could benefit from an HPV test. In Morocco, in absence of organized screening, it is also desirable to provide an HPV test as a part of CC screening. The HPV vaccines could improve the situation by reducing the incidence and mortality from this cancer

L’évaluation préopératoire de la profondeur d’invasion des carcinomes épidermoïdes de la langue mobile : connaissances actuelles et rôle diagnostique de la biopsie au poinçon

L’inclusion récente de la profondeur d’invasion (PI) dans la classification des carcinomes épidermoïdes de la cavité orale de l’American Joint Committee on Cancer (AJCC) a des répercussions cliniques majeures. Plusieurs études ont récemment évalué la fiabilité de diverses modalités d’imagerie et techniques de biopsie pour mesurer la PI en préopératoire. L’objectif premier de ce mémoire est de réviser systématiquement la littérature et comparer les différentes méthodes décrites de mesure de PI en préopératoire pour les carcinomes épidermoïdes de la langue mobile. Le second objectif est d’étudier la précision et la fiabilité de la mesure de PI sur une biopsie au poinçon dans les carcinomes épidermoïdes de stade in situ (Tis)-T1-T2, N0 de la langue mobile. Une revue systématique a été effectuée en suivant le guide PRISMA[1]. Les études évaluant la fiabilité de la PI mesurée sur la biopsie ou l’imagerie médicale, en les comparant à la PI histopathologique finale, ont été inclues dans une méta-analyse afin d’obtenir des coefficients de corrélation combinés pour chaque modalité d’imagerie. L’imagerie par résonnance magnétique (IRM) s’est avérée être la modalité d’imagerie la mieux étudiée et présente une bonne fiabilité. Le computed tomography (CT) scan est peu étudié, mais semble moins fiable. L’échographie linguale ne peut être comparée à ces deux modalités d’imagerie car elle est plus fréquemment utilisée pour mesurer l’épaisseur tumorale que la PI. La seconde étude est une preuve de concept prospective. Un poinçon profond a été utilisé pour échantillonner la portion la plus profonde de carcinomes épidermoïdes de la langue mobile de stade Tis-T1-2, N0 chez 27 patients. Des coefficients de corrélation de Spearman ont été calculés entre la PI estimée à la palpation manuelle, mesurée à la biopsie, et à l’histopathologie. La sensibilité et la spécificité de la biopsie au poinçon pour distinguer le Tis du carcinome épidermoïde invasif ont été calculées. Bien que la PI mesurée à la biopsie ne corrèle pas fortement avec la PI histopathologique, cette preuve de concept est limitée par la taille d’échantillon. La biopsie au poinçon semble toutefois être un outil fiable pour distinguer le Tis de l’invasif. D’autres études sont nécessaires avant de pouvoir recommander l’utilisation systématique de la biopsie pour décider en préopératoire si un évidement cervical électif est nécessaire. / The inclusion of depth of invasion (DOI) in the American Joint Committee on Cancer’s staging system for oral cavity squamous cell carcinoma (SCC) has major clinical impacts. Recent studies have evaluated the reliability of imaging modalities and biopsy techniques to measure DOI preoperatively. The first objective of this master’s thesis is to systematically review and compare the preoperative DOI measurement methods that have been studied so far in oral tongue SCC (OTSCC). The second objective is to prospectively study the precision and reliability of punch biopsy to measure DOI preoperatively in early (in situ (Tis)-T1-T2, N0) OTSCC, and its ability to distinguish Tis from invasive carcinoma. A systematic review was conducted according to the PRISMA guidelines. Studies that evaluated the reliability of DOI measured on biopsy or imaging (rDOI) by comparing it to DOI on histopathology (pDOI) were included in a meta-analysis to obtain pooled correlation coefficients for each imaging modality. Overall, magnetic resonance imaging (MRI) is the better studied modality. It has a good reliability to measure preoperative rDOI in OTSCC. CT is less studied but appears to be less reliable. Ultrasound (US) cannot be compared to these imaging modality as it has been used more often to measure tumor thickness (TT) than DOI. The second study is a prospective proof-of-concept. A deep punch biopsy was used to sample tumors preoperatively in the deepest part of the tumor in 27 patients with early (Tis-T12, N0) oral tongue squamous cell carcinoma. Spearman’s correlations were calculated between DOI measured on digital palpation (cDOI), biopsy (bDOI) and final pDOI. The sensitivity and specificity of punch biopsy to distinguish Tis from invasive carcinoma was also calculated. Although bDOI does not seem to correlate strongly with pDOI, this proof-of-concept was limited by a small sample size. Punch biopsy appears to be a reliable tool to distinguish Tis from invasive carcinoma. Further studies on punch biopsy are needed to recommend its use to evaluate pDOI preoperatively and determine whether elective neck dissection is necessary in early OTSCC.
