Neste ínterim, o objetivo deste estudo foi avaliar a correlação entre a prevalência da estomatite por prótese total, o grupo sanguíneo ABO, presença de Candida ssp. e a higienização da prótese total. Foram avaliados 30 pacienttes (média de idade de 61,13 +-11,96 anos) portadores de prótese total superior, sendo que 23 eram do sexo feminino. O exame clínico incluiu a avaliação da mucosa do palato e a avaliação da higienização da prótese total. A qualificação da higiene da prótese total foi feita por meio da visualização do biofilme com o uso de evidenciadores de placa, nas superfícies internas das próteses. As amostras micológicas foram obtidas da mucosa do palato (região central), da superfície interna da prótese total (região central) e do dorso da língua, por meio da técnica de impressão (imprint), utilizando discos de filtro de papel, colocados diretamente em placas de cultura contendo meio seletivo CHROMagar Candida. As espécies isoladas foram identificadas por meio da prova de produção de clamidoconídeo e pelo sistem API 20C AUX, além da pré-identificação fornecida pelo uso do meio seletivo. Amostra sanguínea foi coletada para determinação do tipo sanguíneo do paciente (sistema ABO e Rh). As análises estatísticas dos resultados foram feitas utilizando testes não paramétricos (Spearman, Mann-Whitney e Kruskal-Wallis). Em nosso estudo, 53,33% dos pacientes apresentaram estomatite por prótese total e a identificação de Candida ssp. foi positiva para 80% dos pacientes. O grau de higiene das próteses... / Although the etiology of denture stomatitis has been commonly related to Candida albicans, some previous studies indicate that the infection could be associated to other factors. The aim of this study was to evaluate the correlation between the prevalence of denture stomatitis, ABO blood groups, mycological status and denture cleanliness. Thirty patients with mean age of 61.13+-11.96 yeas being 23 female using complete maxillary dentures were investigated. Oral examinations included clinical diagnosis of denture stomatitis as well as the evaluation of denture cleanliness. The denture cleanliness was assessed using a stain as plaque detector in the fitting denture surface. The yeast cultures from the palatal mucosa, fitting denture surface and dorsum of the tongue were obtained using an imprint at CHROMagar Candida. The determinations of the species of the isolates was performed by chlamydospore formation and sugar assimilation using API 20C AUX. Venous blood was collected for ABO blood group determination. Spearman correlation, non-parametric t test and analysis of variance were performed to analyse the data. In this study, 53.33% of the patients presented denture stomatitis and the identification of Candida spp. was positive in 80% of the samples. The denture clean liness was classified as poor in 93.33% of the dentures. A statistical correlation was found between yeast detection and denture stomatitis (r=0.8250; p<0,0001) and between Candida ssp. and denture cleanliness (r=0,4666; p=0,0248). Although, majority of the patients were from blood group O no statistical correlation was found between clinical variables and mycological results. Avaliação in vitro da citotoxicidade de monômeros, plastificante e produtos de degradação liberados a partir de resinas para reembasamento imediato /

Resumo: O presente estudo teve como objetivo avaliar o efeito citotóxico dos monômeros isobutil metacrilato (IBMA) e 1,6 - Hexanediol dimetacrilato (1,6 - HDMA), do plastificante di-n-butil ftalato (DBP), e dos produtos de degradação ácido metacrílico (AM) e ácido benzóico (AB) sobre células L929. Esses compostos foram testados em faixas de concentrações liberadas, em um período de 30 dias, por materiais reembasadores rígidos, previamente quantificadas em estudos anteriores. O efeito citotóxico foi verificado por meio dos testes de MTT e 3H-timidina, após as células terem sido expostas às substâncias testadas nas concentrações estabelecidas. A classificação da citotoxicidade foi baseada na viabilidade celular em relação ao controle (células expostas ao meio sem as substâncias testadas). A atividade de síntese de DNA foi inibida por todos os compostos. Os resultados do presente estudo demonstraram que o teste de 3H-timidina foi mais sensível que o teste de MTT e que os compostos avaliados mostraram diferentes níveis de citotoxicidade in vitro. A atividade da desidrogenase mitocondrial diminuiu nas células tratadas com os monômeros, o plastificante e o produto de degradação AM; porém, para o AB, a maioria das concentrações testadas não apresentou efeito citotóxico. / Abstract: The aim of this study was to evaluate the cytotoxic effect of the monomers 1,6 - hexanediol dimethacrylate (1,6 - HDMA) and isobutyl methacrylate (IBMA), the plasticizer di-n-butyl phthalate (DBP), and the degradation by-products methacrylic acid (MA) and benzoic acid (BA) on L929 cells. These compounds were tested in the range of concentrations released in 30 days from hard chairside reline resins that were quantified in previous investigations. Cytotoxic effects were assessed by using MTT and 3H - thymidine assays after the cells had been exposed to the test compounds at the given concentrations. Cytotoxicity was rated based on cell viability relative to controls (cells exposed to medium without test substances). The results presented in this study demonstrated that the 3H-thymidine assay was more sensitive than the MTT assay, and all compounds tested showed varying degrees of cytotoxic effect in vitro. DNA synthesis activity was inhibited by all compounds. Minimal inhibitory concentration of antimicrobial and antifungal agents in denture adhesive material against Candida albicans

Approximately 26% of the U.S. population between the ages of 65 and 74 years are completely edentulous. Of the different proposed predictors and risk factors, low income and education levels have the highest correlation with tooth loss. While the incidence of complete edentulism in the United States has progressively declined over the past decade, the continued growth of the population strongly suggests that edentulism prevalence will likely remain constant or increase over the next few decades. In patients wearing complete prosthetic appliances, several post-treatment complications may arise, including denture associated Candida species infections and mucosal stomatitis. These type of fungal infections are associated with patient-reported symptoms (e.g. pain or discomfort) and may impede normal oral function. In this study the activity of 11 (antimicrobial and/or antifungal) agents in a dental adhesive carrier against two strains of C. albicans was assessed. In conventional minimal inhibitory concentration (MIC) assays, C. albicans were resistant to histatin 5 and lactoferricin B, yet very susceptible to SMAP28; susceptible to long chain bases sphingosine, dihydrosphingosine, and phytosphingosine; and susceptible to anti-fungal agents amphotericin B, chlorhexidine dihydrochloride, chlorhexidine gluconate, fluconazole, and nystatin. However, in 1% dental adhesive (final concentration) C. albicans were resistant to histatin 5, lactoferricin B, SMAP28, sphingosine, dihydrosphingosine, and phytosphingosine suggesting that the components in denture adhesive may inactivate local innate immune factors in the oral cavity possibly predisposing users to fungal infections in relation to their dental prostheses. In MIC assays in 1% dental adhesive (final concentration) C. albicans were susceptible (p value < 0.05) to amphotericin B, chlorhexidine, dihydrochloride, chlorhexidine gluconate, fluconazole, and nystatin strongly suggesting that these anti-fungal agents could be candidates for inclusion in denture adhesive formulations, and also be used as a prescribed topical treatment in individuals with fungal infections of the oral mucosa.

Oral health technology assessment : study of mandibular 2-implant overdentures

There is little evidence that Health Technology Assessment (HTA) is much used in dentistry. Dental implant technology is an example of innovative oral health technology. The objectives of this research were to gather the evidence needed for the assessment of overdenture implant treatment so that both patients and dental practitioners can make informed decisions about this technology. These objectives included 1) investigating what types of dental clinicians adopt and provide dental implants 2) determining the effect of the clinicians' experience in the provision of implant supported prostheses and 3) measuring the patients' preference in provision of mandibular 2-implants overdenture technology. / For the first part, a cross-sectional survey was sent to all licensed Canadian Dentists to measure the adoption and provision of implant technology. For the second part, we used the data on the first 140 edentulous elders who were enrolled in a randomized controlled clinical trial to compare the effects of mandibular conventional (CD) and 2-implant overdentures (IOD) on nutrition. The change in patient ratings of satisfaction after treatment, laboratory costs and the number of unscheduled visits were compared. For the last part, edentulous elders (N=36) who were wearing maxillary dentures and either a mandibular conventional denture (CD, n=13) or a two-implant overdenture (IOD, n=23) participated in this study. Participants' preference was measured during a 20-minute interview. / Multivariate regression analysis on the data from the first part of the study shows that the Dentist's gender, province of practice, specialty, and whether they practice alone or in association with other practitioners, are significantly associated with the adoption of implant technology (p&lt;0.05). It is also shown that there was no difference in satisfaction scores for either prosthesis between the groups treated by experienced specialists or new dentists. Furthermore, it is shown that IOD wearers were willing to pay three times more than the current cost of conventional dentures for implant prostheses (p&lt;0.05). / Overall, the results of this study 1) inform decision makers on what types of clinicians provide implant technology and 2) suggest that, with minimal training, all dental clinicians irrespective of their specialty, can provide successful implant overdenture prostheses that edentulous patients are willing to pay for.
