O uso do doador "marginal" no transplante de figado : resultados em trinta pacientes

Previous issue date: 1995 / Resumo: Este estudo teve como objetivo avaliar os resultados dos transplantes de ti gado, realizados com órgãos de doadores considerados "marginais". Foram estudados, prospectivamente, 213 pacientes submetidos a transplantes de fígado, realizados no período de 18 meses. No momento da captação dos órgãos, os doadores foram classificados em duas categorias: doadores "marginais" e doadores ideais. Do total, 30 doadores foram considerados "marginais", por serem portadores de uma ou mais das seguintes condições: história de alcoolismo, provas de função hepática alteradas, intoxicação por superdose de paracetamol, doença cardiovascular avançada, infecção, período longo de hipotensão, altas doses de substâncias inotrópicas, doença de von / Abstract: The aim of this study was to avaluate the outcome of liver transplantation perfonned with organs that carne from marginal donors. During a period af 18 month, 213 liver transplantations were studied prospectivelly. The donors accepted were classified in two categories: "marginal" donors and "good" donors. From the total, 30 donors were considered marginaIs due to present one or more ofthe following conditions: abnonnalliver function tests, history of alcohol hepatic steatosis, von Willebrand disease, paracetamol overdose, advanced cardiovascular disease, high-dose inotropic use, lengthy hipotension, infection, advanced age, obesity. The remainig 183 donors, fullfilled the criteria for "good" donors as following: age between two and 45 years old, nonnalliver function tests, no history of alcoholism, no steatosis, trauma or hepatic ischemia, no cardiac no hipotensioll or hipoxia, dose inotropic use < 10 µ g/Kglmin, hematocrit > 30016, UTI stay < five days, no previous disease or condition that could cause any dammage to the liver or hepatobiliary system. The outcome ofthe liver transplantation perfonned with organs from marginal donors (group ~ n = 30) were compared with the liver transplantation performed with good livers (group B, n = 183), at the sarne period. The transplant in both groups were similar concerning to: the type of the procedure (elective, urgent or emergency) total of blood units transfused during the intra-operative period, cold alld wann ischemia time, UTI and ward stay, graft prlmary llon function, one month graft survival alld one year graft, alld recipient survival. The groups differed only regarding greater AST levels on the first day and between the first and the fifth day post-transplantation, on the group A cornpared to the group B. These data showhed that the liver transplantations perfonned With "marginal" livers and "good" livers presented similar outcome. The results ofthis study indicatet that many livers, wruch carne from marginal donors, are potelltially good for transplantation / Doutorado / Doutor em Cirurgia
