Effects of short term stretching on ankle stiffness and range of motion in people with multiple sclerosis

Hypertonia is seen in 85% of people with Multiple Sclerosis (pwMS) resulting in disability and functional restrictions. Hypertonia can be caused by increases in passive stiffness and enhanced stretch reflexes (spasticity) and is frequently managed clinically using passive stretches. However, the optimal parameters of stretching such as the applied torque and stretch duration remain unclear. During commonly prescribed ankle plantarflexor stretches pwMS produced higher torques when standing in a weight bearing position compared to stretches applied using the upper limbs. Stretches could be held for 120 seconds on average and stretch duration was mainly limited by fatigue. People with higher disability tended to favour more supported stretching positions. The effects of stretching for either 30 or 10 minutes using a customised motor at three torque levels covering the range that MS participants could produce was investigated. Compared to the 10 minute stretch, greater reductions in passive stiffness and greater increases in range of movement (ROM) were seen immediately following the 30 minute stretch with the effects being sustained for the 30 minute post stretch period. Higher levels of applied torque resulted in a greater change in ROM however; there was no effect of applied torque on passive stiffness. Stretch reflex mediated stiffness was unaffected by the stretching intervention and showed transient post stretch increases. Ultrasonography was used to investigate changes in muscle–tendon length and strain in pwMS and controls and following stretching. PwMS showed evidence of stiffer muscles and increased tendon length at baseline compared to controls. Following a 10 minute stretch overall muscle length did not increase in pwMS, although increases in strain in the musculotendinous junction region were observed suggesting that more proximal regions of the muscle was likely to have contributed significantly to overall stiffness. This work highlights that stretch duration and levels of applied torque are critical factors in determining the effectiveness of stretches. The pathological mechanisms underlying hypertonia at a molecular and structural level and the effects of stretching on components of the musculo-tendinous structure and on functional ability should be ascertained.

An investigation into the mechanisms of acute effects of dynamic stretching on ankle joint mechanics and running economy

Warm-up routines commonly include stretching to increase flexibility (joint range of motion - ROM), optimise performance, and reduce the risk of injury. Literature suggests that static stretching as part of the warm-up routines decreases force and power production compared to an active warm-up or a warm-up including dynamic stretching, and therefore could be detrimental to performance. This has led to an increased interest in the use of dynamic stretching by many athletes while the benefits of such interventions and their potential mechanisms of action are not well understood. Studies presented in this thesis were conducted to examine the effects of acute dynamic stretching on aspects of performance (e.g. torque production capacity of the plantarflexors and running economy) and to identify possible neuromechanical mechanisms underpinning any potential changes. Furthermore, we attempted to examine whether altered pain tolerance/perception to stretch may be a contributing factor to the increased ROM using adaptations in the neural substrates involved by using functional magnetic resonance imaging (fMRI) technique. In the first study, both slow dynamic stretching and fast dynamic stretching increased ROM, and this was due to an increased tendon elongation. Importantly, dynamic stretching was not detrimental to the torque producing capacity of the ankle plantarflexors. Effects of dynamic stretching on the sensorimotor performance remained mainly unclear. Employment of shear wave elastography technique in the second study suggested an increase in muscle stiffness, a decrease in fascicle strain, and showed an increase in muscle thickness after dynamic stretching, supporting an increase in tendon compliance as a contributing factor to increased flexibility after dynamic stretching. In the third study, the improved running economy by dynamic stretching may be attributable to the decreased dynamic joint ankle and vertical stiffness. The fMRI study was not conclusive due to methodological issues. Present findings have practical implications for the use of dynamic stretching in sporting contexts.

The neuromuscular effects of a long-term static stretching program on the human soleus

The purpose of this research was to evaluate the effects of a long-term static stretching program on flexibility and spinal reflexes in the human soleus muscle. Day-to-day reliability over two days of H-reflex gain (Hslp/Mslp) and pre-synaptic inhibition of spinal reflexes in the human soleus was measured in an initial study. Thirty subjects (age=23.4 ± 3.9 yr, height=175.64 ± 10.87 cm, weight=84.5 ± 24.18 kg) with no history of lower leg pathology and/or injury within the previous 12 months volunteered. The slopes of the ascending portions of the H-reflex and M-wave recruitment curves were used to evaluate H-reflex gain (Hslp/Mslp). The mean soleus H-reflex from 10 conditioning stimulations and 10 unconditioned stimulations was used to calculate the amount of pre-synaptic inhibition. Measurements of H-reflex gain and pre-synaptic inhibition yielded test-retest reliability of R=.95 and R=.91, respectively. The contribution of pre-synaptic and post-synaptic reciprocal mechanisms in flexibility adaptations has not been measured during the same study. A long-term static stretching program (5 times per week for 6 weeks) of the soleus implemented within an experimental group of 20 subjects was used as an intervention to measure both spinal control flexibility changes. Additionally, 20 control subjects were measured that did not participate in the stretching program. Passive ankle dorsiflexion, Hmax/Mmax, H-reflex gain (Hslp/Mslp), pre-synaptic and post-synaptic reciprocal mechanisms were measured at baseline, 3 weeks, and 6 weeks for all 40 subjects. A 2 (Group) X 2 (Sex) X 3 (Time) mixed MANOVA with Tukey HSD with (a<0.05) was used. Only ROM had a significant interaction between Group and Time, whereas, a significant difference was not detected in the other dependent variables. The experimental group demonstrated significantly improved dorsiflexion ROM from baseline to 3 weeks (mean=6.2 ± .88, P<0.001), 3 weeks to 6 weeks (mean=4.9 ± .84, P<0.001), and baseline to 6 weeks (mean=11.2 ± .91, P<0.001). Consequently, the increase in flexibility by 42% after 30 stretching sessions was not the result of spinal reflex changes. / Graduation date: 2006
