Postoje k porodu u českých žen / Attitudes toward childbirth in Czech women

Nowadays, women have a wide range of sources of information about childbirth. Women's attitudes toward childbirth are made by sharing experiences between women and in families but also they are based on information from media and internet. Consequently attitudes toward childbirth influence the choice of kind of delivery and women's childbirth experience. These days we can see some trends in obstetrics - the rising interest in natural births, on the one hand, and the rising number of women delivering their babies by cesarean section on the other hand. The goal of this socio-psychologically oriented thesis is to map women's attitudes to these most striking birth trends, and also to identify the reasons for these women's attitudes. The research part is thus focused qualitatively and uses the data from the analysis of the internet discussion forums for mothers. In the previous theoretical part are presented the key information necessary to understand the studied topic, such as the issues of attitudes, the specifics of the communication on the internet discussion forums and above all the current scientific knowledge about the attitudes of women to childbirth in the Czech Republic and the world. The Abstract The aim of my thesis is to find and describe the problematic aspects of the determination of parenthood. I chose four areas of the determination of parenthood, which can lead to various complications - assisted reproduction, surrogacy, secret childbirths and anonymous births. In the first chapter of the thesis I deal with the legal aspects of the determination of parenthood. I examine mandatory determination of maternity and various assumptions of paternity. With the development of relations in the society and of the reproductive medicine, there emerge possibilities doubting the traditional legal principles of the determination of parenthood. Mother is not always certain anymore and there are five presumptions of paternity in the Czech legal order nowadays, compared to the traditional three. The second chapter is devoted to the issue of assisted reproduction. Assisted reproduction is a modern and rapidly developing area of medicine that allows infertile couples to have their own child. It is a medical specialty that requires a broad legal framework to prevent its abuse. At the end of a successful medical procedure a new life is coming into existence and such area must be regulated. Structured summary Aim of the study: To compare the diagnostic reliability, accuracy, and safety of amniocentesis and amniotic fluid Interleukin-6 testing in the diagnosis of intrauterine inflammation of patients with preterm premature rupture of membranes. Type of study: Prospective cohort study Name and location of study site: Department of Gynaecology and Obstetrics, Faculty of Medicine, Charles University in Pilsen Set and methodology: We prospectively examined patients with pPROM between the 23rd and 34th week of gestation in 2014 - 2017. All of them underwent amniocentesis and determination of IL-6 levels in amniotic fluid, leukocytes and bacteria in amniotic fluid as well as maternal blood examination for inflammation parameters. The results were compared to histological examination of the placenta after delivery for the presence of chorioamnionitis. Based on the values mentioned above the sensitivity, specificity, negative and positive predictive value, false positive and negative predictive value and accuracy of the test were determined together with an assessment of statistical significance. The present diploma thesis is focused on the evaluation of steroid metabolome in body fluids in human pregnancy and parturition and also concentrates on the estimation of steroids in men with epilepsy (MWE) regarding the influence of epilepsy and its treatment with antiepileptic drugs (AEDs). The results obtained eventuate to the following conclusions: 1. They support the proposition about the key importance of corticoliberin for the timing of human parturition via stimulation of the steroidogenesis in the fetal zone of the fetal adrenal (FZ). Changes in steroid levels, however, much better predict the timing of parturition than the variation in corticoliberin. 2. The distribution of isoforms of placental oxidoreductases determines the excess of oxidized forms of the sex steroids and NAS in the fetus and the excess of their reduced forms in mother. This mechanism fundamentally affects the steroid bioactivity. The results indicate the role of NAS in pregnancy sustaining, their analgesic effect around parturition and their role in the pathophysiology of postpartum blues.

I focus on the nature of the discourses of the birth themselves and on the form of their controversies, which are tied to their confrontations in Czech web articles. I examine the discourses and their strife in the articles of active participants of the dispute and on the most popular website in the articles dedicated to (future) parents. The discourse of the actively medically controlled delivery, perceives a birth as an inscrutable risky phenomenon which is medically necessary to keep always in a hospital with a staffing and material background to deal with possible complications. This discourse criticizes the births that are led outside hospitals. The discourse of the natural birth sees a birth as a natural part of life, during which usually no complications occur, if there is present a trained expert, who does not interfere in the natural process of labor, except any required medical reasons. A natural birth can be taken place in home. The reader are warned by this discourse on the improper routine care in Czech hospitals, which is inconsistent with the needs of the mother and the child, and on complications of a birth caused by the active medical leading of labor. This diploma thesis deals with home births and how they were referred to in the media regarding the medial cause with a midwife Ivana Königsmarková. The goal of this work is to describe approaches that can be traced on this cause, what is the gender dimension of these approaches like and how does the aspect of power manifest itself on them. To achieve that I've chosen a method of discourse analysis which is the best one to point out the relation between language, power and ideology. Analysed material is represented by the articles from Czech media that are associated with the given cause and on basis of which the most frequently occurring topics can be traced along with their tendencies throughout time. Last but not least I'm closely analysing two chosen articles which represent two dominant ways of thinking - the medical approach of labour and natural or alternative approach.

The topic of this thesis is woman's experience with natural childbirth and its potential for disruption of medical discourse, which is in the Czech Republic in obstetrics still dominating. Through the method of discoursive analysis I am trying to generate any behavior that may be in conflict with social expectations and may question norms and standards in the field of pregnancy and obstetrics. I am also trying to trace whether and how does the deepening and gaining of the birth knowledge lead to the transformation of woman's identity. Theoretically, the work is grounded in a post-structuralist paradigm and builds on the concepts of biopower, performative acting and authoritative knowledge. Data sources are stories written in support of a midwife Ivana Königsmarková and published on the internet (www.pribehyproivanu.cz).

In the diploma thesis, objectives were set that were intended to find out medical, mental and social problems of women after complicated childbirth and to compare the problems with those of women after physiological childbirth. The other objective was to find out what kinds of complicated deliveries occur in Nemocnice Písek, a.s. /Písek Hospital/ the most. Hypotheses were set for the objectives. The first hypothesis was to confirm that birth injuries after complicated childbirth are healed per secundam. The second hypothesis was to confirm that women after complicated childbirth feel that they will not manage care for their family. The third hypothesis was to confirm problems with economic support of a family in the cases of single mothers after complicated childbirth. The fourth hypothesis was to confirm that women after complicated childbirth do not have enough information about social security. The fifth hypothesis was to confirm that in Písek Hospital, the most frequent complicated deliveries include Cesarean sections.

Determination of Parenthood The topic of my thesis is Determination of parenthood. The reason why I chose this topic is because it combines legal, historical, social and ethical aspects. Goal of my thesis is to analyse legal institute and raise awareness about its problems. The thesis is concerned with determination of maternity, determination of paternity and denying of paternity. Diploma thesis is divided into five chapters. First chapter is Introduction, in which I determine the extent of the thesis. Second chapter deals with historical development of legal regulation of determination of parenthood. This chapter comprises of three subchapters. First is concerned with determination of parenthood in ancient Rome. Second explains determination of parenthood in ABGB. Third explains the Czech legal regulation between 1948 and 2014.
