Memoria para optar al título de Químico Farmacéutico / La Atención Farmacéutica es el ejercicio profesional que implica la interacción
directa entre el farmacéutico y el paciente en la cual ambos se comprometen a
trabajar conjuntamente para alcanzar una meta terapéutica y tiene como objetivos
fundamentales entregar una farmacoterapia efectiva; identificar, prevenir y resolver
problemas relacionados con el uso de medicamentos y mejorar la calidad de vida
de los pacientes.
Esto condujo a implementar un Plan de Atención Farmacéutica para pacientes
ambulatorios hipertensos en un Centro de Atención Primaria en Salud y evaluar el
uso de medicamentos de estos pacientes, con el fin de mejorar la efectividad y la
seguridad de los tratamientos farmacológicos y no farmacológicos.
El diseño del Plan incluyó la elaboración de material educativo, la utilización de
material didáctico y la realización de encuestas.
En la realización del mismo se introdujo un grupo control para comparar
estadísticamente si los resultados obtenidos eran significativos o no.
Al analizar los resultados obtenidos del Plan se observó un aumento en los
conocimientos generales obtenidos por los pacientes de sus patologías y
medicamentos, además se consiguió incrementar la adherencia al tratamiento del
grupo intervenido y tanto los pacientes del grupo intervenido como del grupo control
disminuyeron sus valores de presión arterial. En los primeros la presión sistólica
varió de 157,37 a 137,26 mmHg y la presión diastólica de 88,58 a 81,05 mmHg,
mientras en los del segundo grupo la presión sistólica varió de 156,47 a 150,16
mmHg y la presión diastólica de 89,91 a 86,62 mmHg.
Se podría concluir que el programa fue efectivo ya que se cumplieron todos los
objetivos planteados en este Plan lo cual pudo verificarse al finalizar el seguimiento
y se lograron diferencias estadísticamente significativas con el grupo control en las
variables más relevantes / Pharmaceutical Care is the professional exercise that implies the direct interaction
between the pharmacist and the patient in which both are committed to work jointly
to reach a therapeutic goal and has like main targets to give an effective
farmacoterapia; to identify, to prevent and to solve problems related to the medicine
use and to improve the quality of life of the patients.
This lead to implement a Pharmaceutical Care Plan for hypertensive ambulatory
patients in a Center of Primary Attention in Health and to evaluate the medicine use
of these patients, with the purpose of to improve the effectiveness and the security
of the pharmacological and nonpharmacological treatments.
The design of the Plan included the elaboration of educative material, the use of
didactic material and the accomplishment of surveys.
In the accomplishment of the same a control group introduced itself to compare
statistically if the obtained results were significant or no.
When analyzing the obtained results of the Plan an increase in the general
knowledge obtained by the patients of its pathologies and medications was
observed, in addition it was able to increase the adhesion to the treatment of the
taken part group and the patients of the group taken part as of the control group
diminished its values of arterial pressure. In the first, systolic pressure varied from

while in those of the second group the systolic pressure varied from 156,47 to

had statistically significant differences.
It would be possible to be concluded that the program was effective since all the
objectives raised in this Plan were fulfilled which could verify when finalizing the
pursuit and statistically significant differences with the control group were obtained
in the most excellent variables
