Title of diploma thesis: Specific foot changes in figure skaters and options for their physiotherapeutic intervention. Objective: The purpose of this thesis is to research available literary sources concerning anatomy, kinesiology and biomechanics of foot and foot arch in relation to specific foot changes resulting from figure skating. In addition, the thesis seeks to research various physiotherapeutic approaches which could be applied in prevention, compesation or therapy of these changes. Methods: This diploma thesis has descriptively-analytical character. It is structured in the form of literary research. Results: On the basis of the researched literary sources, this thesis provides a comprehensive picture on the topic of foot in figure skaters. Based on the research, the thesis identifies foot changes resulting from deformation of the foot in the skate. Subsequently, the thesis discussess the possible physiotherapeutic approaches which can be applied in prevention, compensation or therapy of these changes. Rôle de l'arginase dans l'atteinte vasculaire associée à l'arthrite chez le rat / Role of arginase in vascular disease associated with arthritis in rats

Les patients atteints de polyarthrite rhumatoide (PR) ont une diminution de l'espérance de vie de 10 à 15 ans. Cette augmentation de la mortalité semble liée à un processus athéromateux accéléré. La dysfonction endothéliale (DE) joue un rôle clé dans ce processus. L'arginase est une enzyme qui régule le niveau de NO par compétition avec la NO Synthase (NOS) pour leur substrat commun, la L-arginine.Nous avons montré que la vasodilatation Acétylcholine (ACh) dépendante était altérée dans le modèle d'Arthrite Induite à Adjuvant (AIA) chez le rat Lewis, témoignant d'une DE. L'incubation des anneaux aortiques avec la nor-NOHA un inhibiteur sélectif d'arginase a amélioré la réponse vasculaire à l'ACh chez les rats AIA. L'activité et l'expression vasculaire de l'arginase se sont révélées augmentées chez les rats AIA et corrélées positivement à la sévérité de l'arthrite.Nous avons caractérisé les mécanismes impliqués dans la DE du rat AIA. Nos résultats ont montré que la DE mettait enjeu une diminution de l'activité de la NO synthase, un déficit en EDHF, une suractivité de la COX-2, ainsi qu'une production excessive des anions superoxydes. Le traitement curatif des rats AIA par la nor-NOHA pendant 3 semaines, a permis de restaurer la fonction endothéliale. L'inhibition de l'arginase n'a pas modifié l'atteinte articulaire des rats AIA.Nos travaux ont permis de mieux comprendre la physiopathologie de la DE associée à la PR et ont déterminé pour la première fois le rôle délétère de l'arginase dans cette anomalie. Ils ouvrent des perspectives quant à l'utilisation des inhibiteurs d'arginase comme futurs traitements pharmacologiques de l'atteinte vasculaire du patient PR / Patients with RA are characterized by a decrease in life expectancy of 10 to 15 years. This increase in mortality seems to be related to an accelerated atheroma process. Endothelial dysfunction (ED) has a key role in these processes. The arginase is an enzyme which regulates the level of NO by competing with the NO synthase (NOS) to their common substrate, L-arginine.We showed that acetylcholine (ACh) dependent vasodilation was altered in the model of Adjuvant Induced Arthritis (AIA) in Lewis rats, indicating a endothelial dysfunction. The incubation of aortic rings with nor-NOHA has improved the vascular response to ACh in AIA rats. The activity and expression of vascular arginase are increased in AIA rats and positively correlated with the severity of arthritis.We characterized the mechanisms involved in DE in AIA rats. Our results showed that ED involved a decrease of activity of NO synthase, a decrease of EDHF, overactiviry of COX-2, thromboxane synthase and prostacyclin synthase, as well as excessive superoxide anions. The cure of AIA rats by a selective inhibitor of arginase, nor-NOHA for 3 weeks, has restored endothelial function. In contrast, inhibition of arginase activity did not change the weight, the diameter of ankles, radiological or histological articular damage in AIA rats.Our work has led to a better understanding of pathophysiology of ED associated with rheumatoid arthritis and determined for the first time the deleterious role of arginase in this vascular anomaly. These results open prospects for the use of arginase inhibitors as future pharmacological treatment of vascular patient PR.

1 Abstract: The aim of this study quantify and compare the differences in the dynamics of foot movement compared with static in adulthood and old age, focusing on the midstance phase. A secondary objective is to outline the relationship with the angular distribution of pressure on the sole. For measurements two measuring methods were used. The kinematic analysis named Qualysis was used as a main method and system of plantar pressure insoles Pedar X as a complementary method. The difference in measured distance of 1-MP calcaneus, 5MP - calcaneus and pronate angle of calcaneus was prooved using the statistical calculation, which confirmed the hypothesis of half clear a significant degree. Subsequently, the changes were further specified using the results of the distribution of maximum force and contact time of the soles.

Title: Problems of the functional flatfoot Objective: The aim of this diploma thesis is problems of the functional flatfoot. Main goal is to describe relation between change of structure, disorder of foot function and development of flatfoot. The thesis tries to describe the behavior of foot and arch in standing and walking. Another goal is to summarize the diagnostic possibilities of flatfoot. Methods: The diploma thesis has descriptive-analytical character. It's written in form of literature review. Results: This thesis demonstrates the problem of disunity nomenclature concerning the problem of flatfoot. Specifically, it concerns the interpretation of the words of functional flatfoot, movement components in foot and laboratory diagnostic methods. There is offered an explanation of the concept of functional flatfoot. Further work indicated that the acquired flatfoot is a question of a functional disorder that may progress to structural changes. The static and dynamic loadings were confirmed by the role of small muscles of the foot in the role of dynamic stabilization and influence of femoral rotation on the foot arch. Title: Use of orthopaedic insoles in childhood Supervisor: PhDr. Tereza Nováková, PhD. Problem definition: The importance of the longitudinal arch is one of the most discussed topics in orthopaedics. Similarly important is the indication of orthopaedic insoles for flat feet especially in children. Experts have different opinions about what age insoles should be prescribed, and if their use is beneficial. So far, a clearly defined boundary has not been established between real pathology and a flexible flat foot formed on the basis of hypermobility which is to some extent physiological in childhood. Aim: To compare the plantograms of groups of six-year-old children with diagnosed pes planus/planovalgus before and after six month (or more) use of, individually prepared, insoles. To compare the results with a group of children that did not have insoles prescribed and on this basis to judge whether there was change (improvement) that can be proven with the use of a plantogram. Method: Two groups of six-year-old subjects were formed. The experimental group: subjects with prescribed orthopaedic insoles, and the control group: subjects that were not prescribed orthopaedic insoles. Title: Evaluation plantogram and range of motion of the joints of the lower extremity with hallux valgus deformity. Objectives: The aim of my thesis, "Evaluation plantogram and range of motion of the joints of the lower extremity with hallux valgus deformity" is a statistical comparison of the aspects of the lower limbs in healthy subjects and people affected by hallux valgus deformity. It will be a comparison of the measured values, such as the degree of hallux valgus angle, range of motion of the hip joint to spin, Q - angle of the lower limb, position of hocks and strained soles. Methods: Plantographic evaluation of the strain on the soles on Podoscope, goniometry, photography, plantographic diagnostic method - Chippaux - a Šmírák and Sztriter - Godunov. Results: During this research, I observed twenty seven individuals. Five individuals had hallux valgus deformity, fourteen individuals had valgus position of the thumb and seven individuals had no deformity. In the course of evaluating plantogram and range of motion of the joints of the lower limb with hallux valgus deformity I observed degree of flat. Problem: Running is one of the most widespread physical activities. Incorrect running technique, training quantity but also the runner's equipment can strongly affect health condition of the individual, both positively and negatively. For that reason various trends emerge in the area of running footwear. They include classical sports shoes with different types of impact attenuation as well as the minimalist footwear made to imitate barefoot running, which should be much more natural for humans, as insisted by B. Nigg in his book "Born to Run" - where he says that man was born to walk and run barefooted. Inappropriate shoes can cause various health complications, not only in the foot area. Objective: To find out how foot strike changes in the area of the foot-shoe interaction in running on flat ground when an amateur runner uses minimalist and sports footwear and to draw conclusions for training practice on the basis of the results. Methods: The thesis deals with amateur endurance running in different shoe types (minimalist and sports shoes).
