Diagnóstico das reações liquenóides de contato envolvendo teste de hipersensibilidade de contato ao amálgama e tratamento pela substituição do material restaurador / Diagnosis of oral lichenoid reactions based on skin patch test and treatment by amalgam replacement

O objetivo do presente estudo foi diferenciar reações liquenóides de contato (RLC) de lesões de líquen plano oral (LPO) em pacientes do nosso ambulatório. De um total de 45 pacientes (36 mulheres e 9 homens; média idade = 48,7 anos) com lesões sugestivas de LPO, 18 apresentavam características clínicas compatíveis com RLC, ou seja, lesões próximas ou em contato direto com restaurações em amálgama. Estes pacientes foram classificados em classe I (discreta relação da lesão com amálgama), classe II (moderada relação) e classe III (intensa relação). Todos os pacientes com RLC foram encaminhados para realização de teste epicutâneo padrão (alérgenos \"standard\", Grupo Brasileiro de Dermatites de Contato) e específico (materiais odontológicos, Trolab, Hermal, Alemanha). A substituição do amálgama foi indicada para pacientes classe II e III; em pacientes classe I o tratamento foi indicado somente quando apresentavam teste epicutâneo positivo. Dois pacientes foram excluídos deste estudo por não comparecimento às consultas e 1 por não apresentar resultado anátomo-patológico compatível com LPO. Em pacientes classe I (3 casos), apenas um caso apresentou teste epicutâneo positivo, sendo que a substituição da restauração não resultou em melhora clínica da lesão. Em pacientes classe II (9 casos), teste epicutâneo positivo em 8 casos, a substituição resultou em melhora parcial em 7 casos e total em 2 caso. Em pacientes classe III (3 casos), teste epicutâneo positivo em 2 casos, a substituição resultou em melhora total de todos os casos. Existe um número representativo de pacientes com RLC em nosso meio, sendo que estas lesões podem apresentar associação discreta, moderada ou intensa com o amálgama. Pacientes com associação discreta (classe I) não têm benefício após substituição do material restaurador, enquanto pacientes classe II e III beneficiam-se após substituição do amálgama, obtendo resolução parcial ou total das lesões. A caracterização clínica de lesões LPO associada ao amálgama seguida da substituição do material restaurador tem valor superior aos testes epicutâneos em termos de diagnóstico de RLC. / The purpose of this study was to characterize oral lichenoid reactions (OLR) in our clinic. In a group of 45 patients (36 female and 9 male; mean age = 48.7) with oral lichen planus (OLP), 18 were identified as having OLR because these patients had their OLR lesions related to amalgam fillings. These OLR lesions were clinically graded according to their proximity with the amalgam fillings: class I (weak association with amalgam fillings), class II (moderate association), and class III (strong association). All patients were skin patch tested for both standard allergens (Standard - Brazilian Group of Contact Dermatitis, Brazil) and specific allergens (Dental material - TROLAB, Hermal, Germany). Amalgam restoration was replaced in all class II and III patients and in class I only when patients were positive skin patch test. Two patients were excluded from the study because they missed follow ups and one because histopathological analysis does not show OLP features. From 3 cases class I only 1 had positive skin patch test, but did not have improvement of the oral lesions after amalgam replacement. Class II patients (9 cases) positive skin patch test in 8 cases, resulted in substantial improvement in 7 cases and complete resolution in 2 case after amalgam replacement. Class III patients (3 cases) positive patch test in 2 cases, result in complete improvement in all cases after amalgam replacement. We can conclude that there are a representative number of patients with ORL in our clinic showing either a weak, moderate or strong association with amalgam fillings. Patients with a weak association (class I) did not have benefit after amalgam replacement, while patients with a moderate and strong association (class II and III) had benefit after amalgam replacement with substantial improvement or complete resolution of the lesions. Clinical characterization of the association between OLP lesions and amalgam filling, followed by its replacement, outperformed skin patch testing for diagnosing ORL lesions.

Funkcionalne karakteristike smeša maltodekstrina i namenske masti kao komponenti niskoenergetskih masnih punjenja za konditorske proizvode / Functional characteristics of vegetablefat/maltodextrin gel blends as a component for low calorie confectionery fat fillings

<p>Masna punjenja predstavljaju grupu proizvoda koje se<br />koriste u konditorskoj i pekarskoj industriji. Kako ona<br />najče&scaron;će sadrže između 30-40% masti, teži se dobijanju<br />formulacija sa redukovanim sadržajem masti. To se<br />može postići zamenom dela masne faze uz upotrebu<br />zamenjivača masti, najče&scaron;će hidrokoloida koji imaju<br />osobine slične osobinama zamenjene masti.<br />Maltodekstrini i njihovi geli mogu se koristiti kao<br />zamenjivači masti. Oni predstavljaju produkte hidrolize<br />skroba koji su rastvorljivi u vodi, mogu da poseduju<br />osobine formiranja gela, imaju slične mazive osobine<br />kao i masti i blagi i neutralni ukus.<br />U ovom radu ispitivane su reolo&scaron;ke osobine različitih<br />maltodekstrina kao i sme&scaron;a namenske biljne masti i<br />rastovora/gela maltodekstrina u odnosu 9:1. Na<br />osnovu dobijenih rezultata utvrđeno je da su od svih<br />ispitivanih komercijalnih maltodekstrina,<br />maltodekstrini <em>01970 </em>i <em>N-Lite D</em> sa namenskom biljnom<br />masti davali stabilne sisteme. Stoga su maltodekstrini<br /><em>01970</em> i <em>N-Lite D</em> kori&scaron;ćeni za dalja istraživanja.<br />Namenska biljna mast zamenjivana je sa 15 i 20%<br />gelima maltodekstrina <em>01970</em> i <em>N-lite D</em> u tri različita<br />odnosa: 16,7% gela maltodekstrina i 83,3% namenske<br />biljne masti; 33,3% gela maltodekstrina i 66,7%<br />namenske biljne masti i 50% gela maltodekstrina i<br />50% namenske biljne masti. Određivane su reolo&scaron;ke<br />osobine metodom histerezisnog postupka,<br />viskoelastične osobine, kao i teksturne osobine<br />pripremanih sme&scaron;a. Dobijeni rezultati su ukazivali da<br />prisustvo maltodekstrinskih gela u sme&scaron;i sa<br />namenskom biljnom masti utiče na pad tiksotropnosti i<br />čvrstoće i povećanje prividnog viskoziteta i<br />viskoelastičnosti ispitivanih sistema. Sve sme&scaron;e<br />namenske biljne masti i gela maltodekstrina služile su<br />kao komponenta masne faze za pripremu masnih<br />punjenja sa smanjenim sadržajem masti. Povećanjem</p><p>zamene dela masne faze maltodekstrinskim gelima u<br />masnim punjenjima uzrokovalo je povećanje<br />tiksotropnosti i čvrstoće kao i promenu boje krajnjeg<br />proizvoda. Na osnovu senzorske ocene sprovedene od<br />strane iskusnog senzorskog panela i na osnovu testa<br />prihvatljivosti od strane netreniranih potro&scaron;ača,<br />najbolje ocenjena masna punjenja bila su ona kod kojih<br />je 5% masti zamenjeno maltodekstrinskim gelima.<br />Međutim, na osnovu rezultata testa prihvatljivosti<br />uočeno je da su i sistemi sa 15% zamene masti bili<br />senzorski prihvatljivi. Takođe je i na osnovu rezultata<br />sprovedenog upitnika na uzorku od 250 ispitanika<br />utvrđeno da su potro&scaron;ači (64,4%) zainteresovani za<br />konzumiranje konditorskih proizvoda sa smanjenim<br />sadržajem masti.</p> / <p>Confectionery fat fillings are products used in<br />confectionery and baking industry. They contain<br />approximately 30-40% of fat. Due to the high fat<br />content, fat fillings have considerable high caloric<br />value. Therefore, there is a trend to decrease fat<br />content in fat filling formulation. That can be achieved<br />by replacing the portion of the fat phase with fat<br />replacers, most often with hydrocolloids having<br />characteristics which tend to mimic properties of<br />replaced fat. Maltodextrins and their gels can be used<br />as potential fat replacers. Maltodextrins represent<br />starch hydrolysis products soluble in water. Also they<br />posses gel forming ability, spreading ability similar to<br />fats, and bland and non sweet taste.<br />Rheological properties of different types of<br />maltodextrins were examined. Also, blends prepared<br />with 90 % of vegetable fat and 10% of maltodextrin<br />solution/gels were rheologically characterised.<br />According to obtained results, it was found that among<br />all tested commercial maltodextrins,<em> 01970</em> and <em>N-Lite<br />D</em> built stable systems. Therefore, they were used in<br />further measurements.<br />Vegetable fat was partially replaced by 15 and 20% of<br /><em>01970</em> and <em>N-Lite D</em> maltodextrin gels in three different<br />ratios (16.66%, 33.33% and 50% respectively). Steady<br />shear, dynamic oscillatory measurements as well as<br />textural measurements were performed. The obtained<br />results indicated that the presence of maltodextrin gels<br />in blends with vegetable fat influenced the decrease of<br />thixotropy and hardness and increase of viscosity and<br />viscoelasticity of the tested systems. All maltodextrin<br />gel/vegetable fat blends were further used for</p><p>preparation of confectionery fat fillings. The increase in<br />the amount of fat reduction resulted in the increase of<br />thixotropy and hardness and change in colour of the<br />final product. According to sensory analysis performed<br />by trained sensory panel as well as by measurement of<br />product acceptance&ndash;preference performed by<br />untrained panellists (consumers), fat filling with 5% fat<br />reduction had the highest scores. However, according<br />to product acceptance&ndash;preference measurements it<br />was estimated that the final product with 15% fat<br />reduction was also sensory acceptable. The results of<br />survey performed on 250 participants showed that<br />consumers (64.4%) were interested in consuming low<br />fat confectionery product</p>

The Bulletin, 97, 29-33. / The longstanding debate over the safety of mercury in dentistry has latterly moved from scientific argument to public health dilemma. Mercury is a neurotoxin. Adverse psychological outcomes can result from exposure, so The Quicksilver Quest aimed to investigate mercury in dentistry from a qualitative, critical health psychology perspective, and a quantitative, neuropsychology assessment. The qualitative study used focus group methodology to explore micro-mercurialism linked to dental amalgam fillings. A random sample of people, who had been medically diagnosed with mercury poisoning, formed seven focus groups. The discussion of experiences, beliefs, and health was analysed for themes and issues. The main findings were that the participants were not a homogeneous group, as had been anticipated, but fell into categories differentiated by their symptoms, fiscal resources, and motivation. A placebo effect was rejected as an exclusive explanation for the positive health outcomes reported by those who had had amalgam removal and detoxification. The quantitative study investigated the long-term effects of occupational mercury exposure on a cohort of women in dentistry. The aim was to test the null hypothesis: that women who endured high occupational mercury exposure in the 1970s (43 participants), and matched controls (32 participants), would show no between-group differences on a general and reproductive health survey, and a nine-test neurobehavioural assessment. Results generally supported accepting the null hypothesis. Significant exceptions were current symptom experience, reproductive health, and two mood subscales. There was a suggestion of peripheral nerve damage in the exposed group. Overall, the general discussion systematically reviews tension points in the debate, in light of a proposed model of tolerance to mercury. This begins to explain how it might appear that mercury in dentistry is safe for dental personnel, as pro-amalgam debaters claim, yet unsafe for some dental patients, as anti-amalgam debaters claim. Further study is suggested for occupationally exposed women, on tremor, and to test the proposed tolerance to mercury model. Finally, as the debate has a political aspect, a recommendation is made for a shift in public health policy to dental amalgam being restricted to use only in an adult population.

The theme of the diploma thesis is the preparation of project documentation for the construction of two designed buildings of the Primary School. Building plot for the planned new building is located near the existing nursery school near the center of Vracov. The campus is designed to provide basic education in a total of 18 classes with a proposed total capacity of 540 pupils. The complex consists of 4 main buildings that functionally divide individual operations. These are designed as free-standing, SO01 and SO04 objects will be two-storey, without basement. SO02 and SO03 objects as four-storey without basement. The construction system is designed as a prefabricated reinforced concrete skeleton with filling masonry from ceramic blocks to thin-walled masonry mortar. The ceiling construction consists of pre-stressed ceiling panels SPIROLL in combination with reinforced concrete. The staircases connecting the floors are designed in reinforced concrete. Vertical communication between floors will also be made possible by barrier-free passenger lifts. The roof cladding consists of a single-shell construction with thermal insulation made of EPS, the covering will be made of PVC foil weighted with a layer of peacock. The building will be insulated with a façade thermal insulation system with a ventilated gap and external surface treatment of HPL façade cladding panels. Paved pedestrian roads will be made of concrete interlocking pavement. The road for vehicle traffic will be with an asphalt surface. Parking spaces are designed from drainage concrete pavement. The project documentation was processed in ArchiCAD.

In my diploma thesis I designed a new office building in Brno. The building will function as a business incubator, so it will provide space for developing companies for favorable lease terms. While projecting the building I took into consideration future changes in dispositions. Part of the building has a basement and it has one underground and four aboveground floors. The building is designed as monolithic ferroconcrete unit and it has reinforcing perimeter walls with unit core. The ceiling is made of monolithic ferroconcrete panels. The building has flat rooftops, ventilated facade and is based on piles.

Les matériaux d'obturation et de coiffage pulpaire des dents lactéales

Les soins pulpaires en denture lactéale sont les soins fréquemment réalisés en odontologie pédiatrique et la réaction pulpaire ainsi que les mécanismes de réparation tissulaire lors d’un traumatisme ne sont pas très bien connus ou investigués. En se référant à la littérature dentaire, on constate que les recherches sont souvent focalisées sur la denture définitive.<p>Objectifs<p>1) Etudier l’influence du traitement avec divers matériaux sur la formation dentinaire, la structure tissulaire et le degré d’inflammation.<p>2) Acquérir des connaissances sur l’interaction à l’interface matériau/tissu.<p>3) Etudier le rôle de la voie de signalisation Notch (1 et 2) dans le processus de remaniement et réparation du tissu pulpaire en cas de traumatisme.<p>Matériels et méthodes<p>Deux types de soins pulpaires, la pulpotomie et le coiffage direct, sont réalisés sur les dents lactéales porcines et les résultats sont étudiés après 3 périodes :7 jours, 21-28 jours et 90 jours.<p>La pulpotomie :8 matériaux, 10 dents par matériau, 2 matériaux par cochon et par période pour un total de 240 dents.<p>Le coiffage direct :10 matériaux, 10 dents par matériau, 2 matériaux par cochon et par période pour un total de 300 dents.<p>Par la suite, différentes techniques ont été utilisées :<p>1. La microscopie optique :étude des coupes histologiques après coloration.<p>2. La microscopie électronique à transmission :observer les caractéristiques morphologiques du tissu pulpaire et analyser les structures cellulaires en détail. L’objectif principal de cette recherche était centré sur les biomatériaux pour la période de 21-28 jours afin d’examiner l’interaction entre le biomatériau et le complexe dentino-pulpaire après leur placement.<p>12<p>3. La voie de signalisation Notch (1 et 2): investiguer l’intervention éventuelle de cette voie dans le processus de régénération tissulaire par différenciation des cellules pour les deux périodes de 7 et 21-28 jours pour tous les matériaux utilisés.<p>Résultats<p>A. Microscopie optique :<p>1. La pulpotomie :les biomatériaux provoquent moins d’infiltrat de cellules inflammatoires au niveau du tissu pulpaire et favorisent la déposition dentinaire.<p>2. Le coiffage direct :on obtient les mêmes résultats que dans le cadre de la pulpotomie.<p>B. Microscopie électronique en transmission :<p>1. L’induction du tissu calcifié ou la formation de néo-dentine s’est seulement produite au niveau du site de l’exposition après le placement des biomatériaux dans les 2 types du traitement (pulpotomie & coiffage direct). En revanche aucune formation du tissu calcifié n’a été observée dans le parenchyme pulpaire à distance du site de l’exposition.<p>2. Cet examen montre que les cellules en contact des biomatériaux ou même à proximité de ces derniers présentent un réticulum endoplasmique élargi parallèle à la longueur de la cellule. Des mitochondries, plusieurs appareils de Golgi et des éléments denses sont observés dans le cytoplasme cellulaire.<p>3. Cet examen a également montré que le MTA et le WPC, considérés par la littérature dentaire comme non-résorbables, sont phagocytés par les cellules histiocytaires.<p>C. Voie de signalisation de Notch :<p>Nos recherches n’ont pas montré de marquage concluant pour aucun de matériaux sauf dans le cadre de coiffage direct pour les dents traitées au MTA et Ca(OH)2 pour la période de 7 jours. The goal of this diploma thesis is to create a building technology project for construction of 4 flat houses within stage 3a. of the building project „Green town of Prague – Jarov, phase II“. The flat houses are in two pairs interconnected by basement, each is a 7 floor high reinforced concrete building with flat roof and is functionally connected to others by a terrace garden system. This document focuses on organisation and preparation of the construction. It includes building site equipment, cost calculation for setting up and servicing of the building site with graphical illustrations for selected construction phases. Additionally a detailed time schedule was created with a proposal for stream construction of the reinforced concrete scelet and lining in the pairs of interconnected buildings. A realization study of the main technological phases was also prepared. The diploma thesis covers in detail construction of reinforced steel scelet and circumferential casing including a layer of contact insulation system. A blueprint of formwork for walls and ceiling is included in the manual for construction of reinforced steel scelet. On these particular actions probationary and control plans were added. The required machinery set with detailed placement was also drafted. An itemized budget for the main building was prepared as well. Attached to this document is a detailed manual on how to use housing units including repairs and revisions plan for next 30 years. Part of this diploma thesis is also a detailed study covering the construction in relation to the surronding living and educational areas.

The project solves a multifunctional Duma building in a vacant lot, contemplated the construction site is located in Carlsbad, in the street Vyhlíce. This is a protected site spa. Part of the project's layout and structural design of the house. It is a six-storey house with an attic and a basement floor. It is designed as a free-standing in the gap as the final house. The layout is divided into two complete units with their own input. There are spaces for business and residential units for permanent housing. Part of the living area are also room house equipment. Inputs to both parts are wheelchair accessible. The house is not wheelchair The house is designed as a brick building of brick masonry Porotherm the module dimensions of 250 (125) mm with reinforced concrete ceilings. Roofed by a hipped roof. The house is located on a private plot of 519 m2 built-up area of 221 m2. The land is gently sloping. The main orientation of the building to the cardinal's east and west. The south wall is adjacent to the neighboring house.
