Studying Co-Medication Patterns: The Impact of Definitions

Purpose: To show the necessity of distinguishing several patterns of drug prescribing that may lead to co-medication. It is demonstrated how these different patterns can be investigated using large databases containing pharmacy data or reimbursement data. Methods: Two examples illustrate how the particular pattern of co-medication studied will influence the reported proportion of patients having co-medication, the use of antidepressants among people using anticonvulsants, and the use of antihistamines among people receiving penicittines. Results: Depending on definition and period considered, the percentage of anticonvulsant users co-medicated with antidepressants ranged from 5.8% (95%CI 5.0%, 6.8%) to 14.5% (95%CI 13.2%, 15.9%) in 2000. Comparing 2002 with 2000, the ratio of proportions ranged from 1.3 to 2.1. The percentage of people who received penicillines and were co-medicated with antihistamines ranged from 0.5% (95%CI 0.4%, 0.6%) to 9.7% (95%CI 9.3%,10.2%) in 2000. Comparing 2002 with 2000, the ratio of proportions ranged from 1.2 to 1.6. Conclusion: The co-medication patterns investigated yielded clinical as well as statistically significant different estimates. The estimates differed up to a factor 2.5 for the drugs usually prescribed for long periods, and a factor 12 for drugs prescribed for short periods. Hence, we propose to distinguish the patterns 'co-prescribing', 'concomitant medication,' and 'possibly concurrent medication.' The research question determines the co-medication pattern of interest, and the drug and disease under study determine the time window.

Optimum Enterprise Combinations for Representative Farms in Seven Counties of Southwestern Utah, 1967

Resources available for agricultural production were ascertained and trends in their use were studied for seven counties of southwestern Utah. Special consideration was given to cropland, irrigation water, and operating capital as resource restrictions for a linear programming model. Budget data were obtained for crop and livestock enterprises by interviewing farmers and ranchers in the area. Budgets for four representative farm types representing climatic and irrigation differences were made to study crop possibilities. Acreage minimums or maximums for selected crops were established as conservation measures. Budgets were prepared for farm livestock enterprises to use with crop budgets for each representative farm. Results provided profit maximizing enterprise combinations for each representative farm.

Modeling and Analysis of Regulatory Elements in Arabidopsis thaliana from Annotated Genomes and Gene Expression Data

Modeling of cis-elements in the upstream regions of genes is a challenging computational problem. A set of regulatory motifs present in the promoters of a set of genes can be modeled by a biclique. Combinations of cis-elements play a vital role in ascertaining that the correct co-action of transcription factors binding to the gene promoter, results in appropriate gene expression in response to various stimuli. Geometrical and spatial constraints in transcription factor binding also impose restrictions on order and separation of cis-elements. Not all regulatory elements that coexist are biologically significant. If the set of genes in which a set of regulatory elements co-occur, are tightly correlated with respect to gene expression data over a set of treatments, the regulatory element combination can be biologically directed. The system developed in this work, XcisClique, consists of a comprehensive infrastructure for annotated genome and gene expression data for Arabidopsis thaliana. XcisClique models cis-regulatory elements as regular expressions and detects maximal bicliques of genes and motifs, called itemsets. An itemset consists of a set of genes (called a geneset) and a set of motifs (called a motifset) such that every motif in the motifset occurs in the promoter of every gene in the geneset. XcisClique differs from existing tools of the same kind in that, it offers a common platform for the integration of sequence and gene expression data. Itemsets identified by XcisClique are not only evaluated for statistical over-representation in sequence data, but are also examined with respect to the expression patterns of the corresponding geneset. Thus, the results produced are biologically directed. XcisClique is also the only tool of its kind for Arabidopsis thaliana, and can also be used for other organisms in the presence of appropriate sequence, expression, and regulatory element data. In vitro anti-malarial interaction and gametocytocidal activity of cryptolepine

Yes / Background: Discovery of novel gametocytocidal molecules is a major pharmacological strategy in the elimination and eradication of malaria. The high patronage of the aqueous root extract of the popular West African anti-malarial plant Cryptolepis sanguinolenta (Periplocaceae) in traditional and hospital settings in Ghana has directed this study investigating the gametocytocidal activity of the plant and its major alkaloid, cryptolepine. This study also investigates the anti-malarial interaction of cryptolepine with standard anti-malarials, as the search for new anti-malarial combinations continues. Methods: The resazurin-based assay was employed in evaluating the gametocytocidal properties of C. sanguinolenta and cryptolepine against the late stage (IV/V) gametocytes of Plasmodium falciparum (NF54). A fixed ratio method based on the SYBR Green I fluorescence-based assay was used to build isobolograms from a combination of cryptolepine with four standard anti-malarial drugs in vitro using the chloroquine sensitive strain 3D7. Results: Cryptolepis sanguinolenta ( IC50 = 49.65 nM) and its major alkaloid, cryptolepine ( IC50 = 1965 nM), showed high inhibitory activity against the late stage gametocytes of P. falciparum (NF54). In the interaction assays in asexual stage, cryptolepine showed an additive effect with both lumefantrine and chloroquine with mean ΣFIC50s of 1.017 ± 0.06 and 1.465 ± 0.17, respectively. Cryptolepine combination with amodiaquine at therapeutically relevant concentration ratios showed a synergistic effect (mean ΣFIC50 = 0.287 ± 0.10) whereas an antagonistic activity (mean ΣFIC50 = 4.182 ± 0.99) was seen with mefloquine. Conclusions: The findings of this study shed light on the high gametocytocidal properties of C. sanguinolenta and cryptolepine attributing their potent anti-malarial activity mainly to their effect on both the sexual and asexual stages of the parasite. Amodiaquine is a potential drug partner for cryptolepine in the development of novel fixed dose combinations.

Impact of Particle Morphology on the Rheology of PCC-Based Coatings

The impact of particle size, size distribution, and particle shape on the rheology of precipitated calcium carbonate (PCC) based coatings was studied. Evaluating the interactions between different particle sizes and shapes leads to a better understanding of the packing fraction of PCC. HIgh packing fraction is desirable because of the positive impact on the fluidity of suspensions. Suspension with higher levels of fluidity can potentially load larger amounts of solids while keeping low viscosities. High solids suspensions are key factors to improve the efficiency of coating processes. To address this issue, PCC of different sizes and shapes where mixed in different ratiosto find mixtures with higher packing fractions that could result in coatings with lower viscosities. When coatings containing 90% of large particles and 10% by weight of small particles of different shape, viscosity decreases by 50% for one combination. The effect is caused by the higher packing fraction achieved. Future research is also described here.

The experimental determination of structural design parameters for roof covering systems

For roof structures the critical loading combinations are predominantly wind actions. The accumulative effect of wind actions, by wind entering through dominant openings to exert pressure on the inside of roof structures together with the suction of wind vortices on the outside of the roof, can contribute to extreme load combinations. Frequently recorded failures on roof structures suggest that either the loads are underestimated or the resisting capacity of the roof coverings is overestimated. The focus of this study is directed on the latter, determining the effective resistance of roof coverings in the form of sheeting against a Uniformly Distributed Load (UDL) such as wind actions. To determine the carrying capacity of a roofing structure, the standard approach used involves experimental tests on certain configurations with two or more spans. The structural test set-up is loaded with sandbags until failure is reached. For the design of roofing systems, design tables are used that list the maximum allowable purlin spacing. The purlin spacing is presented in the form of a fixed value in units of length and is shown independent of a UDL that the roof needs to be designed for. The need to a new approach to determining the resistance of roof covering systems was identified. The resistance of roof coverings for the Ultimate Limit State (ULS) and the Serviceability Limit State (SLS) depends on a number of parameters such as the bending resistance, the stiffness of the sheeting in bending and the carrying capacity of the fastening system. To evaluate these structural parameters, experimental tests were performed. A full-scale experimental test setup, capable of simulating a UDL on roof sheeting, was developed. The experimental test set-up consists of four different configurations, each specifically schematized to evaluate a certain structural design parameter. The magnitude of the structural design parameters depends on the applied UDL and the span length, which is the distance between consecutive supports of the sheeting system. Therefore, by using the structural design parameters determined experimentally, a set of design tables could be generated. The design tables produce the maximum allowable span length of a roofing system that uses a desired UDL as a variable. By using the design tables, the purlin spacing for any roof structure can be calculated given its design loading combination. Characterisation of the combined effects of physicochemical parameters and toxicants on microbial cells

This thesis reports on the combined effects of toxicants and physicochemical factors on micro-organisms. The main objective of the project was to use multi-sensing systems such as mediated and non-mediated sensor systems, growth tests and physicochemical sensors to investigate novel stressor-toxicant-assay combinations. Screen-printed, disposable, developmental-phase, physicochemical sensor constructs (conductivity and dissolved oxygen) were validated under conditions compatible with microbial bioassays, to ascertain their potential role in toxicity testing. The conductivity sensor construct could be used to indirectly inform on the osmolality of the test samples, but the dissolved oxygen sensor construct was not found to give reproducible results. The results were thought to be compromised by in-house screen-printing using a complex carbon ink formulation for the working electrode. Escherichia coli and a consortium with ammonia oxidation capacity (CAOC) were used as the test species for the bioassays. The combined effects of four inorganic salts (NaCl, NaN03, KCl and KN03) and two toxicants (3,5-DCP and HgCh) on E. coli were investigated using the CellSense™ biosensor system, Clark oxygen electrode and microtitre plate growth assays. A variety of trends were observed with each salt-toxicantbioassay combination, emphasising a need for better understanding of the assay media and factors such as bioavailability, to interpret the toxicity data. The results also suggested the importance of using multiple bioassays with varied end points, for toxicity testing. The CAOC, which was isolated from the activated sludge, was tested for physicochemical stressor and toxicant effects using the mediated biosensors. The results were very different from those obtained with E. coli, indicating that each species reacts to toxicants and changes in physicochemical factors differently. Although the full potential of disposable, physicochemical sensors, at the point of toxicity testing was not achieved, the study did investigate previously uncharacterised, combined effects of salts and toxicants on microbial cells. It highlighted the need for development of hybrid systems and also offered a route towards integration of physicochemical and biological sensing systems for simultaneous monitoring of both environmental and biological elements.

The subject of the graduation theses is accounting solution of the cross-border mergers according to Czech trade-law legislation and comparison of their reflection in the Czech and Slovak legislation. The graduation theses contain also an analysis of the cross-border mergers from the accounting point of view according to International Financial Reporting Standards.
