Este artigo aborda a Política Nacional de Saúde do Trabalhador e da Trabalhadora, destacando-se as atuais diretrizes e prioridades do modelo de atenção integral à saúde do trabalhador, com enfoque no processo de descentralização das ações, no âmbito regional. Partindo-se da percepção de contradições no interior da própria política - a universalidade da atenção à saúde do trabalhador e a real efetivação da equidade de direitos, objetiva-se apreender possíveis entraves que possam estar dificultando a implantação e consolidação desta política, regionalmente. Trata-se de um estudo de revisão de literatura do tipo descritiva, baseado em publicações de documentos oficiais, tais como leis, políticas, portarias, relatórios, normas e manuais técnicos, assim como artigos e capítulos de livros pertinentes ao tema no período de 2002 a 2012. O artigo desenvolveu uma análise dos determinantes que levam ao surgimento desta política, seus objetivos, princípios, diretrizes e estratégias, com o intuito de que este exame possa contribuir para a compreensão das dificuldades e possibilidades quanto à sua real efetivação. Considera-se relevante esta análise, tanto pelo seu caráter de novidade, quanto pela necessidade de aprofundamentos e reflexões para a busca da melhoria da qualidade de serviços prestados aos trabalhadores, e por desdobramento, à população. Percebe-se, ao longo da análise das evoluções desta política, que a saúde do trabalhador refere-se a um processo social complexo, trazendo as marcas do seu tempo. Este processo é caracterizado pela produção de saberes e fazeres que devem se concretizar em novas modalidades de atenção e cuidado ao sofrimento do trabalhador em toda a Rede de Atenção do SUS. Conclui-se que não há dúvida quanto à extrema importância da conquista de um aparato legislativo que garanta o acesso universal à saúde do trabalhador e coloque o Estado como responsável por isto; todavia, é necessário, também, que esse movimento não se esgote com essa garantia. / This article discusses the National Policy on Occupational Health and Working, highlighting the current guidelines and priorities of the comprehensive health care worker model, focusing on the process of decentralization of actions at the regional level. Based on the perception of contradictions within the policy itself - the universality of health care worker and the actual realization of equality rights, the objective is to seize any obstacles that may be hindering the deployment and consolidation of this policy, regionally. This is a study of the literature review descriptive, based on publications of official documents such as laws, policies, regulations, reports, standards and technical manuals, as well as articles and chapters in books related to the theme from 2002 to 2012. The article developed an analysis of the determinants that lead to the emergence of this policy, its objectives, principles, guidelines and strategies, in order that this examination may contribute to the understanding of the difficulties and possibilities as to its real effectiveness. Considers being relevant this analysis, both by its novelty, as the need for insights and reflections to the pursuit of improving the quality of services provided to workers, and by unfolding the population. It can be seen throughout the analysis of the evolution of this policy, that health worker refers to a complex social process, bringing the brands of its time. This process is characterized by the production of knowledge and practices that must be realized in new forms of care and attention to the suffering of workers across the NHS Care Network. We conclude that there is no doubt as to the extreme importance of achieving a legal apparatus that ensures universal access to health worker and put the state as responsible for this; however, it is also necessary that this movement is not exhausted with this guarantee.
