Souvislosti mezi užíváním metamfetaminu a propuknutím onemocnění schizofrenního okruhu / Relationship between methamphetamine use and outbreak of schizophrenic circuit disease

The issue of dual diagnoses has resonated across many fields for several decades. It has also been proved that the number of methamphetamine users grows dramatically, so does the number of patients hospitalized for "toxic psychosis" or other psychotic disorders. Schizophrenia and related diseases then form the third largest group of psychiatric patients in the Czech Republic. The aim of the research part was to analyze data of patients sample from Havlíčkův Brod Psychiatric Hospital (PNHB), with concurrent diagnoses F20-F25 and F15.x or F19.x and to describe associations between methamphetamine use and outbreak of schizophrenic mental illness. The research sample was obtained by combining method of selection according to criteria, method of intentional selection through institutions and method of total selection. Havlíčkův Brod Psychiatric Hospital provided anonymized data of 108 patients meeting the required criteria. The data were categorized within the content analysis of the medical documentation text, coded in a simple form and recorded in Excel spreadsheet. In the last phase, the data were analyzed by combining the first, the second and the third degree in contingency tables. Methodics of drug crime investigation Abstract The subject of my diploma work is the methodics of drug crime investigation. Above all, I have chosen this subject because I am deeply interested in the problem of drug and circumstances that are linked with this issue. As police officer I have a lot of experience on the field of drug crime investigation and I decided to focus my diploma work especially on the methodics of serious organized and international drug crime. My diploma work is structured into two parts and seven chapters. In the General part I tried to explain most important facts about drug crime, its history, its causes and I mentioned some specifics of the drug scene in the Czech Republic. Chapter 2 of my diploma work is aimed to provide detailed information about various kinds of drugs, about their origin, appearance and effects on human health. Very important part of my diploma work is also next chapter that is aimed on legal regulation of drug treatment in our country. of the thesis Introduction: This dissertation focuses on socioeconomic status and problems with law of problematic opioid and methamphetamine users. Long-term intensive use of drugs leads not only to health impacts, but also social such as the inability of financial autonomy, indebtedness, unemployment, failure to comply with the respective social roles, fiduciary social functioning, poor housing etc. Great number of users are influenced by the drug in situations such as interpersonal relationship, family functioning and partnerships, there is a change of social status, social isolation or exclusion. Effective interventions and service development should always start with a good knowledge of the situation and needs of the user, who claims the service. Claim: The goal of this thesis is finding socioeconomic status and criminal situation of problematic methamphetamine and opioids users consequently draw recommendations towards services that work with these clients. BACKGROUND: Despite the high number of methamphetamine users in the Czech Republic, the treatment is still only abstinent. Pharmacotherapy in terms of substitution treatment is not yet in place. No study has yet been conducted to examine the views of workers working with methamphetamine users on the possibility of introducing substitution treatment for methamphetamine dependence in the Czech Republic. OBJECTIVE: The aim of the practical part was to map the opinions of outpatient staff on the possibility of introducing substitution treatment for methamphetamine dependence. The monitored area included the evaluation of the success of the treatment of methamphetamine dependence in outpatient settings, disparity in working with users by substance use, percentage of methamphetamine users in the facilities, the suitability of drug addiction to methamphetamine for clients of outpatient facilities and position of substitution treatment for methamphetamine, according to respondents. The objective of this thesis is to summarize the effects of drugs, especially of the cannabinoids and amphetamine type stimulants, and to analyse which of these substances leads to high-risk activities, that could endanger life, health or result in damage to property. One of these activities, an important one, is driving under the influence of drugs. Driving itself is a complex demand, involving perception, decision making, psychomotor skills, knowledge, as well as attitudes and personality characteristics. Substance use and abuse has an impact on the functioning of the brain and mental processes and therefore on the performance of the driving task. Driving under the influence of drugs is a high risk activity which endangers not only the driver but also their surroundings. In the last few years, the Czech Republic registered a rapid increase in the number of impaired drivers, namely at police check points and in car accidents. The young drivers are mostly under influence of marijuana, respectively tetrahydrocannabinol (THC). Its negative effect on driving results from impaired co-ordination, slowed complex reaction time and in space and time distortion. The second group of drugs which can impair drivers is amphetamine type stimulants. Amphetamines are frequently sought by truck drivers, especially long-haul drivers, in Western Europe for its stimulative effects. In the Czech Republic the methamphetamine, Pervitin, is much more common. Intoxication is characterized by increased wakefulness, increased physical activity, increased respiration, rapid heart rate and an irregular heartbeat. The risk factors for driving are a lack of concentration and an overestimation of one{\crq}s own capabilities. After the acute phase of intoxication subsides, fatigue or states of narcolepsy may occur. Another amphetamine-based drug is methylenedioxymethamphetamine, MDMA or Ecstasy. This synthetic substance, chemically related to mescaline, is dangerous due to its ability to regulate body temperature. On rare but unpredictable occasions, this can lead to sharp hyperthermia, resulting in liver, kidney and cardiovascular system failure, and death. It can also cause hallucinations. All common drugs are easily detectable with ``drug test-cards{\crqq}, which are used during police checks around the the world. The Czech Republic is currently testing their utilization and is preparing legislation accordingly. Utilization of these tests during random checks could help to decrease the number of fatal road accidents and could help to decrease the number of impaired drivers on the roads.

Introduction: Methamphetamine is a drug frequently abused by drug-addicted pregnant women and also one of the mostcommonly used drugs in the CzechRepublic. This drug passes easily through a placental barrier into the fetus. Thus it can negatively affect not only the mother but also the prenatal development of her offspring. Objectives: In the framework of the grant project GA CR: 14-03708S, the long-term effects of prenatal exposure to methamphetamine were detected. It was determined whether the prenatal methamphetamine exposure affects the generation of offspring of exposed females at the level of gene expression of genes in specific regions of the brain, striatum, hippocampus and prefrontal cortex. Methods: In the selected parts of the brain, which were removed from the rat, the microarray hybridization and the real-time PCR to express changes in expression of selected genes were performed. Results: Statistical analysis of microarray hybridization did not show the significantly altered gene expression in tested genes significantly. Only boundary values for 13 genes were measured, which were further tested by real-time PCR.After a statistic evaluation of real-time PCR, the significantly altered expression was found in 2 genes. Objectives: This research aims to provide a general screening of gene expression in selected regions of the F1 generation of the brain prenatally affected by methamphetamine, to verify whether exposure to methamphetamine affects the generation of offspring of exposed females at the level of gene expression in selected regions of the brain, and to valuate possible changes in gene expression. Methods: In selected parts of the brain, collected from a rat, the microarray hybridization and the real-time PCR were set to evaluate express changes in the expression of selected genes. Results: Statistical analysis of the microarray hybridization did not show a significantly altered gene expression in the tested genes. Background: Injecting drug users (IDUs) represent considerable group of patients infected with hepatitis C virus (HCV). HCV treatment is an effective tool for reduction of HCV transmissions among IDUs. Nevertheless treatment rate among IDUs is rather insufficient. Treatment uptake, provision and adherence as well as its efficacy in IDUs are determined by number of specific factors. Aims: Mapping an extent of the provision of HCV treatment to IDUs in the Czech Republic, rules and practices for the admission of IDUs into HCV treatment and its provision, describing relevant factors related to drug use. Material and methods: From January to March 2011, a questionnaire survey among centres for treatment of viral hepatitis in the Czech Republic was conducted. 76 identified centres were addressed, of which 45 (59%) responded, and 40 (53%) filled in an online questionnaire. Results: Estimated number of centres treated HCV with combination of pegylated interferon α and ribavirin in the Czech Republic in 2010 was 61, 39 of them treated IDUs. Estimated 780 persons were treated, of whom 370 were (mostly ex-) IDUs. Reported treatment uptake in IDUs was 60% on average (range 0-90%).
