Previous issue date: 2017-09-12 / The present work sought to understand the importance of the diagnosis in the treatment direction
in the clinical care processes in a clinic-school of psychology in the state of Tocantins. The
realization of this thesis arose from the questions about the direction of the treatment. Discussion
object of this study will be the Barbara case that will be analyzed from a psychoanalytic
perspective and whose objective is to present the reconstruction of a clinical case with the
purpose of discussing the psychological complaint with the consultations performed in order to
reflect and rethink the importance of the diagnosis During the course of psychotherapeutic care.
From this reconstitution, some reflections on "psychological complaint", the psychic suffering,
the diagnosis and the direction of the treatment are woven. To preserve the anonymity of the case
and proceed to its ethical respect the patient will be referred to by the pseudonym "Barbara".
Psychological appointments were performed by a psychology trainee from the ninth period who
received weekly supervisions. The sessions lasted 50 minutes, once a week at the clinic school of
the Colinas Integrated Higher Education College. The research is qualitative with an emphasis on
the psychoanalytic approach with the aim of contributing to the training of professionals working
with psychotherapeutic processes. When dealing with the field of speech psychotherapy, it is not
possible to dissociate therapy from psychopathological aspects, since it may imply a loss in the
process. For example, a patient who has a dynamic mode of psychotic subjectivation, treatment
can not be conducted with the same clinical orientation of a neurotic. The diagnostic hypothesis
is important in this orientation and can not be treated with something static but rather dynamic.
The perspective of the diagnosis in psychoanalytic psychology does not adopt a generalist
position, but a study of each case to understand the dynamics and the operation of each subject to
be able to treat their suffering. Thus, the intention was to seek a clinical methodology that could
understand the subject and with that to guide the subject so that he could learn from his own
suffering. / O presente trabalho buscou compreender a importância do diagnóstico na direção do tratamento
nos processos de atendimentos clínicos em uma clínica-escola de psicologia no estado do
Tocantins. Suas origens remontam a questionamentos sobre a direção do tratamento. Seu objeto
de estudo foi o “caso Bárbara”, analisado na perspectiva psicanalítica. O objetivo foi reconstruir
um caso clínico problematizando a queixa psicológica com os atendimentos realizados para
refletir sobre a importância do diagnóstico no percurso dos atendimentos psicoterápicos. Tal
reconstrução apresenta reflexões sobre a “queixa psicológica”, o sofrimento psíquico, o
diagnóstico e a direção do tratamento. Para preservar o anonimato do caso e para o respeito ético,
a paciente será referida através do pseudônimo de Bárbara. Os atendimentos psicológicos foram
realizados por uma estagiária de psicologia do nono período e que recebia supervisões semanais.
As sessões tinham duração de 50 minutos, uma vez por semana na clínica escola da Faculdade
Integrada de Ensino Superior de Colinas. A pesquisa é de cunho qualitativo com ênfase na
abordagem psicanalítica com o intuito de contribuir para a formação de profissionais que
trabalham com processos psicoterápicos. Ao se tratar do campo das psicoterapias pela fala não se
pode dissociar a terapêutica dos aspectos psicopatológicos, pois pode implicar um prejuízo no
processo. A um paciente que tem um modo dinâmico de subjetivação psicótica, o tratamento não
pode ser conduzido com a mesmo orientação clínica de um neurótico. A hipótese diagnóstica é
importante nessa orientação e tem de ser tratada não como algo estático, mas com algo dinâmico.
A perspectiva do diagnóstico em psicologia psicanalítica não adota uma postura generalista e sim
um estudo de cada caso para compreender a dinâmica e o funcionamento de cada sujeito para
poder tratar o seu sofrimento. A intenção foi buscar uma metodologia clínica que pudesse
compreender o sujeito e poder orientá-lo a aprender com seu próprio sofrimento.
