Minding the body : questions of embodiment and the practice of psychoanalytic psychotherapy.

It is well understood that psychoanalysis began with Freud’s encounter with hysteria and his work with illnesses of the mind which manifested in bodily terms. However, despite its close connection to the body and the understanding that psychoanalytic theory and practice develop hand-in-hand, psychological conflict that expresses itself in physical terms and more especially the role of the two bodies in the therapy room has received relatively little attention. The topic of this research project is captured in its title: “Minding the Body”, and the four journal articles it presents interrogate the relationship between the mind and body of both the patient and therapist. The thesis begins with two published papers which focus on the body of the patient, rehearsing and extending the psychoanalytic theory of bodily psychopathology and the implications that the different understandings of the relationship between body and mind in different forms of psychosoma have for clinical interventions. The second two papers examine what the analyst’s interpretation of her somatic responses to the patient, and the patient’s engagement with the analyst’s body, can reveal about the dynamics of the therapeutic dyad. The project concludes with a discussion of the clinical implications of a greater focus on the two bodies in the room, suggesting that the techniques developed to make sense of the patient’s physical symptoms can be usefully applied to decode the somatic countertransference as it manifests in a particular therapeutic dyad. That process, coupled with an awareness of the patient’s engagement with the therapist’s body, can create conditions under which the analyst’s body may become an analytic object and this can add significantly to the analytic repertoire.

O dispositivo do aconselhamento na resposta à aids

O presente trabalho, apresentado sob a forma de quatro artigos, teve por objetivo problematizar a prática do aconselhamento em HIV/aids, realizada em Centros de Testagem e Aconselhamento em DST/Aids (CTA). Fundamentou a investigação, referenciais teóricos e metodológicos alinhados com esforços na construção de conhecimento-emancipação. A realidade é concebida como complexa, dinâmica e aberta, marcada por jogo de forças em disputa cujos vetores que o constituem informam sobre tensões, conflitos e arranjos específicos entre eles. Abordaram-se, na pesquisa, dois aspectos do trabalho de aconselhamento, dentre tantos, processos subjetivos em torno da aids e a intervenção, isto é, os modos de aconselhar. Compôs a pesquisa, análise de Manuais do Ministério da Saúde (MS) para a prática do aconselhamento e análise de Entrevistas com aconselhadores/as atuando em unidades de CTA de Pernambuco. Conclui-se que o aconselhamento merece ser tratado como um dispositivo e não meramente como uma técnica de trabalho, pois envolve diversos aspectos na sua configuração, dentre eles, discursos, enunciados científicos, regulamentações, preceitos morais, estruturas arquitetônicas. De acordo com a pesquisa, o aconselhamento tem se apresentado como dispositivo de controle dos corpos, já que apoiado em concepção de subjetividade-identidade, definida pelas práticas sexuais do sujeito e em recomendações diretivas e prescritivas para o aconselhador, que caracterizam o modo-instrução de aconselhar. Outra perspectiva, que aparece sutilmente nas análises, como potência que pode ser investida, é que o aconselhamento seja um dispositivo de emancipação de sujeitos e coletividades. Caracteriza-se pela concepção de subjetividade-cidadã na qual o sujeito é ativo e participante das questões em torno da saúde como direito e pelo modo-relação de aconselhar, marcado pela horizontalidade e referências democráticas na relação profissional-usuário. As duas etapas de investigação subsidiaram o debate sobre a formação de aconselhadores/as. A partir de uma analítica do aconselhamento como modalidade de trabalho em saúde, que na sociedade capitalista convive com fortes contradições, como a possibilidade da saúde ser tratada como uma mercadoria, são apresentadas proposições para a formação de aconselhadores em HIV/aids, na direção de uma perspectiva emancipatória de sujeitos e coletividades. À Psicologia cabe assumir a tarefa de participante no jogo de forças em questão, parecendo estratégico aproximar suas formulações dos produtivos debates em torno de direitos humanos e direitos sexuais, no campo da aids.

African worldviews : their impact on psychopathology and psychological couselling

This study investigates the role that African traditional beliefs and practices play in defining psychological problems, determines how these beliefs and practices manifest in a counselling relationship and explores how Western based forms of counselling manage these manifestations in counselling. This investigation is in the context of the on-going debate on the relevance of Western Psychological counselling in South Africa and the rest of Africa, including my experience during my internship to register as a Counsellor. It explores the impact of conducting counselling with clients whose worldviews are different from those of the counsellor and focuses on the impact of the client’s worldviews on psychological well-being, psychological ill health and the resolution of psychological problems. Psychological well-being, ill health and counselling were discussed from a Western perspective. The study found that the client participants defined their psychological problems in terms of their African traditional beliefs and practices. They communicated their presenting psychological problems in ways that created possibility of miscommunication between themselves and their counsellors, for example by using figurative language. There was also a clear distinction between how psychological problems are managed from an African traditional perspective (ritualistic) and a Western perspective (talking therapy). The study recommended the creation of specific departments in Universities to embark on research aimed at establishing foundational structures on which to build an African Indigenous Psychology as an alternative to Western Psychology. More comprehensive research on African people’s attitudes is, suggested, on what traditional Africans think of psychological counselling. Another recommendation accruing from the limitation on sampling in this study is that future studies should be conducted with larger and more diverse samples; moreover, data should be gathered on a wider variety of demographics and cultural belief systems and practices. To counter prejudice and ignorance, the counsellor ‘to be’ should study African culture and customs during their BA Honours studies. On-going training and workshops on cross-cultural issues from various cultures should be part of the counselling profession. More emphasis should be placed on prevention and therefore more mental health clinics in the rural areas need to be opened and general education on psychological issues and cultural integration be initiated. / Psychology / M.A. (Psychology)
