Modelování vývoje povrchu patra člověka od 7 let do dospělosti / Modelling of human palate surface development from 7 years to adulthood

The diploma thesis deals with the differences in the development of the palate in girls and boys within five age groups in the age range of 7 to 19 years, using methods of geometric morphometry. Furthermore, the work deals with the monitoring of sexual dimorphism in the development of the palate. Understanding palate growth, knowledge of differences in its development and changes related to sexual dimorphism are very useful, as the results of this study will serve as a comparative standard for palate defects in the Czech population, but also for appropriate planning of orthodontic procedures. The material consists of 228 gypsum castings of palate without any pathologies, 112 of which belonged to boys and 116 girls of the Czech population. Thus, transverse data are used in our study. The subjects were divided into five age groups 7, 10, 12, 15 and 19 with respect to gender. Age changes of the entire palate surface were modeled using geometric morphometry methods (Coherent point drift - Dense correspondence analysis, Per vertex T- test and Principal component analysis). Hodnocení morfologie patra u BCLP pacientů s palatoláliemi / Evaluation of the palate morphology in bilatelar cleft lip and palate clefts with palatolaly

The diploma work was based on the evaluation of dental casts of patients with bilateral cleft lip and palate (BCLP) with a mean age of 10. Patients consist of two groups, patients without defect in speech and with speech impairment (palatolaly). Palatolalies in the literature are primarily associated with velopharyngeal insufficiency. The study tested the working hypothesis that in the failure of speech is involved a different, specific in some way, palatal shape. Dental casts were scanned using a laser scanner and analyzed by 3-D geometric morphometry and multivariate statistics: principal component analysis (PCA), linear regression analysis and finite element analysis (FESA). Using linear regression it was found that the shape of the palate is affected in younger individuals by age, and so had to be 5 patients excluded for further analysis. Patients with palatolaly have lower variability the palatal shape than patients without palatolalie, so their palates are similar to each other and have a specific shape. Palates are wider and lower than in individuals without speech disorder and they have a characteristic deepening behind the anterior part of the palate. Clefts of orofacial komplex are the most common birth development defects. One of them is Pierre Robin sequence, which is characterized by presence of micrognathia, glossoptosis and upper airway obstruction. Because of these symptoms, isolated cleft palate "U" or "V" shaped can be part of the diagnose. Other clinical manifestations are feeding difficulties, obstructive sleep apnea syndrome and / or gastroesophageal reflux disease. The aim of the treatment is to secure the airway and the child's thriving after birth by using conservative or invasive treatment methods. Choosing of methods and procedures should be established by multidisciplinary cooperation. The other part of diploma thesis is trying to find out, which clinical manifestation are the most common in patients from the Czech Republic compare to foreign patients and the differences in management of the treatment. Furthermore we are trying to investigate, whether there is higher percentage of patients with delayed neuromotor development among PRS patients, which are under physical therapy. The last part is trying to find out the relationship between the poor posture and PRS diagnose in children age 4 - 6 years.

The presented thesis summarizes the results of research on craniofacial morphology in patients with facial cleft defects in relation to therapeutic approaches (Caganova et al., 2014; Dadáková et al., 2016; Hoffmannova et al., 2016; Hoffmannova et al., 2018; Moslerová et al., 2018). The effect of therapy in individuals with pathological growth disorders cannot be evaluated without detailed auxological studies of control subjects whose facial morphology, longitudinal changes, or manifestations of sexual dimorphism were evaluated upon similar methodology (Koudelová et al. 2015). Therefore, the thesis was conceived as a volume of six publications complemented with a general synthetic introduction into the area of study. Together, the thesis includes probands in a broad age spectrum from birth to 15 years with a total of 294 facial 3D scans, 36 tele-X-ray face images, 3D scans of 112 gypsum palate castings. The methods of geometric morphometry and multidimensional statistics prevail in the assessment. The main clinical part of the thesis deals with the influence of two types of surgery on the facial growth and development of patients with cleft palate, namely secondary spongioplasty (SS) and neonatal cheiloplasty (NCH). The topic of this diploma thesis is an analysis of interdisciplinary approach to children with orofacial clefts. The first chapter deals with theoretical base and defines terminology - communication, communication disabilites. It also defines anatomy and physiology of nasal cavity and oral cavity. The second chapter relates to issues of orofacial clefts. It describes classification, etiology, symptomatology, prevention, aestetic and psychosocial impacts and orofacial cleft children care. The third chapter deals with occurence of communication disabilites of children with orofacial clefts. It focuses on the definition of palatholaly, velopharyngeal inadequacy and it describes various linguistic levels and speech therapy and intervention. The fourth chapter is empiric and it deals with analysis of interdisciplinary approach to children with orofacial clefts. In research processes there were used scientific literature analysis, anamnestic data analysis and available specialized materials analysis. The diploma thesis elaborates the project documentation for the construction of a new apartment of central fire station type C1 with the administrative part of the Territorial Department. The fire station is technologically linked to the second building SO.02 the technical and technological background of the fire station. The building is situated on the eastern edge of Svitavy. The building is non-cellular, it has three floors and a flat green roof. Three floors fulfilling matter with mainly administrative function is connected with two floors blocks fire station with an indoor gym. Above the floor containing garages for emergency vehicles facilities for the emergency unit are placed with a curtain that creates fading of the emergency door. The matter of the gym is optically separated from the station and highlighted with a volume narrow neck and height elevation. In the station there are training places and places for general physical preparation. The buildings are located in a sloping terrain. The ground fire station has a heliport. The establishment supporting columns of the building are for made of prefabricated calyx, and are concreted into monolithic foundation pads. Infill wall vertical bearing and non-bearing structures are designed from clay blocks type THERM. The bearing system is reinforced concrete prefabricated frame. Horizontally bearing structures are designed from prestressed hollow core slabs. The building envelope is made of a curtain wall in dark grey and red combined with a light contact thermal insulation system used on the indoor gym. The envelopes are in the administrative part and the part of the station divided by a glazed area. In the workshops and technology room the envelope is divided by lamellae. Usually there are 13 firemen during one shift, in three shifts there work 39 firemen all together. There are supposed to be 17 employees in the administrative department.
