Objetivo: Comparar la calidad de vida (CV) en mujeres sobrevivientes de cáncer de cuello uterino de acuerdo con el estadio en el momento del diagnóstico.
Materiales y métodos: Se desarrolló un estudio de corte transversal analítico en mujeres que acudían a los consultorios externos del servicio de ginecología oncológica de un hospital de referencia nacional del Seguro Social en Salud en Perú. Las participantes fueron divididas en dos grupos: grupo A, conformado por mujeres con antecedente de estadio I y grupo B, por los estadios II y III. La calidad de vida se midió con los cuestionarios EORTC QLQ-C30 y QLQ-CX24. Se midieron otras variables sociodemográficas y clínicas. Se utilizaron modelos de regresión lineal crudos y ajustados con bootstrap no paramétrico con sesgo corregido y acelerado con un intervalo de confianza al 95%.
Resultados: El grupo A y B estuvieron conformados por 59 y 61 mujeres, respectivamente. En los modelos ajustados por edad, instrucción y presencia de comorbilidades, se encontraron diferencias estadísticamente significativas en:CV global (β=-8,84; p<0,001), función física (β=-3,68; p=0,025), emocional (β=-4,34; p=0,046), cognitiva (β=-7,18; p=0,001), social (β=-12,40; p<0,001), fatiga (β=6,02; p=0,022), constipación (β=14,82; p=0,002), diarrea (β=8,02; p=0,007), dificultades financieras (β=10,18; p<0,001), imagen corporal (β=-8,37; p<0,001), goce sexual (β=8,72; p=0,025), funcionamiento sexual/vaginal (β=-13,36; p<0,001), experiencia sintomática (β=4,99; p=0,001) y neuropatía periférica (β=12,95; p<0,001).
Conclusiones: La calidad de vida de las sobrevivientes de cáncer de cuello uterino tiene mayor deterioro en mujeres cuyo estadio al diagnóstico fue de II y III. Se sugiere una valoración médica integral e intervenciones médicas. / Objective: To compare quality of life (QoL) in cervical cancer survivors according to the stage at the time of diagnosis.
Material and methods: An analytical cross-sectional study was carried out in adult female patients in gynecology oncology clinics of a national reference hospital of the Social Security in Health in Peru. The participants were divided into two groups: group A, consisting of women with a history of stage I and group B cancer, stages II and III. Quality of life was measured by the EORTC QLQ-C30 and QLQ-CX24 questionnaires. Other sociodemographic and clinical variables were also measured. We performed crude and adjusted linear regression models with non-parametric bootstrap with corrected and accelerated bias with a 95% confidence interval.
Results: Group A and B were 59 and 61 women, respectively. In the models adjusted by age, education and presence of comorbidities, we found statistically significant differences in: global QoL (β=-8,84; p<0,001), physical functioning (β=-3,68; p=0,025), emotional (β=-4,34; p=0,046), cognitive (β=-7,18; p=0,001), social (β=-12,40; p<0,001), fatigue (β=6,02; p=0,022), constipation (β=14,82; p=0,002), diarrhea (β=8,02; p=0,007), financial difficulties (β=10,18; p<0,001), body image (β=-8,37; p<0,001), sexual enjoyment (β=8,72; p=0,025), sexual/vaginal functioning (β=-13,36; p<0,001), symptom experience (β=4,99; p=0,001) and peripheral neuropathy (β=12,95; p<0,001).
Conclusions: The QoL of cervical cancer survivors has more deterioration in women whose stage at diagnosis was II and III. A comprehensive medical assessment and medical interventions are suggested.
