Resultam em uma série de seqüelas que podem acompanhar o portador durante toda a vida apresentando ônus em termos de morbidade, estigmatização e exclusão social. O tratamento requer um acompanhamento contínuo desde o nascimento até a fase final do crescimento com o objetivo de prevenir e tratar transtornos estéticos, funcionais e psico-sociais, sendo que o cirurgião-dentista participa de todas as fases da reabilitação. O objetivo do trabalho foi avaliar o sistema de referência e contra-referência procurando conhecer as possibilidades para a implementação do atendimento integral ao portador de fissura de lábio e/ou palato (FLP), referente às ações e serviços odontológicos no âmbito do SUS. Avaliar o sistema de referência e contra-referência implicou em avaliar o sistema propriamente dito e o processo de trabalho dos cirurgiões-dentistas envolvidos no estudo. A metodologia envolveu a utilização de questionários dirigidos aos cirurgiões-dentistas envolvidos na atenção ao portador de FLP nos três níveis de atenção à saúde e uma entrevista semi-estruturada dirigida aos pais de crianças portadoras de FLP. Os resultados demonstram que há fragilidades referentes ao processo de trabalho bem como à organização do funcionamento do sistema. Os mecanismos de referência e contra-referência são realizados por meios formais e informais, porém sem a garantia do acesso aos serviços requeridos nos três níveis de atenção. Problemas de ordenação e hierarquização do sistema e de iniqüidade de acesso aos serviços, tornam a atenção fora do alcance de muitos pacientes e famílias. O processo de trabalho evidencia uma prática centrada em medidas curativo/assistenciais onde há fragmentação do tratamento o qual fica restrito à assistência realizada pelas especialidades e, a atuação da atenção básica, quando realizada, é centrada em atendimentos pontuais.
The cleft lip/palate is one of the most frequent congenital defects on the head and neck parts. They result in a series of sequelae that the affected person can carry for all his life, exacerbating the morbidity, stigmatization and social exclusion aspects. The treatment requires a constant evaluation from birth to the final development stage, its aim is prevent and treat the aesthetic, functional and psychological and social problems. The Dental Surgeon integrates all the rehabilitation stages. The aim of this study was to analyse the reference and contra-reference system to find the possibilities for implementation of the total health care of the FLP carrier, reporting to the actions and dental services at the SUS (Health Unique System). Evaluate the reference and contra-reference system implicated in evaluating all the health system and dental surgeon work process. The methodology involved the question/answer mechanism applied to the Dental Surgeons involved in the FLP carrier care in the three levels of health care and a structured interview directed to the carriers' parents. The results present fragilities related to the work process and organization of the system operation. The reference and contra-reference mechanisms are realized by formal and informal methods, but without the guaranty of access to the required services in the three levels of health care. Problems in organization and hierarchy of the system and iniquity in access to the services result in many patients and their families without treatment. The work process brings out a practice focused in curative procedures that results in treatment fragmentation and restricts the realization of assistance by the specialities. Furthermore, the basic attention is focused in punctual treatment when realized.
