Previous issue date: 2015-07-02 / Introduction: The objective of this study was to analyze the factors associated with variability of body mass index (BMI) and percentage of excess weight loss (%EWL) of patients undergoing Roux-en-Y gastric by-pass (RYGB) throughout 60 months. The following factors were analyzed: gender, age, surgical access (laparotomy vs. laparoscopy), preoperative BMI, waist circumference (WC), type 2 diabetes mellitus (T2DM), high blood pressure, and dyslipidemia.Material and Methods: Retrospective cohort study using a convenience sample of 2,070 patients of both gender, aged 18 to 65 years, undergoing RYGB between 2000 and 2013. Patients were divided into quartiles according to the distribution of minimum BMI and maximum %EWL. Follow-up lasted 60 months. Results: The four most significant factors in terms of variation of BMI over 60 months in descending order were preoperative BMI, preoperative WC, surgical access, and age. Regarding %EWL, the most important factors were surgical access, preoperative BMI, preoperative WC, and age (p<0.001). After 36, 48, and 60 months, approximately 50% of patients had BMI>30 kg/m?. As for %EWL, we found that 17% of patients had %EWL <50% and 40 % of patients had between 50% to 75% %EWL and 24 % of patients had %EWL above 75% to 90% and 19% of patients had %EWL above 90%. Conclusion: There was a more significant decrease in BMI and %EWL in the first 6 months, followed by a gradual increase in both outcomes. Considering the association with BMI and %EWL, the most important factors were: pre-operatory BMI and waist circumference, surgical approach and age. Further studies are needed to better understand the role of these factors, including their role as possible event predictors. / Introdu??o: O objetivo desse estudo foi analisar os fatores associados ? variabilidade do IMC e do %PEP no p?s-operat?rio de pacientes submetidos by-pass g?strico em? Y de Roux? (RYGB) ao longo de 60 meses, avaliando o impacto dos seguintes fatores: g?nero, idade, acesso cir?rgico (laparotomia vs. laparoscopia), IMC pr?-operat?rio, circunfer?ncia abdominal (CA), diabetes mellitus tipo 2 (DM2), hipertens?o arterial (HAS) e dislipidemia. Materiais e M?todos: Realizamos um estudo de coorte retrospectivo, utilizando uma amostra de conveni?ncia de 2070 pacientes de ambos os g?neros entre 18 a 65 anos submetidos ao RYGB nos anos de 2000 a 2013. Os pacientes foram divididos em quartis conforme a distribui??o do IMC m?nimo e do %PEP m?ximo, observados ao longo do per?odo de 60 meses. Resultados: Os quatro fatores que se destacaram em ordem decrescente na varia??o do IMC ao longo de 60 meses foram: IMC pr?-operat?rio/circunfer?ncia abdominal pr?-operat?ria, acesso cir?rgico e idade. Para o %PEP os fatores que mais se destacaram foram acesso cir?rgico, IMC pr?-operat?rio/circunfer?ncia abdominal pr?-operat?ria e idade, todos com P<0,001. Ap?s 36, 48 e 60 meses, aproximadamente, 50% dos individuos apresentaram IMC>30kg/m?. Quanto ao %PEP, aos 60 meses observou-se que aproximadamente 17% dos individuos apresentaram %PEP<50%, 40% dos indiv?duos alcan?aram entre 50% e 75% do %PEP, 24% dos individuos apresentaram %PEP acima de 75% a 90% e 19% dos individuos atingiram mais de 90% da perda do excesso do peso. Conclus?o: Aos 60 meses de acompanhamento, observamos que a queda do IMC e do %PEP foram mais acentuadas nos primeiros seis meses, seguidas de um reganho gradual para ambos os desfechos. Na associa??o com o IMC e o %PEP, os fatores de maior relev?ncia foram: IMC e circunfer?ncia abdominal pr?-operat?rios, acesso cir?rgico e idade. Investiga??es futuras ser?o necess?rias para melhor elucidar o papel desses fatores e inclusive avaliar sua contribui??o como poss?veis preditores de desfecho.
