Problematika ošetřovatelské péče v domovech pro seniory / The Issue of Nursing Care in Old People's Homes

The thesis deals with the issue of nursing care in old people's homes. Given the improving medical and nursing care and also the prevention of diseases, the human life extends. This leads to the aging of population. Therefore, it is essential to understand this issue, its interconnection with nursing care, but also to find the right approach to the elderly in residential facilities providing social services. The theoretical part of the thesis deals with old age and aging, behavioural changes in old age, specifics of communication with the elderly, long-term care for the elderly, care for a man in nursing homes. It deals with the work of nurses in old people's homes and with their education. Five objectives were set within the framework of the research. The first objective was to determine areas of nursing care, in which the nurses working in old people's homes have most problems. The second objective was to find out how the nurses manage physical strain. The third objective was to determine how the nurses manage psychical strain. The fourth objective was to find out what theoretical knowledge and practical skills the nurse must have. The fifth objective was to identify possibilities of further training for nurses working in old people's homes. Qualitative research was applied in the empirical part of the thesis. The method of non - standardized interview was used. The research group consisted of 10 nurses who work in residential facilities for the elderly in old people's homes. To process the interviews, the open coding technique was applied - a paper-and-pencil method. To complement psychical resilience of nurses, S.O.C questionnaire by A. Antonovsky was used. There were 42 questionnaires used in the research. The interviews with the respondents showed that the work of a nurse in homes for the elderly is physically and mentally demanding. And it is obvious that nurses feel the mental strain as more damaging. The physical demands are connected with the state of the client and his diseases but also with the space limitations of homes and night shifts. The mental burden is caused by similar factors and also by the family of the client, specifics of the communication with the elderly, cooperation with the hospital, working atmosphere, responsibility and keeping records. Next important factor that negatively influences mental state of the nurses is a lack of prestige and recognition of their work. Even though the nurses feel both types of stress, most of them cope with it without problems and they try to restore their balance in their leisure time. The conclusions of the thesis also point to the high demands and wide range of expertise and skills required from the nurses when providing care. Even specialized procedures are carried out in homes. Nurses must know how to communicate, handle conflict situations, co-operate with colleagues from different fields. Great emphasis is placed on independence in evaluating the health state of the client while ensuring the nursing care. The nurse must have personal qualities for work with the elderly. Another important component of their work is their knowledge and practical skills in the field of social work theoretically backed by the Act 108/2006 Coll.

The thesis on Information Technology in Nursing Education focuses in the theoretical part on defining the history of nursing education and defines the differences with the education of the present, also points to legislative norms related to nursing education. It then describes the learning process and defines the concepts of online education and e-learning. Two objectives and two hypotheses have been identified within the research part of the thesis. The first objective was to determine the use of online education by general nurses. The second objective was to determine the use of e-learning by students of both daily and combined forms. Therefore, two hypotheses were set for these objectives: H1: Higher education nurses use online learning more than non-college nurses and also H2: Students in the combined form of study use e-learning more than full-time students. Hypotheses have been set for chi quadrate testing and are included in the thesis including an alternative and zero hypothesis. The data for the quantitative research of this diploma thesis was obtained through two non-standardized questionnaires. The first questionnaire focused on the use of eLearning in students of both the day and the combined form of study, and the second was designed to identify the use of online nursing education. Both questionnaires were structured and contained both open and closed questions. The questionnaire survey was anonymous. Respondents were addressed throughout the Czech Republic. It was a deliberate choice. The first questionnaire contained 29 questions and the return was 387 questionnaires, of which 4 questionnaires were excluded due to incomplete or incorrect filling. The second questionnaire contained 31 questions, returning 246 questionnaires, of which 6 questionnaires were excluded due to incomplete or incorrect filling. A quantitative research survey was conducted between March and April 2017. Both questionnaires were distributed only electronically through social networks in sister and healthcare groups and societies. The research has confirmed the hypothesis that students use e-learning for learning, and in particular, students from the combined form of study use it more than in full-time study. Nurses do not use online lessons, but college-educated nurses use it more than high school educated. Among the barriers to education, the two groups of respondents most often have the same items, namely lack of time, lack of motivation and lack of courses that the respondents would be interested in. For the benefit of eLearning and online learning, compared to traditional forms of education, both groups agree on the positive values of consensus. In terms of the positive aspects of these forms of education, the two groups again agree on the low financial demands of both forms, the benefits of home accessibility and the possibility of selecting a topic according to one another. The groups also agree with the limitations of the studied forms of education - they refer in particular to the focus of the courses on the theory and the inability to test the findings in practice.

Bezpečná ošetřovatelská péče / Safe Nursing Care

Patients{\crq} safety is a global problem and we are increasingly thinking about the possibilities of improving the quality of care provided. This is a priority for all health care providers, including nurses. We must not forget the patients and their close family because they are our partners and the nurses should seek to ensure that the patients themselves are actively involved in the care of their health and play an important role in preventing some unwanted errors. The theoretical part of the thesis is devoted to a detailed analysis of the concept of safety and safe care and the associated negatives. The goal of the thesis was to determine which resources are used to maintain the level of safe nursing care by nurses with specialized training. Six hypotheses were determined. The quantitative survey technique was used to achieve the objectives of this thesis and to verify the hypotheses. The group consisted of nurses from all over the Czech Republic in the month of February 2010 studying on a specialized training course of the NCO/ NZO (National Center of Nursing/ Non- medical health disciplines) in Brno. The results imply that the nurses interviewed do not use enough resources to help reduce the risk of errors leading to an increase in the level of safe nursing care. The proposed measures are intended to increase the use of resources for maintaining the level of safe nursing care which is a clearly dependent on the awareness of their existence. This also strongly urges management to make more use of various resources and inform all medical staff not only nurses about new procedures and analyses carried out on a regular basis. Managers should conduct a regular and detailed analysis of past errors or incidents to establish the risk of error and take steps to minimize such unwanted events. The results of the survey will be provided to the professional public, published in professional journals and can serve as a basis for comparing other survey research.

Then, on the basis of gained results and determined hypothesis evaluate, if men are perceived same as women, whether their low number is caused by gender prejudices and if healthcare personnel and the general public want more men on this position. Theoretical part is devoted to gender, history of nursing, education and specification of general nurse's work and position of men and women in health care in the Czech Republic and the world. The research part deals with the analysis and interpretation of results from the questionnaire surveys designed not only for general nurses, but also for the public.

Introduction: This thesis deals with the time management of nursing care for patients in the ICU after bronchogenic carcinoma surgery. The treatment of patients with this diagnosis requires a multidisciplinary approach by all members of the medical team. Aims and methodology: The aim of this thesis was to map the time demands of nursing care for a patient who is the first day after lung cancer surgery, about current recommended procedures of nursing care. Furthermore, the aim was to find out whether a methodical guideline or a standard defining postoperative care of a patient after lung cancer surgery is available for the nurses. The way nurses perceive their working conditions and relationships in the workplace was also investigated. Finally, the aim of the diploma thesis was to map time management in nursing care for patients with lung cancer, all in the intensive care unit from nine medical facilities that participated in the research survey. Results: The research group consisted of nurses working in surgical intensive care units of the 1st, 2nd and 3rd degree and cardiac surgical intensive care unit from four university hospitals, 3 regional hospitals and 2 other hospitals. This thesis is aimed at aggressive and violent behaviour of patients towards general nurses. Theoretical part of this thesis contains chapters which are dealing with direct aggression, its' causes and also with possible defences, preventive elements and principles, which are necessary to be used in contact and communication with conflicting patient. Because this research is focused on general nurses, one chapter is also dedicated to this profession and its' difficulties. The practical part was realized by means of quantitative research. Created questionnaire was placed on the webpage of České Budějovice hospital where it could be filled by nurses for a period of one month. Data obtained from respondents were subsequently processed into tables and graphs and statistically evaluated. The aim of this thesis was to map the problem of violence and aggression of patients towards general nurses in their profession at České Budějovice Hospital a. s., to describe the problematics of patient aggression in healthcare and through method to find out the experience of general nurses with aggressive behaviour of their patients. According to this research, 95 % of respondents encountered aggressive behaviour of the patient during the last year. 94,5 % of respondents encountered aggressive behaviour of verbal character and 70 % with aggressive behaviour of physical character. According to the research, it was also found that respondents had more experience with aggressive behaviour from male population than from female population. Respondents from the hospital are regularly trained in dealing with conflicting patient. However, the research showed that the respondents would welcome a greater intensity of training on this subject. More than half of respondents perceive the deficit in training in communication with a conflicting patient.

The diploma thesis deals with problems of healthcare-associated infection in selected nursing activities. The thesis is a theoretical one; it has been processed by means of review and synthesis. In this thesis, four objectives were set, namely to create a comprehensive overview of the problems of healthcare-associated infection in selected nursing activities. Other objectives were to map and analyze clinically relevant sites of transmission and possibilities of prevention of healthcare-associated infection in selected nursing activities and to propose recommendations for the prevention of these infections. The thesis is divided into several chapters that deal with the characteristics of healthcare-associated infection, focusing on the causative agents, formation, spreading and prevention. It also deals with particular nursing activities. The first chosen activity was the care of medical devices intended for repeated use, with a particular focus on the implementation of their disinfection. Secondly, hand sanitation as a basic factor affecting the transmission of healthcare-associated infection activity was selected. This chapter deals primarily with the hygienic hand disinfection, its indications, methods and implementation of disinfection products. It also deals with strategies for improving hand hygiene and glove use in providing nursing care. As the third nursing activity, aseptic techniques, which are an essential part in providing nursing care, were chosen. The thesis describes three specific clinical nursing procedures: aseptic collection of venous blood from a peripheral vein, preparation and administration of a drug into an existing peripheral venous catheter, and uncomplicated (aseptic) wounds dressing. Processing of the thesis was based on a range of relevant sources from the Czech Republic but also from abroad. It also contains various recommendations of national and international institutions. The thesis can serve as a basis for more effective prevention of healthcare-associated infection in certain areas.

A planned operation performance always affects patient´s needs for assurance and safety. It is important to provide necessary information connected with the planned operation performance itself so that the patient can feel assure and safe. The key role for the patient and her/his needs for assurance and safety is in general nurse´s hands because she is the closest person to the patient and can help to inspire her/his needs for assurance and safety with konfidence best through the aid of her knowledge and skills that she should have as well as through the aid of her attitudes. The target of the thesis was to find out how a planned operation performance affects patient´s needs for assurance and safety and how a general nurse inspires patient´s needs for assurance and safety connected with a planned operation performance with confidence. The following research questions were stated: How does a planned operation performance affect patient´s needs for assurance and safety? How does a general nurse affect patient´s needs for assurance and safety connected with a planned operation performance to inspire her/him with confidence? What is insufficient in filling patient´s needs for assurance and safety before a planned operation performance? How can a general nurse help a patient control her/his fear and state of anxiety related to a planned operation performance? The method qualitative research with the use of depth interview was used to figure out the investigation. The respondents were willing to record the interview which were verbally rewritten and then subjected to the contents analysis by the method paper and pencil. From the research it is clear that a planned operation performance affects patient´s needs for assurance and safety in a negative way disregarding to her/his previous experience. Patient´s worries and negative feelings can be eliminated by a general nurse who treats her/him as equal, taking up an individual approach, informing her/him about all performances and proceedings, having a calming and welcoming way of communication, using empathy and listening as well as trying to meet patient´s wishes according to her possibilities. The research reveals the advantage of patients´ readiness for planned operation performances. Patients can learn it in the framework of educational proceeding in the out-patients´ department at the same time when they learn that they are going to be operated. The research also shows that general nurses do not have enough knowledge in the sphere of therapeutical communication. The results indicate that it would be suitable to start educating patients as soon as it is known how their disease will be solved and treated in order to reduce the negative impact of surgical treatment to patients´ needs for assurance and safety. General nurses should complete their knowledge and skills to be able to help patients to cope with their difficult life-situation. On the basis of the research investigation and after the consultation with the senior nursing officer, the training course will be organized. The topic of the training course will concentrate on communication with patients before planned operation performances with a view to therapeutical communications and therapeutical interviews. Other discovered facts, for example, the lack of information provided to patients and possibilities regarding better patients´ readiness for planned operation performances will surely be solved by the hospital management.
