Příspěvek k patofyziologii trombofilního stavu po operaci pro zlomeninu horního konce stehenní kosti u pacientů starších 75 let / Contribution to the pathophysiology of thrombophilic state after fractures surgery in patients over 75 years old

Kudrnová, Zuzana January 2011 (has links)
Introduction: Hip fracture surgery is the particular problem of very old patinets (>75 years), with high risk of VTE (up to 80%). It is essential to provide VTE prophylaxis. Patients advanced age and their polymorbidity contribute to the thrombophilic status. Objectives and methods: The aim of the study was to determine the changes of coagulation within the 28 post-operative days in 41 patients over 75 years who underwent hip fracture surgery. Another object was to determine acute phase response and an endothelial activation. The third task was to determine how affected is a key component of hemostasis, FXa activity by its specific inhibitor fondaparinux and enoxaparin, which inhibits FXa and thrombin in a 4:1 ratio and if there is bleeding complication in such a risk group patiens after antithrombotics long-term administration. Patients were randomly divided into two anticoagulant groups: fondaparinux (n = 23) and enoxaparin (n = 18). Results: Thrombophilia is demonstrated by a reactive increase of the most of these parameters preoperatively and reveals the effect of the initial trauma. A surgery further aggravates this reaction. This inflammatory and secondary prothrombogenic condition persisted until postoperative day 28. Both antithrombotics effectively inhibit thrombin generation without...

Soběstačnost pacientů v personálních všedních denních činnostech po operačním řešení fraktury v oblasti proximálního konce femuru / Self-sufficiency of patients in personal activities of daily living after surgical treatment of a fracture in the proximal end of the femur

Brindzová, Veronika January 2020 (has links)
Introduction: Fractures of the proximal end of the femur are one of the most frequent diagnoses. Patients with this diagnosis have to deal with performing daily activities. Both for painas well for the necessity of adherence to antiluxal measures. Thanks to this decrease thein independence from the other person. As one of the maingoals in this diploma thesis we determined means how occupational therapy canincrease self - sufficiency in this target group. As other goals, we found in formativ about the selection and financing of compensatory aids and we made a manual about this information. We focused on the factors that affect the return of patients to the home environment. Methodology: In the personal parts we chose 6 case studies (3 men and 3 women), in the averageage 73.5 years (median 72, 5), youngest 64 years, oldest 85 years. Patients underwent 5 therapies dedicated to the training of self - sufficiency of personal everyday activities (pADL). We measured the effectiveness of th erapeutic units using the Barthel Index. We observed them and found out chat compensatory aids they use for self-sufficiency Results: Probands recese daverage 33.3 points in the evaluationat the initialexamination. After graduating therapiest heave rage performance of all probands obtained at 67, 5 points. They used...

Implementace paliativní péče do nemocnic / Implementation of palliative care in hospital setting

Kovaříková, Julie January 2018 (has links)
This thesis deals with the implementation of palliative care in hospitals and perceives the topic in context of change management in health facilities. In the Czech Republic, this type of care is still at its beginning in hospitals and its implementation is a big challenge. The aim of the thesis is to find out what factors influence the implementation of palliative care in hospitals. In the first part of the text, the theoretical bases concerning palliative care and change management are outlined. The empirical part introduces a qualitative survey identifying barriers that had to be overcome during the implementation of palliative care in hospitals and factors that were instrumental in the implementation. Within the survey, 66 interviews with members of multidisciplinary teams from 17 hospitals were made. All the hospitals participated in the Together until the end programme of the Avast Foundation. The assessment of the survey results focuses on recommendations for hospitals that decide to extend or launch the palliative attitude in their premises and on challenges for the future of palliative care in the Czech Republic. The survey was carried out under the supervision of the Center for Palliative Care and it was used as the final assessment of the Together until the end programme that mapped the...

Analýza regulace komplexů cytoplazmatických poly(A) polymeráz / Analýza regulace komplexů cytoplazmatických poly(A) polymeráz

Novák, Jakub January 2011 (has links)
The regulation of gene expression is achieved at many levels. Chromatin-based gene regulation has been the central focus of many decades of research; however, posttranscriptional control mechanisms are emerging as a fundamental complement to direct protein synthesis. This thesis is focused on a specific mechanism of posttranscriptional control - the translational regulation of mRNAs in the cell cytoplasm. This control is a consequence of the balance between translational repression and activation and hinges on the selective recognition of regulated mRNAs by RNA-binding proteins and their ability to recruit RNA modifying proteins. In this thesis, Caenorhabditis elegans germline was used to study translational control of the germ cell-enriched gene, gld-2. Mutants of known RNA-binding proteins of the PUF and CPB protein families were analyzed by performing Western blots, using anti-GLD-2 antibodies. Yeast 3-Hybrid system was used to identify the cis-regulatory sites in the gld-2 mRNA conferring translational regulation by members of PUF and CPB protein families. Potential autoregulatory loop of gld-2 gene expression was also investigated. This thesis shows that FBF proteins positively regulate expression of gld-2 and bind to a conserved sequence in the 3'UTR of its mRNA. Mutations of gld-2 negatively affect...

Jana Štroblová jako překladatelka a vykladačka Mariny Cvetajevové / Jana Štroblová as a Translator and Exegete of Marina Tsvetaeva

Krausová, Aneta January 2017 (has links)
This master thesis is devoted to Jana Stroblova, the Czech poet, novelist and translator from Russian language. The thesis is aimed at several goals: 1) to summarize and unite the known facts about life of this author and try to capture her personality, 2) to describe the relationship of Jana Stroblova to the Russian poet Marina Tsvetaeva, as the vast part of Stroblova's translations is dedicated to her, 3) to establish the similarities between the personalities of Stroblova and Tsvetaeva as well as to find the parallels in their poems, and, afterwards, to clarify the reasons that led Stroblova to decision to translate Tsvetaeva, 4) to summarize the reflections of Stroblova's translations while emphasizing the translations of Tsvetaeva's work and to use these reflections to define Stroblova's translation methodology, 5) to confront the findings of the thesis with opinions of Jana Stroblova during an interview with her.
