Comparison of a piezoelectric and a standard surgical handpeice in third molar surgeryIshmael Gopal January 2010 (has links)

To compare the use of a piezoelectric with a standard surgical handpiece in third molar surgery. Thirty patients requiring removal of third molars were included in the study. Panoramic radiographs were used to assess the third molars. The patients were randomly subdivided and the split-mouth technique applied. In split-mouth design, divisions of the mouth, such as right (upper and lower) and left (upper and lower) quadrants constitute the experimental units, which are randomly assigned to two treatment groups. Each patient serves as his or her own control, which increases statistical efficiency (Siddiqi et al. 2010). Each side was operated with either a piezoelectric or a conventional handpiece. All aspects of preoperative care, general anaesthesia, surgery and postoperative care were standardized for the groups.

Previous issue date: 2006-03-31 / Infection control is considered an essential matter for the safety of patients and professionals The high speed handpiece is a frequently used device during its function it produces aerosols which potentiales microbial dissemination Besides its decontamination is quite polemic According to previous survey carried through in the main health care providers of Goiânia city it is observed a huge utilization of the 70% ethylic alcohol in the disinfection procedures of the high speed handpiece substituting other sterilization methods This study aimed to identify the routine of the high speed handpiece decontamination at the main health care providers in Goiânia city and to verify the bactericide and fungicide action of 70% ethylic alcohol in the high speed handpiece decontamination according to the adopted routine It was realized in seven Center of Integral Attendance to Health which offer dentistry attendance to the population in general of Goiânia city Two different quizzes were applied The first one was for the technician responsible for the high speed handpiece processing and the second one for the dentists in their respective centers The data was analyzed with Epi Info 2000 software version 3.3.2 and presented in a descriptive way For the microbiology analysis 70 samples of 19 high speed handpieces were collected at the seven main health care providers under study The collection was realized with sterile swab all over the high speed handpiece that were surface ready to be used after the disinfection procedure with the 70% ethylic alcohol between appointments There was no interference in the routine of the main health care providers due to the high speed handpiece procedure during in the collection data At all the main health care providers it was observed unsuitable procedures of the high speed handpiece which endanger its quality The procedures related to the suitable decontamination process to high speed handpiece were not realized by any technician responsible for its processing which transgress the recommendation of the Ministry of Health and Centers for Disease Control and Prevention It is worth pointing out that the high speed handpiece are semicritical devices and require sterilization between uses It was identified the presence of potentially pathogenic microorganisms on the high speed handpiece surface after the use of 70% ethylic alcohol The growth in culture of Gram positive coccus was verified in 29 (41,5%) samples, yeast in eight (11,4%) and Gram positive coccus associated with yeast in nine (12,8%) The consequences of the lack of standardization in the high speed handpiece processing go beyond the non-obtainment of the expected effect to the 70% ethylic alcohol and can result into risks for those responsible for the sterilization for dentists and mainly for the patients / O controle de infecção é considerado medida essencial à segurança da equipe odontológica e pacientes A caneta de alta rotação é um instrumento muito utilizado durante seu funcionamento gera aerossóis e facilita a disseminação microbiana além de apresentar polêmica descontaminação Segundo levantamento prévio realizado nas Unidades Básicas de Saúde do município de Goiânia verificou-se na atividade clínica odontológica a ampla utilização do álcool etílico a 70% nos procedimentos de desinfecção de canetas de alta rotação em substituição aos métodos de esterilização Este estudo teve como objetivos: identificar a rotina de descontaminação das canetas de alta rotação nas Unidades Básicas de Saúde do município de Goiânia e verificar a ação bactericida e fungicida do álcool etílico a 70% na descontaminação de canetas de alta rotação segundo a rotina adotada para o seu processamento Foi realizado em sete Centros de Atendimento Integral a Saúde (CAIS) que oferecem atendimento odontológico a população em geral do município de Goiânia Foram aplicados dois questionários distintos O primeiro ao responsável técnico pelo processamento da caneta de alta rotação e o segundo aos cirurgiões-dentistas nas suas respectivas unidades Os dados dos questionários foram analisados no programa Epi Info 2000 versão 3 3 2 e apresentados de forma descritiva Para a análise microbiológica foram coletadas 70 amostras de 19 canetas odontológicas de alta rotação nas sete Unidades Básicas de Saúde em estudo A coleta foi realizada com auxílio de swab estéril em toda a superfície da caneta de alta rotação pronta para o uso após o procedimento de desinfecção pelo álcool etílico a 70% entre atendimentos Não houve interferência na rotina das Unidades Básicas de Saúde quanto ao processamento da caneta de alta rotação durante a coleta de dados Observou-se em todas as Unidades Básicas de Saúde inadequações no processamento da caneta de alta rotação que comprometem a sua qualidade As condutas relacionadas ao processo de descontaminação apropriado para a caneta de alta rotação não eram realizadas por nenhum dos responsáveis técnicos pelo seu processamento o que contraria as recomendações do Ministério da Saúde e Centers for Disease Control and Prevention Vale destacar que as canetas de alta rotação são artigos semicríticos e requerem esterilização entre atendimentos Identificou-se a presença de microrganismos potencialmente patogênicos na superfície da caneta de alta rotação após o uso do álcool etílico 70% Foi verificado o crescimento em cultura de cocos Gram positivos em 29 (41,5%) amostras leveduras em oito (11,4%) e cocos Gram positivos associados a leveduras em nove (12,8%) As conseqüências da falta de padronização no processamento da caneta de alta rotação vão além da não obtenção do efeito esperado para o álcool etílico a 70% e podem resultar em riscos aos próprios responsáveis pelo processamento cirurgiões-dentistas e principalmente para os pacientes
