Aktivizace seniorů v prostředí sociálních služeb / Activation of Elderly in an Environment of Social Services

The diploma is dedicated to the activation of seniors with dementia. The aim of the work is to outline the issue of progress and benefit of activation in seniors with dementia. The theoretical part deals with the theme of aging and aging with respect to various theoretical definitions. I have guided the dementia chapter, which is one of the most common problems of the aging population. I also deal with the overview of the social services that focus to seniors. At the end of the theoretical part I outline the benefits of activation of seniors with dementia and provide an overview of commonly used therapies in social care facilities. In the practical part I focused on the activities carried out in two residential establishments with seniors with dementia. For my research, I have chosen a qualitative approach and a semi- structured interview technique with residential workers. The output from the research is a discussion in which I describe the comparison of theoretical and factual information and propose other possibilities for research in this field.

Objectives: The aim of this non-interventional observational study was to determine the nutritional statu s of long-term hospitalized pacients in a psychiatric hospital using anthropometric indicators of nutritional status. Assess the presence of overweight, obesity and pathological waist circumference in patients with schizophrenia. Evaluate the incidence of malnutrition and risk of its development in patients with dementia. Methods: The data were obtained by measuring the selected patients in Psychiatric hospital Kosmonosy, were collected data on body weight, arm circumference, waist and calf. Data were completed by information from nursing records - body height of the patient, age, length of hospital stay and used antipsychotic medication. The data of weight and height were used to calculated BMI. Results: The study included a total of 121 patients, of whom 56 (46.28 %) men and 65 (53.72 %) women. Patients were divided into two groups by diagnosis, the first group of schizophrenia, and schizoaffective disorders (93 patients) and another group with a diagnosis of dementia (28 patients). In the first group, the incidence of overweight was present in 26.88 % of patients, obesity in 25.81 % of patients and abdominal obesity was present in 79.57 % of patients. The aim of this diploma thesis is to implement a presentation layer for the Reminiscence Computer Therapy application. Reminiscence Therapy involves the discussion of the past memories and it is believed to help people with dementia to improve their well-being and sustain their memories. Computer Reminiscence Therapy involves technology in this therapy. The research introduces into Dementia and Reminiscence Therapy with and without using technologies. The application developed as part of this diploma thesis uses picture upload and speech recognition techniques for the memory input. This is used to support remembering of past memories and to increase discussion. The application can be used with or without the carer as well as with friends and family members. The six most important requirements on the developed application are the speech recognition, picture upload, asking questions about the previous recordings, asking questions about tagged friends, using old stories for discussion and presenting the statistics. The application was developed as a part of the competition made by AngelHack organization. Presentation, business and persistence layers of the application are discussed in this diploma thesis as well as user manual and persistence layer implementation of the requirements. The presentation layer was developed using React framework for JavaScript. Other libraries have to be used as well with the most important being Redux library for storing application state. This library affected the structure of the persistence layer.

Alzheimer's disease is a neurodegenerative disorder with the highest prevalence in the population and for which we do not have a cure so far. The aim of this thesis was to test the mediator system of the N-methyl-D-aspartate receptor and nitric oxide in an animal model of sporadic form of Alzheimer's disease (Samaritan Alzheimer's Rat Model; Taconic Pharmaceuticals, USA). Then compare these results with changes in hippocampal cholinergic system and cognitive tests. The Samaritan rat model is based on the unilateral in vivo application of β-amyloid42 and the pro-oxidative substances (ferrous sulfate heptahydrate and L-buthionine-(S,R)-sulfoximine). Neurochemical methods included testing of the NR1/NR2A/NR2B subunits of the N-methyl-D-aspartate receptor and activity of nitric oxide synthases (neuronal, endothelial, inducible) in the cortex, in both cases in the right and left hemisphere separately. Our results show that Samaritan rats exhibited significant changes in expression of NR2A/NR2B subunits of the N-methyl-D-aspartate receptor and activity of inducible nitric oxide synthase in cortex compared to control rats. The results of glutamatergic system are consistent with changes in activity of cholinergic transporter and cognitive tests (Morris water maze and active allothetic place avoidance)....

This This thesis concentrates on the issues faced by family carers, who take care of their family members diagnosed with the Alzheimer's disease. Special education also relates to seniors and its principles can therefore be used in this age group. The goal of this thesis is to discover and describe challenges faced by the family carers while they support their family members with Alzheimer's. The thesis focuses on the Alzheimer's disease itself, its causes and risk factors. The thesis also concentrates on the origins of the disease, its stages, diagnostics and therapy. Next, psychic and physical burdens of the family carers are depicted. The thesis also introduces organizations that specialize in providing informal support for the family carers and relief care, and disease related legislation is listed. Title: Estimation of normative values for the semantic verbal fluency test in older adults over 50 Objective: The aim of the diploma thesis was to create normative values for the semantic verbal fluency test (animal category) for older adults over 50 years, depending on the level of education achieved. Methods: The diploma thesis is constructed as a cross-sectional study. All data were obtained from the international project Survey of Health Aging and Retirement in Europe (SHARE). The descriptive statistics of participants were produced with calculated means and standard deviation for continuous variables and percentage for categorical variables. The median and interquartile range for age and semantic verbal fluency test (animal category), and 2nd , 7th , and 16th percentile as the cut-off points for verbal fluency test (animal category) were calculated for participants divided into ISCED - 97 education levels and for each country separately. Results: Data from 64,242 participants aged 66.4 were analyzed, and the female ratio was 55.8 %. All respondents were older adults over 50 years old from 20 European countries. This thesis deals with effects of some chosen 7-methoxitacrine (7-MEOTA) analogues on enzymatic activity of acetylcholinesterase (AChE). 7-MEOTA is a derivative of tacrine, which had been used for symptomatic treatment of Alzheimer's disease (AD), until drugs with better therapeutic index were developed. 7-MEOTA the same way as tacrine therapeutically acts by inhibition of acetylcholinesterase and a neurotransmitter acetylcholine rise in the organism. It shows similar strength and type of inhibition, but it's less toxic contrary to tacrine. Some of the previously examined analogues of 7-MEOTA were as strong or even stronger AChE inhibitors than 7-MEOTA and so promising future medicaments. However, all the compounds analyzed in this thesis showed weaker enzymatic reaction inhibition and AChE affinity. For each of the examined compounds IC50, Ki and Ki' were calculated and AChE inhibition type was determined. All the 7-MEOTA analogues showed a mixed type of the inhibition. The theoretical part of this thesis deals with manifestations and origins of AD, its genetic factors etc. and tries to show some of the anthropological findings a theories connected with the theme.

1 Annotation This thesis is focused on the issues of burnout syndrome and supervision as a form of prevention. The topic is focused on staff dedicated to the seniors with dementia in day-care centers. The thesis is divided into two parts. The theoretical part is devoted to the definition of basic terms related to burnout syndrome, dementia, daily care center and supervision. Results in the practical part come from the research methods used, specifically self-prepared questionnaire and questionnaire prepared by Ch. Maslachová and Ayala Pines "Tedium Measure". Furthermore I expanded the thesis with interview with selected expert. Using open questions the area of supervision and burnout syndrome is being discussed. The primary target of this thesis is to contribute to the prevention of burnout syndrome and also to map, whether supervision is being used in day-care facilities and whether it helps preventing the burnout syndrome.

Alzheimerova choroba a prostředí intenzivní péče / Alzheimer's disease and department of intensive care

Introduction: The thesis "Alzheimer's disease and department of intensive care" deals with the specifics and difficulties of nursing care for patients with this chronic and degenerative disease in intensive care units (ICUs). The number of patients with Alzheimer's disease is constantly growing and caring for them in such specific wards is extremely demanding. The purpose of this thesis was to map the most problematic areas in the care provided and propose their solution. Methods and results: The empirical part presents the results of a conducted qualitative research which took place in the form of interviews with 10 general nurses who have experience with such patients. Following a word-for-word transcription of the obtained interviews, the results are summarized in a final discussion. They show that when caring for a patient with dementia, general nurses see the main problems in his decreased ability to cooperate, behavioral changes, which often appear as a result of environment change, and difficult communication. They consider the patient's confusion, his negativism, and the inability to find the right words to be communication barriers. This results in enormous demands on the caring staff's patience.
