The sagittal soft-tissue changes of the lower lip and chin associated with surgical maxillary impaction and consequent mandibular autorotation

Student Number : 9000040F - M Dentistry research report - Faculty of Health Sciences / This study assessed the sagittal soft-tissue changes of the lower lip and chin area in 22 patients subsequent to mandibular autorotation following surgical vertical impaction of the maxilla. A subgroup of six patients in addition had undergone advancement genioplasty procedures. Lateral cephalometric radiographs were taken immediately prior to surgery and on average 15 months following surgery. Sixteen cephalometric landmarks were identified on each radiograph and these were digitized using a Kontron Videoplan Image Analysis System to enable differences reflecting changes to be assessed. The comparison between those cases that had had maxillary elevation only and the six cases that had received additional advancement genioplasty procedures revealed statistically significant differences in relation to the proportional changes in the chin area. Therefore, when studying the soft-tissue chin changes following mandibular autorotation, these six patients were excluded from the sample. It was found that there was no significant difference in the lower lip response between the two groups and therefore when studying the lower lip changes, the two groups could be pooled. The soft-tissue changes in the chin area showed statistically and clinically significant correlations. In the horizontal plane, a ratio of 0.9:1 was found for the changes between sulcus inferior and point B, between soft-tissue pogonion and hard-tissue pogonion, and between soft-tissue gnathion and hard-tissue gnathion. In the vertical dimension, soft-tissue gnathion followed hard-tissue gnathion in a ratio of 0.9:1, whereas soft-tissue menton followed hardtissue menton in a ratio of 1:1. In the study of the lower lip response, a significant correlation with a ratio of 1:1 existed for the horizontal change in the lower lip as measured at labrale inferius relative to both lower incisor tip and lower incisor anterius. In the vertical dimension, stomion inferius followed lower incisor anterius in a ratio of 1.3:1, while labrale inferius followed lower incisor anterius in a ratio of 1.5:1. Multiple regression analyses revealed that presurgical tissue thickness exerted no influence upon the strength of the correlations between changes expressed at corresponding soft- and hard-tissue landmarks located in the lower lip and soft-tissue chin area. Based on the findings of this study, it is suggested that the soft-tissue to hard-tissue ratios may be applied to prediction tracings with enhanced confidence. As a result, the tracings will reflect a more accurate prediction of the lower lip and soft-tissue chin positions following autorotation of the mandible.

Sex Estimation in Subadult Skeletons: A Test of Eight Nonmetric Traits of the Mandible and Ilium

Diagnosing the sex of subadult skeletal material is a difficult task for bioarchaeologists and forensic anthropologists. Metric and nonmetric techniques have been tested utilizing both fetal (Boucher 1957, Weaver 1980) and juvenile (Loth and Henneberg 2001, Weaver 1980) skeletal remains. Some metric techniques show promise in estimating the sex of juvenile skeletons, but most are found to be lacking in adequate accuracies (Holcomb and Konigsberg 1995, Weaver 1980). Nonmetric techniques have been found to be more accurate, performing better than chance (Loth and Henneberg 2001, Mittler and Sheridan 1992, Weaver 1980). Análise retrospectiva de 129 implantes osseointegráveis de ancoragem zigomática utilizados na reabilitação de maxilas severamente reabsorvidas em protocolo de dois estágios em um período de 7 anos / Retrospective analysis of 129 consecutive zygomatic implants applied for rehabilitation of severely resorbed maxilla in a double-stage protocol in a period of 7 years

Este estudo tem como objetivo apresentar uma análise retrospectiva de dados contidos em prontuários de atendimento, sobre a evolução clínica de 129 implantes zigomáticos colocados no seio maxilar de 37 pacientes com maxila desdentada severamente reabsorvida (total ou parcialmente), os quais foram reabilitados através da instalação de implantes zigomáticos pela equipe de Cirurgia e Traumatologia Buco-Maxilo-Facial da Faculdade de Odontologia de Ribeirão Preto (FROP-USP), durante o período março/2007 à novembro/2014. Todos os pacientes incluídos no estudo foram tratados usando o mesmo protocolo cirúrgico, técnica ´´sinus slot´´ modificado por Stella. A análise foi direcionada a registro de dados relativos as seguintes variáveis: gênero, idade, raça, história médica, etiologia, vícios, grau de atrofia óssea, tipo e tamanho dos implantes zigomáticos, número de implantes colocados, reabilitação protética, a taxa de sobrevivência e de sucesso dos implantes, além das complicações. Os indivíduos foram acompanhados por meio de exames clínicos e de tomográficos padronizados. O critério de inclusão dos prontuários na amostra foram de pacientes submetidos a esse tipo de tratamento, com preenchimento completo de informações e de possuírem o Termo de Consentimento Livre e Esclarecido. Os dados coletados por um único examinador foram transportados para uma planilha eletrônica utilizando o programa Microsoft Excel, sendo realizada neste momento uma análise descritiva dos resultados. Trinta e sete pacientes consecutivos (25 mulheres, 12 homens; média de idade de 55,64 anos [variação: 40-77 anos]) foram tratados. Todos estavam em necessidade de reabilitação bucal e tinham atrofia maxilar severa com indicação para a colocação de implantes zigomáticos. Cento e vinte e nove implantes zigomáticos foram colocados nesses 37 pacientes. Dois dos 129 implantes falharam, resultando em uma taxa de sucesso de 98,44%, com esses dados de acompanhamento. Sinusite do seio maxilar foi a ocorrência mais comum (21,62%) entre as complicações encontradas. No entanto, entre os implantes que falharam, nenhum foi relacionado com sinusite ou fumo. Concluímos que tratamento com implantes zigomáticos representa uma opção previsível para reabilitação da maxila atrófica desdentada. No entanto, mais estudos são necessários para se relacionar diretamente esses implantes com a sinusite, que apresenta uma elevada prevalência nestes casos. / This study aims to present a retrospective analysis of data contained in patient records on the clinical course of 129 zygomatic implants placed in the maxillary sinus of 37 patients with severely resorbed edentulous maxilla (fully or partially), which were rehabilitated by installing of zygomatic implants by staff of Surgery and Traumatology Oral and Maxillofacial Ribeirão Preto Dental School (FROP-USP), during the period from March/2007 to November/2014. All patients included in the study were treated using the same surgical protocol, \'\'sinus slot\'\' modified by Stella technique. The analysis was aimed at data record the following variables: gender, age, race, medical history, etiology, addictions, degree of bone atrophy, type and size of zygomatic implants, number of placed implants, prosthetic rehabilitation, the survival rate and success of the implant, in addition to complications. The subjects were followed through clinical and standardized CT. The criterion for inclusion of the records in the sample were patients undergoing this type of treatment, with complete filling of information and possess the Informed Consent and Informed. The data collected by a single examiner were transported to a spreadsheet using Microsoft Excel program, being carried out at present a descriptive analysis of the results. Thirty-seven consecutive patients (25 women, 12 men; mean age of 55.64 years [range, 40-77 years]) were treated. All were in need of oral rehabilitation and had severe jaw atrophy indicated for the placement of zygomatic implants. One hundred and twenty-nine zygomatic implants were placed in these 37 patients. Two of the 129 implants fail, resulting in a success rate of 98.44%, with these tracking data. Maxillary sinus sinusitis was the most common event (21.62%) among the complications found. However, between implants that have failed, none has been associated with sinusitis or smoke. We conclude that treatment with zygomatic implants is a predictable option for rehabilitation of edentulous atrophic maxilla. However, more studies are needed to relate directly with these implants sinusitis, which has a high prevalence in these cases.

Foraminas acessórias da região anterior da maxila em tomografia computadorizada de feixe cônico / Additional foraminas in anterior maxilla through cone beam computed tomography

Por meio do exame tridimensional de Tomografia computadorizada de feixe cônico (TCFC) é possível avaliar a área de interesse previamente aos procedimentos com maior precisão. As variações anatômicas são estruturas que devem ser consideradas para a realização de cirurgias, como a foramina acessória da região anterior da maxila (FARAM). O objetivo principal do presente estudo foi avaliar a FARAM, em 150 exames de TCFC, obtidas no aparelho ICat Classic®. Por meio do programa ICat Vision® foram mensuradas o diâmetro, a distância da FARAM ao forame incisivo (FARAM-FI), da FARAM ao rebordo alveolar vestibular (FARAM-RAV) e da FARAM a sutura intermaxilar (FARAM-SI). Também foi avaliada a trajetória dos canais ósseos formados a partir da FARAM. Nos pacientes dentados, foram localizados por meio da área dos dentes: incisivo central, incisivo lateral e caninos. Foi realizada a correlação entre a presença da FARAM e o gênero e a idade. Dos 150 exames, a FARAM estava presente em 21 exames (14%). Destes, dez pacientes eram edêntulos na região de interesse de estudo e não foi possível localizar a foramina em relação aos dentes. Nenhum exame apresentou a FARAM relacionada ao incisivo central, em sete estavam relacionadas ao incisivo lateral e em quatro estavam relacionadas ao canino. Nenhum exame apresentou a FARAM bilateralmente, sendo que treze foram localizadas no lado direito e oito do lado esquerdo. O diâmetro médio da FARAM foi de 1,57 ± 0,38 mm. As médias das distâncias de FARAM-RAV foi de 11,65 ± 3,34 mm, FARAM-FI foi de 8,86 ± 2,72 mm e FARAM-SI, foi de 6,37 ± 2,9 mm. A trajetória do canal ósseo mais comum foi ascendente/oblíqua em direção a região anterior do assoalho da cavidade nasal. A correlação entre a presença da FARAM e o gênero foi estaticamente significante para os homens (p=0,055) no teste do qui-quadrado. No entanto, não foi estatisticamente significante para a idade. / Through cone beam computed tomography (CBCT) is possible to evaluate the region of interest before surgical procedures in tridimensional view. Anatomical variations should be considered for performing surgeries, as the additional foraminas in anterior maxilla (AFAM). The main aim of the present study is evaluate AFAM in 150 CBCT exams, obtained through ICat Classic® device. In ICat Vision® software, the diameter, the distance from the AFAM to the incisive foramen (AFAM-IF), AFAM to the vestibular alveolar ridge (AFAM-VAR) and AFAM intermaxillary suture (AFAM-IS) were measured. It was also evaluated the trajectory of the bony canals related to AFAM. In dentate patients, AFAM was located through the teeth area: central incisor, lateral incisor and canines. The correlation between the presence of FARAM and gender and age was investigated. The AFAM was presented in 21 exams (14%). In the positive exams, 10 patients were edentulous in the region of study interest and it was not possible to locate the foramen in relation to the teeth. Any exam presented the AFAM related to the central incisor, in seven were related to the lateral incisor and in four were related to the canine. Any exam presented the AFAM bilaterally, of which thirteen were located on the right side and eight on the left side. The mean diameter of the AFAM was 1.57 ± 0.38 mm. The distance of the AFAM-VAR 11.65 ± 3.34 mm, AFAMIF was 8.86 ± 2.72 mm and AFAM-IS was 6.37 ± 2.9 mm. The most common bone canal trajectory was upward/ oblique to the anterior region of the floor of the nasal cavity. The correlation between the presence of AFAM and the gender was statically significant for males (p=0.055) in the chi-square test. However, it was not statistically significant for age.
