The thesis ,,The social program of support for a client with physical handicap" is devided into two parts. The theoretical part presents the filosofy of the spirit and body, the physicality, body dissability, cerebral palsy and the Act No. 108/2006 Coll., on social services. The empirical part consists of the case managment of the client with cerebral palsy. I describe the goal of the thesis here and the method of collecting data. I also introduce my respondent in this part and mention the tasks of the research. The chapter called ,,Data and it's interpretation" consists of the qualitative interview. The aim of the interview is to show in what extent my respondent use a social services. The interview is the part of the thesis.

TITLE Efficacy of physiotherapeutic interventions in stroke patients with pusher and neglect syndrome. OBJECTIVES The aim of the thesis is to summarise current literature concerning two peculiar phenomena following stroke - pusher syndrome and neglect. Furthermore the work focuses on evaluation of efficacy of physiotherapeutic interventions. METHODS The thesis represents a critical review of a literature and it consists of three parts. The first one summarize issues concerning the pusher syndrome and neglect in general. In a special part you can find a detailed description of physiotherapeutic methods that are widely used in a rehabilitation treatment of these two neuronal symptoms. This work is concluded by evaluating results of effects of the therapeutic interventions that are discussed with respect to the future research. RESULTS Up till now there are no clinical trials concerning efficiacy of physiotherapeutic methods in a treatment of a pusher syndrome. The similar situation seems to be in case of a neglect syndrome as number of available clinical trials testing the efficiacy of physiotherapeutic techniques is very limited. This diploma thesis deals with the issue of robotic-assisted therapy, namely with the mechanical exoskeleton robotic device Armeo Spring, and it is focused on an acute stroke population. The aim of this study is to evaluate the efficiency of a robotic-assisted therapy using Armeo Spring device and to compare it with the efficiency of a common individual occupational therapy focused on improving motor function of upper extremity of patients with the acute stroke. There were 19 patients involved in the research, who were hospitalized in early rehabilitation acute beds. They were selected on the basis of entry criteria and further divided into two groups. An experimental group included 10 participants (n=10), a control group included 9 participants (n=9). The patients in the experimental group using Armeo Spring device underwent four to five 30-minutes therapies per week for three week period, the control group underwent the same amount of individual occupational therapies focused on improving motor function of a paretic upper limp. Each participant, evaluated before and after the intervention, underwent the total number of 12 therapy sessions. Aim: The aim of this study was to determine the impact of therapy by active videogame Nintendo Wii and by Vojta reflex locomotion on a static and dynamic postural control at a group of children with mild cerebral palsy. Methodology: 14 children (from 6 to 18 years old) were randomly divided into two groups. Both groups received both therapies (active videogame therapy and Vojta reflex locomotion) with a 6 months gap, but in an opposite order. Posturographic measures were obtained before the start of the therapy, after therapy and 8 weeks after the end of the therapy. Results: After both therapies, there was a significant increase in the COP sway velocity in the mCTSIB subtest on a firm surface with closed eyes and the COP sway velocity in the mCTSIB subtest was reduced on a foam surface with closed eyes. Further, after both therapies, there was a significant reduction in the time required to perform the turnabout in SQT test. Other tests (LOS, RWS, WA, TW) and subtests did not change significantly. There was not a significant difference between therapies, but in the post- therapy tests, VRL had a greater tendency to maintain its effect 8 weeks after therapy. Potřeby pacientů po poškození mozku / The Needs Of Patients After Brain Damage

This diploma thesis is a part of the GAJU project entitled "Coordinated rehabilitation of patients with brain injury (reg. number GAJU 128/2016/S)". At the time when the thesis was elaborated, only the patients who suffered the cerebrovascular accident were engaged in the project, since in the hospital, no suitable patient after brain injury who would need a multidisciplinary team in home care had ever been sought. The selection of the patients was based on the communicative skills criteria e.g. if the skills to participate in the directed conversation based on the Functional Independence Measure Questionnaire (FIM), WHO Disability Assessment Schedule 2.0 and the follow-up questions concerning social issues, physiotherapeutic methods and techniques were preserved. Within the framework of the project, the research has not been finished yet. Given the above mentioned reasons, this thesis is therefore mainly focused on the identification of patients' needs after the cerebrovascular accident (CVA), on the period starting from its first incidence during the first three months period after the discharge from the hospital to home care. The complex results will be presented as a part of the publication activity of the project. The thesis comprises a theoretical and a practical part. The theoretical part presents a comprehensive view on the care for the patients who suffered the cerebrovascular accident. I addressed both the basic characteristics of the cerebrovascular accident and the organisation of the care for the patients in the Czech Republic; furthermore, the coordinated rehabilitation team, the role of a social worker, possibilities of social help provided by the welfare system to patients and their families, and the psychical condition of the patient. In the last chapter, I outline the general concept of human needs according to A. Maslow and describe important needs of the sick people. As far as the research part is concerned, my goal is to identify the needs of patients in home care after the brain injury. In connection with the aim of the thesis, two research questions were defined. The first question is: "What are the needs of the patients after brain injury?". The second question is: "What is the role of a social worker in meeting the needs of a patient with brain injury?" The qualitative research strategy was used; the instructional dialogue technique with patients after brain injury in the South Bohemia Region within the project. When analyzing the data collected from the patients during the first three months after the discharge from the hospital to the home care, seven major patient needs emerged need of help, need of psychical equilibrium, need of self-reliance, need of being without paint, need of recovering to the condition before CVA incidence, and need of establishing a new system of activities. Two of these needs, the need of recovering to the condition before CVA incidence and the need of establishing a new system of activities could be considered as principal, because the other needs are in general heading towards them. Furthermore, the analysis of the results showed the role of a social worker by means of which these needs can be saturated The results of the present thesis are used as a partial part from which the complex results of coordinated rehabilitation of patients after brain injury of the GAJU project are complied. The needs analysis of patients brought the proposals of dealing with/saturation of these needs and therefore had a direct impact on the lives of the respondents. The results of the research will become a part of the publication outputs of the above-mentioned projects.

The Occupational Therapist's Intervention for Adult Patients with Neurogenic Dysphagia Abstract of the Thesis: Dysphagia is a serious disorder, often caused by a cerebrovascular accident; in many cases it may be its only or primary symptom, and even the cause of death. The main objective of this thesis is to raise awareness of swallowing disorders among occupational therapists. Despite the fact that the occupational therapist's intervention for patients with swallowing disorder is common practice among occupational therapists abroad, this issue still is not well known among occupational therapists in the Czech Republic. There is also a considerable lack of Czech literature dealing with swallowing disorders from the occupational therapy point of view. This thesis therefore not only presents theoretical findings in this field but also recommends practical procedures for occupational therapists. The theoretical part of the thesis describes functional anatomy of the swallowing structures and swallowing physiology and pathophysiology. It analyses different causes of dysphagia with a focus on neurological disorders, defines the term dysphagia and other related terms, and briefly explains several classification methods of dysphagia. Profil mírné kognitivní poruchy u pacientů po ischemické cévní mozkové příhodě. / Mild cognitive impairment after stroke

The goal of this thesis was to describe cognitive profile of mild cognitive impairment (MCI) in patients witch ischemic stroke (IST). The theoretical introduction focuses on IST, the concept of MCI, and neuropsychological test of cognitive deficits related to IST. The empirical part of the thesis describes the research results. 64 patients with various types of brain lesions after first-ever stroke were examined within 3-6 months after hospital admission. The cognitive performance was assessed across five cognitive domains: memory, attention, executive functions, visuospacial abilities, and language. All cognitive domains were impaired to the similar extend; the impairment quite severe, and ranged from 1.5 to 2.0 SD below the mean of the control group. The results showed that right hemisphere lesions were associated with visuospacial impairment whereas left hemisphere lesions were associated with language impairment. Also, it was examined which combination of tests differentiates best between patients from the experimental groups and the control group, and among individual experimental groups. It is necessary to develop and use sensitive and valid tests that can detect mild cognitive deficits in patients after ischemic stroke, which could be useful for rehabilitation planning. Intenzivní hipoterapie u dětí s diagnózou DMO a její krátkodobý a střednědobý efekt / Intensive hippotherapy in children diagnosed with infantile paralysis and its short-term and intermediate effect

Hippotherapy belongs to the family of proprioceptive neuromuscular facilitation methods. It especially benefits the rehabilitation care of complex neurological patients. This paper theoretically examines the methodology of hippotherapy. Further, this paper examines an evaluative survey of 21 subjects after intensive hippotherapy who were initially diagnosed with cerebral palsy. The immediate, short and medium term effect of the therapy was exami-ned. For the evaluation the intercondylic index, Collis symptom and position reaction tests were used. The testing is complemented with a questionnaire filled in by parents with 11 pa-rameters, which included changes in the functions of knowledge and psyche of the child. The results show that there is a statistically significant improvement in all examined parameters after immediate and weekly therapy.
