A angústia espiritual é um fenômeno subjetivo e foco de atenção do enfermeiro, que pode ser presenciado quando o ser humano está vivenciando algum momento conflituoso na sua vida, geralmente perante uma enfermidade, como o câncer. A relação entre a presença do câncer e o fenômeno de angústia espiritual é algo que merece atenção, visto que a junção dos dois pode comprometer ainda mais o estado de saúde. Assim, buscam-se ferramentas que sejam capazes de identificar e, se possível, mensurar esse fenômeno. Entretanto, a literatura brasileira não apresenta nenhum instrumento padronizado que seja capaz de avaliá-lo. Desse modo, o presente estudo teve por objetivo realizar a tradução, a adaptação transcultural e avaliar as qualidades psicométricas de uma Escala de Angústia Espiritual que se mostra presente na literatura no idioma inglês. Esse processo foi realizado seguindo as etapas de traduções, a síntese dessas traduções; as retrotraduções; a avaliação de equivalência cultural; a revisão por um comitê de juízes; o pré-teste; os testes de confiabilidade e a validade. Uma vez obtida a versão final da escala de angústia espiritual no idioma português, foi realizado o pré-teste e o teste de confiabilidade inter e intraexaminador com 20 pacientes diagnosticados de câncer em tratamento na Santa Casa de Caridade Nossa Senhora do Perpétuo Socorro de Alfenas. Para a avaliação da validade, foi utilizada a Escala de Bem-Estar Espiritual que foram aplicadas concomitantemente em 150 pacientes da mesma população. A análise de confiabilidade da escala foi realizada por meio dos coeficientes estatísticos de Kappa e Alfa de Cronbach e a de validade pela análise fatorial e coeficiente de correlação de Spearman. A análise intra e interexaminador foi considerada satisfatória, uma vez que mais da metade dos itens tiveram valores de Kappa acima de 0,75. Na análise de consistência interna utilizando o Alfa de Cronbach e na análise fatorial, apenas o domínio três “Relação com Deus” apresentou valores discrepantes em ambas as análises. Entretanto, esse fato não trouxe prejuízos ao estudo, visto que a escala no geral e três dos quatro domínios presentes na mesma, apresentaram parâmetros significativos e corresponderam à proposta da escala original. Como a angústia espiritual é um diagnóstico de enfermagem difícil de lidar na prática clínica por apresentar características definidoras complexas e subjetivas, além de ser extremamente pessoal e dinâmico. A utilização da Escala de Angústia Espiritual, adaptada e validada neste estudo no contexto brasileiro, poderá facilitar o processo de raciocínio clínico e atenderá à necessidade espiritual dessa população. Além disso, permitirá a identificação desse diagnóstico, uma vez que demonstrou ser um instrumento confiável, válido e eficiente, capaz de avaliar o fenômeno que se propõe a mensurar (angústia espiritual). / The spiritual distress is a subjective phenomenon and focus from nurse of attention who can be seen when the human being is experiencing sometime confrontational in their life, usually before an illness, as the cancer. The relationship between the presence of cancer and the phenomenon of spiritual distress is something that deserves attention, since the joining of the two can further compromise the health. So, search for tools that are able to identify and, if possible, measure this phenomenon. However, Brazilian literature presents no standardized instrument that is able to evaluate it. Thereby, the present study aimed to perform the translation, cultural adaptation and evaluate the psychometric properties of a Spiritual Distress Scale that if shows present literature in the English language. This process was carried out following the steps of translation, the synthesis of these translations; the back-translations; the cultural equivalence evaluation; the review by a committee of judges; the pretest; the tests of reliability and the validity. Once you get to the final version of the scale of spiritual distress in Portuguese, was performed pre-test and reliability test inter-and intra-examiner with 20 patients diagnosed if cancer in treatment in Holy House of Charity Our Lady of Perpetual Help of Alfenas. To evaluation the validity, was used the Scale Wellness Spiritual that were applied concomitantly in 150 patients from the same population. The reliability analysis of the scale was performed using the coefficient statistical Kappa and Alpha Cronbach and of validity by factor analysis and Spearman correlation coefficient. The analysis of intra and interexaminer was considered satisfactory, since more than half of the items had Kappa values above 0.75. In the analysis of internal consistency using Cronbach's alpha and factor analysis, only the domain three “Relationship with God” presented values discrepant in both the analyzes. However, this fact brought no harm to study, since the scale overall and three of the four domains present in the same showed significant parameters and corresponded to the proposed of original scale. As the spiritual distress is a nursing diagnosis difficult to cope with in practice by presenting complex and subjective defining characteristics, besides being extremely personal and dynamic. The use of Spiritual Distress Scale, adapted and validated in this study in the Brazilian context, can facilitate the process of clinical reasoning and answer the spiritual needs of this population. It will also enable the identification of this diagnosis, since proved to be a instrument reliable, valid and efficient, able to evaluate the phenomenon that purport to measure (spiritual distress). / Coordenação de Aperfeiçoamento de Pessoal de Nível Superior - CAPES
