Colelit?ase em pacientes bari?tricos: correla??o da perda de peso com a incid?ncia de colelit?ase em pacientes ap?s a realiza??o do bypass gastrintestinal

Previous issue date: 2014-07-07 / Objective: Obesity treatment with Roux-en-Y gastroplasty has been widely used for the last few decades. Patients who undergo this surgery can develop gallstones in the months following the procedure, just as occur in people who lose weight with conventional treatments. The purpose of this study is to determine the incidence of cholelithiasis in patients that have had this surgery and to analyze its possible correlation with the rapidity of weight loss. Methods: A retrospective cohort of morbidly obese patients submitted to Roux-en-Y gastroplasty was studied. The patients were evaluated at specific times: Time 1 was before obesity treatment surgery, and Time 2 was postoperatively, before 12 months time had passed. During Time 2, the speed of weight loss and the occurrence or not of cholelithiasis within this period was noted. Results: Records of 1,161 patients were analyzed, 312 (26.9%) of which had cholelithiasis or had already had cholecystectomy preoperatively. Among the 849 remaining patients, 447 (52.6%) maintained follow-up for 6 months postoperatively, and 128 (28.6%) developed cholelithiasis during this period and presented with an average weight loss of 66.3 ? 16.8%. Between 7 and 12 months, 238 individuals completed the planned follow-up, and of these, 27 (11.3%) developed cholelithiasis and had an average weight loss 22.9 ? 9.8%. Conclusion: The time needed to lose weight appears to maintain a direct correlation to the development of cholelithiasis in patients who have undergone surgical obesity treatment, mainly in the first six months of postoperative. / Objetivo: O tratamento cir?rgico da obesidade por meio da cirurgia de gastroplastia em Y de Roux est? sendo amplamente empregado nas ?ltimas d?cadas. Sabe-se que os pacientes submetidos a essa terap?utica podem desenvolver c?lculos em ves?cula biliar nos meses subsequentes ao procedimento, assim como ocorre em pessoas que perdem peso com tratamentos convencionais. Propomos um estudo com intuito de determinar a incid?ncia de colelit?ase nos pacientes submetidos a esse procedimento e avaliar sua poss?vel correla??o com o tempo de perda de peso. M?todos: Foi realizada uma coorte hist?rica em pacientes obesos, com ?ndice de Massa Corporal (IMC) ≥35kg/m? submetidos ? cirurgia de gastroplastia em Y de Roux. Os pacientes foram analisados em momentos distintos: momento 1: antes da realiza??o da cirurgia para tratamento da obesidade; momento 2: ap?s a realiza??o do procedimento at? 12 meses de p?s operat?rio. Nesse segundo tempo foi avaliada a perda de peso e o surgimento ou n?o de colelit?ase ao longo do tempo especialmente nos 6 meses e nos 12 meses. Resultados: Foram avaliados prontu?rios de 1161 pacientes, sendo que desses 312(26,9%) apresentaram colelit?ase ou eram colecistectomizados no pr?-operat?rio. Dentre os 849 pacientes restantes, 447(52,6%) mantiveram o acompanhamento esperado nos primeiros 6 meses de p?s-operat?rio, sendo que 128(28,6%) desenvolveram colelit?ase nesse per?odo, apresentando perda de excesso de peso m?dia (%EWL) de 66,3?16,8%. No per?odo entre 7 e 12 meses, 238 indiv?duos completaram o seguimento conforme preconizado, desses 27(11,3%) desenvolveram colelit?ase, com %EWL m?dia de 22,9?9,8%. Conclus?o: A perda de peso parece manter correla??o com o desenvolvimento de colelit?ase nos pacientes submetidos ao tratamento cir?rgico da obesidade principalmente nos primeiros seis meses de p?s-operat?rio.
