'Journeys through depression' : patients' experiences of transformational change through mindfulness based cognitive therapy (MBCT) and antidepressant medication (ADM)

Background: Mindfulness-based cognitive therapy (MBCT) is a promising new alternative to anti-depressant medication (Kuyken et al., 2015) and whilst some qualitative studies have explored participants' experiences of MBCT, none yet have explored experiences of participants who are considering coming off their antidepressant medication alongside MBCT or how patients experience change in relationships with self, others and illness. Aim: To examine MBCT participants' experience of change across 24 months, particularly in relation to change in views of their self and their illness over time. Method: Thematic analysis of in-depth retrospective interviews with 42 participants, two years after attending an 8 week MBCT group with an invitation to taper their antidepressant medication (ADM). Each participant took part in one retrospective interview which was semi-structured and focused on experiences of MBCT and ADM over the previous two years since attending an MBCT group and how these have impacted on a change in self and experience of illness. Findings and conclusion: Four over-arching themes were found: taking control, relationships (with self, other and illness), rebuilding the self and shifts in perspective. The findings in the current study are very similar to those found in transformation in the physical chronic illness literature (e.g. Paterson et al., 1999). Perhaps MBCT could be the challenge which lead patients suffering from chronic depression towards change and creates a context in which patients can consider self and identity.

In the city of Huambo, the second largest in the country and the center of the armed conflict, destroyed by combats, the Baptist Church creatively worked as a community in solidarity. It did not accept the misery imposed by the war, but with the people, discovered that the crisis offered a wide variety of opportunities to fulfill its mission. It performed as if it were a catalyst, gathering together desires, organizing partnerships, provoquing reflexions, serving the war victims and the poor in general. In the middle of the suffering, churches discover the importance of independence, see that only through the mutual cooperation and with the will to seek the other s interest, that it is possible to achieve common goals and change realities. At the same time that the church acted as an agent of change, it also had to adapt its methodology and change its missionary practices. In another words, the church made a change and was changed at the same time. Chapter one offers a historical background beginning with the colonial period until contemporary times. Chapter two presents the effects of a devastating war on the life of a population, and how the church interacted with this culture of war. Finally, chapter three analyzes the pastoral praxis of the church in the midst of the armed conflict, acting as a solidary community, partner in the national reconstruction enterprise and as a promoter of the praxis of hope. Due to the fact that it is a case study, this text offers a critical analysis of the pastoral praxis developed by the Baptist Church of Huambo, during the period of 1985 to 2002, offering insights on those living in a situation of armed conflicts. / Quatro décadas de guerra trouxeram conseqüências desastrosas para a nação angolana. Na cidade do Huambo - a segunda do país e o epicentro do conflito armado - destruída pelos combates, a Igreja Batista, exerceu de maneira criativa o seu papel de comunidade solidária. A Igreja não se resignou à miséria imposta pela guerra, mas junto com o povo descobriu que a crise traz oportunidades imperdíveis para exercer sua missão. Seu papel foi catalisador, reunindo vontades, organizando parcerias, provocando reflexão, servindo vítimas de guerra e os carentes em geral. Em meio à dor, as igrejas descobrem a importância da interdependência, percebem que somente através da cooperação mútua e na busca do bem do outro é que será possível alcançar objetivos comuns e transformar realidades. Ao mesmo tempo em que a igreja era um agente de transformação, ela também teve que adequar sua metodologia e mudar suas práticas missionárias, ou seja, a igreja transformava e era transformada.O primeiro capítulo traz uma retrospectiva histórica do período colonial até a contemporaneidade; o capítulo dois trabalha os efeitos de uma guerra devastadora na vida da população e como a igreja interagiu com esta cultura de guerra. Finalmente, no capítulo três, a análise da práxis pastoral da igreja em meio ao conflito armado atuando como uma comunidade solidária, parceira no empreendimento de reconstrução nacional e promotora da práxis de esperança. Por ser um estudo de caso, o trabalho apresenta uma análise crítica da práxis pastoral desenvolvida pela Igreja Batista na cidade do Huambo no período de 1985 a 2002, podendo vir a ser uma leitura suplementar para quem conviver em situações de conflito armado.

Analýza transformace civilní ochrany v České republice od roku 1918 do současnosti s návrhem dalšího směřování ochrany obyvateltva / An Analysis of the Civil Protection Transformation in the Czech Republic from 1918 to the Present with a Proposal of the Future Inhabitants Protection

Civil protection as well as the protection of the population is a dynamic field which has a deep history in the Czech Republic dating back to the dark time of World War I. The civil protection was first based on the voluntariness principle. The real origin of the civil protection came with Act No. 82/1935 Coll. on the protection and defense against air attacks. The aim of this thesis is to characterize the protection of the population and propose the future direction of protecting of the population through changes or innovations that would strengthen the protection of the population. In the practical part of this thesis is based on the research question: "Has the civil protection transformation established a reliable system that does not require either major changes or minor changes?" The research part of the thesis is based on a qualitative research. The practical part of the thesis is based on a research method "structured interview". The structured interviews were conducted with the experts at the regional level all seven territorial departments of the Fire and Rescue Services of the South Bohemian Region. The conclusion of this thesis (mainly based on the strucutred interviews with the experts) is a statement that by the civil protection transformation was created a reliable system of protection of the population with a minor space for improvement. The fundamental change in the system would be a consolidation of the system legislative into a separate law on civil protection / protection of the population. The current crisis legislation would be subordinate to this Act. Other proposed changes to the protection of the population and its further direction of protection of the population are presented in the section "Conclusion".
