Orientador: Marcelo Silva Monnazzi / Resumo: Defeitos segmentares no osso mandibular podem ocorrer após traumatismo local, infecção ou ressecção cirúrgica. Não realizar reconstrução nesses pacientes pode acarretar em prejuízo estético grave, dificuldades na mastigação, fala e consequente diminuição de qualidade de vida. Várias possibilidades reconstrutivas estão disponíveis atualmente, e a estabilização temporária dos segmentos ósseos com placas de reconstrução é comumente utilizada quando a reconstrução óssea imediata não é possível. No entanto, pouca atenção tem sido dada aos aspectos biomecânicos envolvidos neste tipo de abordagem e estudos realizados com biomodelos esbarram na dificuldade de simular as forças musculares envolvidas. A utilização da Análise de Elementos Finitos permite avaliar matematicamente estruturas em um ambiente controlado, e a aplicação de forças em qualquer ponto e/ou direção, aferindo a deformação dos materiais, bem como a tensão aplicada aos mesmos. Assim, o objetivo deste estudo foi avaliar placas de reconstrução utilizadas sobre defeitos segmentares, por meio de análise matemática por meio de elementos finitos. Após simulação, foram encontrados menores valores de estresse nas placas de maior espessura, sugerindo maior resistência destas à fratura. Em contrapartida, valores significativamente aumentados de estresse sobre os parafusos estavam presentes nas placas mais espessas. / Abstract: Segmental defects in the mandibular bone may occur after local trauma, infection or surgical resection. Failure to perform reconstruction in these patients can lead to severe aesthetic impairment, difficulties in chewing, speech, and subsequent decrease in quality of life. Numerous reconstructive possibilities are currently available, and the temporary stabilization of bone segments with reconstruction plates is commonly used when immediate bone reconstruction is not possible. However, little attention has been given to the biomechanical aspects involved in this kind of approach and studies carried out with plastic models can not properly reproduce the muscular forces involved. The use of finite element analysis allows to mathematically evaluate structures in a controlled environment and the application of forces at any point and / or direction, assessing the deformation of the materials, as well as the stress applied to them. Thus, the objective of this study is to evaluate reconstruction plates used on segmental defects through mathematical analysis using finite elements. After simulation, lower stress values were found in the thicker plates, suggesting greater resistance to fracture. Material and Methods: Clinical data from 40 cases of KCOTs (30 syndromic and 10 sporadic cases) were coleted and sections from they were immunohistochemically stained and assayed for beta catenin, geminin and MCM2. Results: Sattelites cysts and cellular pleomorfism were more prevalent in syndromic cases. The beta catenin staining pattern was membranous and its reactive extension does not show statistical difference between syndromic and sporadic KCOTs, whereas the syndromic lesions showed less intense reactivity for beta catenin. The reactivity for geminin and MCM2 in both groups showed a nuclear staining pattern. In these groups, the nuclear staining occurred predominantly in the first suprabasal layer. There is no statistical difference in the geminin reactivity between the groups, whereas the means of MCM2 positive cells was higher in sporadic KCOTs than syndromic KCOTs group (p=0.011). Conclusion: Histological features shows evidences of greater aggressiveness in syndromic KCOTs, but there is not significant evidence that ensures the higher proliferative potencial of syndromic KCOT using these markers / Objetivo: O objetivo deste estudo foi avaliar a expressão de beta catenina, geminina e MCM2 em tumors odontogênicos queratocísticos (KCOTs) sindrômicos e esporádicos. Material e Metódos: Dados clínicos de 40 casos de KCOTs (30 casos sindrômicos e 10 esporádicos) foram coletados e cortes histológicos foram imuno-histoquimicamente corados e avaliados para beta catenina, geminina e MCM2. Resultados: Cistos satélites e pleomorfismo celular foram mais prevalentes nos casos sindrômicos. O padrão de marcação da beta catenina foi membranoso e sua reatividade avaliada em extensão não foi estatisticamente diferente entre os grupos de casos sindrômicos e esporádicos, no entanto, lesões sindrômicas apresentaram reatividade menos intensa para beta catenina do que os casos esporádicos. A reatividade da geminina e MCM2 em ambos os grupos foi nuclear. Nesses grupos, a marcação ocorreu predominantemente na camada parabasal. Não houve diferença estatística entre lesões sindrômicas e esporádicas para geminina, já a MCM2 apresentou maior média de células positivas em KCOTs esporádicos (p= 0,011). Conclusão: Características histológicas mostraram evidências de maior agressividade em KCOTs sindrômicos, mas não houve achados que confirmem o maior potencial proliferativo de KCOTs sindrômicos utilizando beta catenina, geminina e MCM2.

What is the role of radiography in diagnosis and treatment of jaw lesions? - a retrospective study

Bakgrund: Vid diagnostik av käkbenslesioner skickas ibland en remiss till röntgenspecialist för ett utlåtande. Röntgenspecialisten ger en eller flera tentativa diagnoser vilka kan underlätta vid beslut om behandling. Efter behandlingen skickas en biopsi till patolog för att erhålla en patologanatomisk diagnos. Syftet med denna studie är att 1) utreda hur väl tentativa röntgenologiska diagnoser överensstämmer med patologanatomiska diagnoser av käkbenslesioner och 2) utvärdera hur ofta röntgenutlåtanden kan leda till felaktig behandling av käkbenslesioner.Material och metod: Röntgenutlåtanden och motsvarande patologanatomiska diagnoser jämfördes retrospektivt under en tioårsperiod för nio vanliga käkbenslesioner. För varje käkbenslesion räknades antalet korrekta respektive inkorrekta röntgenologiska diagnoser som erhållits. Utöver detta jämfördes även den förväntade behandlingen av varje tentativ röntgendiagnos med den förväntade behandlingen av den korrekta patologanatomiska diagnosen. Utifrån detta gjordes sedan en bedömning av huruvida röntgenutlåtandet kunde leda till underbehandling, korrekt behandling eller överbehandling. Resultat: Röntgendiagnoserna överensstämde med de patologanatomiska diagnoserna i 191 av 394 fall (48%). Därutöver gav 61 röntgenutlåtanden (16%) den korrekta diagnosen tillsammans med en eller flera inkorrekta diagnoser. 142 utlåtanden (36%) gav endast en eller flera inkorrekta diagnoser. Drygt en fjärdedel av alla röntgenutlåtanden bedömdes kunna leda till felaktig behandling. Konklusion: Resultaten indikerar en anmärkningsvärd diskrepans mellan de tentativa röntgendiagnoserna och de patologanatomiska diagnoserna av de undersökta käkbenslesionerna. När röntgenspecialisten ger fler än en tentativ diagnos finns en ökad risk för överbehandling vilket kan leda till onödigt lidande för patienten samt en ökad kostnad för både patienten och samhället. Baserat på dessa resultat bör röntgenspecialisternas rutiner för hur många tentativa diagnoser de anger i sina röntgenutlåtanden ses över. / Background: When diagnosing jaw lesions, a referral may be sent to a radiologist for a radiographic report. The radiologist offers one or several tentative diagnoses, which help form the basis for treatment. After treatment a biopsy is sent to a pathologist for a pathoanatomical diagnosis. The aim of this study is to 1) determine the correspondence between tentative radiographic diagnoses and pathoanatomical diagnoses of jaw lesions and 2) evaluate how frequently radiographic reports may lead to incorrect treatment of jaw lesions. Materials and methods: Radiographic reports and pathoanatomical diagnoses of nine common jaw lesions were compared retrospectively over a ten-year period. The numbers of correct and incorrect tentative radiographic diagnoses were counted for each lesion. In addition, the expected treatments of the tentative radiographic diagnoses were compared with the expected treatments of the correct pathoanatomical diagnoses. Each report was then categorized as leading to undertreatment, correct treatment or overtreatment.Results: The radiographic reports corresponded with the pathoanatomical diagnoses in 191 out of 394 cases (48%). In addition, 61 radiographic reports (16%) stated the correct diagnosis along with one or several incorrect diagnoses. 142 reports (36%) only stated one or several incorrect diagnoses. Just over a quarter of the radiographic reports were deemed to lead to incorrect treatment. Conclusion: The results indicate a remarkable discrepancy between tentative radiographic diagnoses and pathoanatomical diagnoses for the studied jaw lesions. There is a tendency towards overtreatment when more than one tentative diagnosis is stated in the radiographic reports. Overtreatment may lead to unnecessary suffering for the patient and financial loss for both the patient and society. Based on these results, radiologists should review their practice of stating several tentative diagnoses in their reports.
