243039.pdf: 3202612 bytes, checksum: bd0b150105874da2227fd10ebcb47e54 (MD5) / Estudo de abordagem qualitativa apoiado na interdisciplinaridade dos pressupostos epistemológicos do novo-paradigma da ciência pós-moderna, cujo objetivo foi desvelar e compreender a dinâmica das relações da família com o idoso portador de Diabetes Mellitus tipo 2, no aspecto da compreensão da dinâmica de inter-relações do sistema familiar em sua dimensionalidade contextual com a doença crônica, o diabetes. Fundamentado no pensamento sistêmico subsidiado por Bertalanffy, Capra e Vasconcellos e na transversalidade com outros autores sistêmicos, sua orientação metodológica derivou-se da observação naturalística desenvolvida na perspectiva de Lincoln e Guba, que convergiu num processo de análise de categorias criadas a partir do próprio discurso das famílias segundo modelo interativo de Huberman e Miles. Para alcançar a compreensão da dinâmica de relações da família foram utilizadas entrevistas em profundidade, genograma e ecomapa com trinta e sete subsistemas individuais de nove famílias que tinham a experiência quotidiana da vivência com a doença no Município de Jequié, Bahia, e cujos dados foram coletados no período de julho/2005 a janeiro/2006. A partir dos instrumentos genograma e ecomapa foram sendo identificados os subsistemas que exerciam influência real ou simbólica na estrutura organizacional dos vínculos no sistema familiar que eram importantes para a compreensão da influência na dinâmica familiar e com os quais foram realizadas as entrevistas. A análise foi desenvolvida num processo de idas e vindas apoiada em Huberman e Miles: redução antecipada, redução concomitante e redução a posteriori. Do resultado dessa análise emergiram oito grandes temas que expressaram o movimento circular e em espiral das inter-relações das famílias e que foram sendo configuradas nas categorias: implicações da doença na dinâmica familiar; influências externas no processo da mudança; a família em busca do #balance# na sua dinâmica; a família e o contexto de suas relações circulares; o sistema familiar e a necessidade de retroalimentação; estratégias de adaptação da família à doença; a família e suas relações de parentesco # maneiras que busca para garantir o cuidado; a relação intergeracional na dinâmica familiar e cada qual com suas dimensões. Os resultados mais expressivos mostraram que: a doença influencia o processo de viver das famílias no quotidiano de suas inter-relações; cada família, como unidade, manifesta uma característica própria influenciada pelo complexo contexto histórico e sociocultural, entre outros; o cuidado familial torna-se essencial num sistema de saúde.
Study of qualitative approach based on the interdisciplinarity of the epistemological presuppositions of the postmodern science new paradigm whose objective was to attend and comprehend the family relations dynamics with the elderly bearer of Diabetes Mellitus type 2, in the aspect of the inter-relations dynamics comprehension of the familiar system in its contextual dimensionality with the chronic disease, the diabetes. Based on the systemic thought subsidized by Bertalanffy, Capra and Vasconcelos and in the transversality with other systemic authors, its methodological orientation has derived itself of the naturalistic observation developed in Lincoln and Guba#s perspective which has converged in a categories analysis process created from the own families discussion according to Hubermans and Miles interactive model. Interviews in depth, genogram and echo map have been utilized with thirty-seven individual subsystems of nine families which had the quotidian experience of existence with the disease in the community of Jequié, Bahia and whose data have been collected in the period from July/2005 to January/2006 to reach the dynamics comprehension of the family relations. The subsystems, which were being identified from the instruments genogram and ecomap, used to exert real or symbolical influence in the organizational structure of the links in the familiar system and with which ones have realized the interviews. The analysis has been developed in a going and coming process based on Hubermans and Miles: antecipated reduction; concomitant reduction and in posteriori reduction. Eight big themes which have emerged from the result of this analysis have expressed the circular movement and in familiar dynamics inter-relations spiral and that were being configured in the categories: disease implications in the familiar dynamics; the external influences in the change process; the family in search of the #balance# in its dynamics; the family and its circular relations context; the familiar system and the feedback necessity; family adaptation strategies to the disease; the family and its relationship relations # manners which search toguarantee the care; the intergenerational relation in the familiar dynamics and each one with its dimensions. The most expressive results have shown that: the disease influences the families living process in the quotidian of its inter-relations; each family, as unit, manifest an own characteristic influenced by the complex historical and sociocultural context, among others, the familiar care becames essencial in the health system.
