Previous issue date: 2016-08-25 / Introduction.Ankylosing spondylitis (AS) is a chronic disease featuring axial changes, peripheral arthritis and systemic involvement. Proinflamammatory cytokines are probably involved in AS pathogenesis. The relationship of circulating cytokines with instruments of AS evaluation is an open field. We herein compared serum levels of cytokines in AS patients and healthy controls, and searched for correlations of cytokines with indexes of disease activity and quality of life. Methods. In this cross-sectional study, 32 AS patients and 32 age-and-sex matched controls were evaluated. BASDAI, BASFI, BASMI, ASDAS-CRP, MASES, ASQol and PGA were measured in AS patients. The soluble cytokines IL-6, IL-8, IL-1, IL-10, TNF-?, IL-12p70 and IL-17 were quantified by flow cytometry. IL-23 concentrations were obtained in an ELISA. Results. Overall, AS patients were predominantly males (59.4%) and Caucasians (96.9%). Mean age was 46.9?10.7 years. HLA B27 was present in 70% of cases. Concentrations of IL-6, IL-8, IL-10 and TNF-? were higher in AS cases than controls. Mean concentration of IL-6 correlated with the BASMI, an index of axial mobility (rs= 0.354, p=0.047). Anti-TNF intake (present in 21 patients, 65.6%) associated with a high BASMI (p=0.042) and lower quality of life as measured by the ASQol (p=0.009). Conclusions. A proinflammatory cytokine profile predominated in AS patients, but interestingly the IL-10 concentrations were also elevated, pointing to an attempt of a suppressive control of inflammation. A defined correlation of serum IL-6 with the BASMI suggests a role for this cytokine in axial disease. Anti-TNF users had more axial activity and lower quality of life. / Introdu??o. A espondilite anquilosante (EA) ? uma doen?a inflamat?ria cr?nica que acomete predominantemente o esqueleto axial (coluna vertebral e articula??es sacril?acas); observa-se tamb?m artrite perif?rica e manifesta??es sist?micas. Citocinas pr?-inflamat?rias provavelmente est?o envolvidas na patog?nese da EA. A correla??o entre as concentra??es s?ricas de citocinas com instrumentos de avalia??o cl?nica na EA ? tema em aberto. Nesta pesquisa comparamos n?veis s?ricos de citocinas em pacientes com EA e controles saud?veis e correlacionamos com ?ndices de atividade de doen?a e qualidade de vida. M?todos. Estudo transversal, controlado, incluiu 32 pacientes pareados por idade e sexo com 32 controles saud?veis. Os instrumentos de avalia??o cl?nica BASDAI, ASDAS-PCR, BASFI, BASMI, MASES, ASQoL e AGP foram aplicados nos pacientes com EA. As citocinas sol?veis IL-6, IL-8, IL1?, IL-10, TNF-?, IL-12p70 e IL-17 foram quantificadas por citometria de fluxo. As concentra??es de IL-23 foram obtidas por ELISA. Resultados. Os paciente com EA eram predominantemente homens (59.4%) e caucasianos (96.9%). A idade m?dia foi de 46.9 (?10.7 anos). O HLA-B27 estava presente em 70% dos casos. Concentra??es de IL-6, IL-8, TNF-? e IL-10 estavam mais elevadas nos pacientes com EA em rela??o aos controles. As concentra??es m?dias de IL-6 se correlacionaram com o BASMI, um ?ndice que avalia a mobilidade axial (rs=0.354, p=0.047). O uso de medica??es anti-TNF (presente em 21 pacientes, 65.6%) foi associado a um BASMI elevado (p= 0.042), significando pior mobilidade axial, e baixa qualidade de vida avaliada pelo ASQoL (p=0.009). Conclus?es. Um perfil de citocinas pr?-inflamat?rias predominou nos pacientes com EA; de forma intrigante a IL-10, uma citocina anti-inflamat?ria, tamb?m estava elevada, indicando uma tend?ncia a um controle supressivo da inflama??o. A correla??o significativa da IL-6 com o BASMI sugere a participa??o desta citocina na les?o axial. Usu?rios de anti-TNF tiveram mais les?o axial e pior qualidade de vida.
