Previous issue date: 2004 / Resumo: Objetivo: Avaliar a resistência insulínica e o perfil lipídico das pacientes com síndrome de Turner (ST) e correlacionar com a idade, o cariótipo, e a presença de hipertensão arterial sistêmica, de obesidade e de desenvolvimento puberal. Casuística e Métodos: Estudo transversal de 35 pacientes com ST sem uso prévio de anabolizantes ou hormônio de crescimento, com avaliação de idade, cariótipo, pressão arterial, desenvolvimento puberal, estatura, peso, IMC, cintura (C), quadril (Q), C/Q. Foram feitas dosagens séricas basais em jejum de colesterol total, HDL, triglicérides, insulina e glicose, e cálculos de LDL, índices de HOMA e QUICKI, e relação entre glicose e insulina (G/I). Foi feita análise descritiva dos dados, e aplicados o teste de Mann-Whitney em relação aos grupos de cariótipo e puberdade e o de Spearman. Resultados: A idade variou de 5 a 43 anos, sendo 10 acima dos 20 anos. Dezessete com cariótipo 45,X e 6 com aberrações estruturais; não se observaram diferenças das variáveis em relação aos cariótipos. Quinze eram impúberes e 20 púberes; os triglicérides e o HOMA foram significativamente maiores na puberdade, e a G/I menor. Sete com estatura normal, 8 com IMC > 25 Kg/m2 (6 entre 25 e 30 e 2 maior que 30), 19 com C/Q > 0,85. O colesterol foi de 180 + 42 mg% (Média + 1DP, 4 acima de 240); o HDL de 57 + 16 mg%; o LDL de 99 + 34 mg%; os triglicérides de 108 + 96 mg% (2 acima de 200); o HOMA de 1,01 + 0,71; o QUICKI de 0,4 + 0,04 e a G/I de 23,5 + 12,1 (2 abaixo de 7,0). Conclusões: Neste estudo foram observadas poucas alterações no perfil lipídico da ST independentemente da faixa etária, do cariótipo, da hipertensão arterial e da obesidade. A resistência insulínica não foi observada neste estudo com a freqüência descrita na literatura / Abstract: Objective: To evaluate the insulin resistance and lipid profile of patients with Turner Syndrome (TS), and to check the influence of age, karyotype, presence of systemic arterial hypertension, obesity and pubertal development. Casuistic and Methods: A transversal study of 35 TS patients without previous use of anabolic steroid or growth hormone, with evaluation of age, karyotype, blood pressure, pubertal development, height, weight, BMI, waist (W), hip (H), and W/H (waist-to-hip ratio). Fasting blood was collected to determine the levels of total cholesterol, high-density lipoprotein, triglycerides, insulin and glucose, followed by the estimate of low-density lipoprotein, HOMA and QUICKI indexes, and relation between glucose and insulin (G/I) was made. A descriptive analysis was done and data were examined by Mann-Whitney test for pubertal and karyotype groups, and Spearman test. Results: The age varied from 5 to 43 years; 10 were above 20 years old. Seventeen had a 45,X karyotype and 6 structural aberrations; differences of the variables in relation to the karyotypes were not observed. Fifteen were nonpubertal and 20 pubertal; triglycerides and HOMA were significantly higher in puberty, while G/I was lower. Seven had normal height, 8 had BMI > 25 Kg/m2 (6 between 25 and 30 and 2 > 30), and 19 W/H > 0.85. The cholesterol level was 180 + 42 mg% (Mean + 1SD, 4 above 240); HDL 57 + 16 mg%; LDL 99 + 34 mg%; triglycerides 108 + 96 mg% (2 above 200); HOMA 1.01 + 0.71; QUICKI 0.4 + 0.04 and G/I 23.5 + 12.1 (2 below 7.0). Conclusions: This sample shows that a few patients with TS had changes in lipid profile independent of age, karyotype, hypertension and obesity.
