Previous issue date: 2012-08-28 / Metabolic syndrome (MetS) is associated with increased risk of incidence type 2 diabetes, cardiovascular disease and insulin resistance and is considered an important cause of mortality and morbidity from cardiovascular disease. According to guidelines from the National Cholesterol Evaluation Program for Adult Treatment Panel III (NCEP ATP III) diagnosis of this syndrome occurs when a person has three or more risk factors between central obesity, hyperglycemia, hypertriglyceridemia, high blood pressure and low HDL-cholesterol. Others risk factors also been associated with MetS are inflammation, genetic and environmental factors. Among the genetic factors investigated as possible risk factors are identified some polymorphisms in the gene interleukin 6 receptor, already associated with dyslipidemia, obesity and insulin resistance. The objective of this study was to evaluate the response of individuals with Metabolic Syndrome (MetS) to a lifestyle modification intervention and the influence of polymorphism 48867A>C of IL6R (rs2228145) in this response. Seventy-seven sedentary individuals with at least three criteria for MetS were included and randomly divided into two groups: NI, nutritional intervention; NIE, nutritional intervention and exercise practice. Parameters were analyzed before and after three months of intervention. When comparing the effect of different intervention according to genotype, a decrease in triglyceride levels was observed (significant for AA genotype; for AC genotype only in NIE group); a significant reduction of fasting glucose level was observed in AA genotype, independent of intervention group, but for AC genotype, only in NI group; systolic blood pressure (SBP) showed significant reduction for AA and AC genotype. When analyzing parameters mean value according to genotype (isolated and in combination) before and after intervention, triglyceride mean levels differed significantly after intervention among genotypes and for AA compared to AC+CC; SBP showed difference before intervention for AA genotype and after intervention, among genotypes and for homozygous A. The analysis of number of individuals with altered parameters revealed statistically significant difference for AA genotype before intervention for triglycerides and diastolic blood pressure (DBP). After intervention, significance was observed for triglycerides, SBP and DBP among genotypes and for AA compared to AC+CC. In conclusion, our results suggest that response to intervention based on nutritional control and exercise practice could benefit mainly individuals homozygous AA of IL6R gene 48867A>C polymorphism / A s?ndrome metab?lica (SM) est? associada a um risco aumentado de incid?ncia de diabetes tipo 2, doen?as cardiovasculares e resist?ncia ? insulina, sendo considerada uma importante causa de mortalidade e morbidade por doen?a cardiovascular. Segundo orienta??es da National Cholesterol Evaluation Program for Adult Treatment Panel III (NCEP ATP III) o diagn?stico dessa s?ndrome acontece quando um indiv?duo apresenta tr?s ou mais fatores de risco entre obesidade central; elevados n?veis de glicemia, de triglicer?deos e de press?o arterial e baixos n?veis de HDL-colesterol. Outros fatores que tamb?m t?m sido associados ? SM, s?o inflama??o, fatores gen?ticos e ambientais. Entre os fatores gen?ticos investigados como poss?vel fator de risco est?o alguns polimorfismos identificados no gene do receptor da interleucina 6, j? associados a dislipidemias, obesidade e resist?ncia ? insulina. O objetivo deste estudo foi avaliar a resposta de indiv?duos com s?ndrome metab?lica (SM) a uma interven??o de modifica??o do estilo e a influ?ncia do polimorfismo 48867A> C do IL6R (rs2228145) nesta resposta. Setenta e sete indiv?duos sedent?rios, com pelo menos tr?s crit?rios para s?ndrome metab?lica foram inclu?dos e divididos aleatoriamente em dois grupos: IN, interven??o nutricional; INE, interven??o nutricional e pr?tica de exerc?cios. Os par?metros foram analisados antes e ap?s tr?s meses de interven??o. Quando foi comparado o efeito das diferentes interven??es de acordo com o gen?tipo, foi observada uma diminui??o dos n?veis de triglicer?deos (significativa para o gen?tipo AA, mas para o gen?tipo AC somente no grupo INE); uma redu??o significativa do n?vel de glicose em jejum foi observada no gen?tipo AA, independente do grupo de interven??o, mas para o gen?tipo AC, somente no IN grupo; a press?o arterial sist?lica (PAS) mostrou redu??o significativa nos gen?tipos AA e AC. Quando valor m?dio dos par?metros foi analisado de acordo com os gen?tipos (isolados e em combina??o) antes e ap?s a interven??o, os n?veis m?dios de triglicer?deos diferiram significativamente ap?s a interven??o entre os gen?tipos e no gen?tipo AA comparado com AC+CC; a PAS mostrou diferen?a antes da interven??o para o gen?tipo AA e ap?s a interven??o, entre os gen?tipos e para homozigotos A. A an?lise do n?mero de indiv?duos com par?metros alterados revelou diferen?a estatisticamente significante para o gen?tipo AA, antes da interven??o, para triglicer?deos e press?o arterial diast?lica (PAD). Ap?s a interven??o, foi observada signific?ncia para triglicer?deos, PAS e PAD entre os gen?tipos e para o gen?tipo AA quando comparado com AC+CC. Em conclus?o, nossos resultados sugerem que a resposta ? interven??o baseada no controle nutricional e na pr?tica de exerc?cios pode beneficiar, principalmente, os indiv?duos homozigotos AA do polimorfismo 48867A> C do gene IL6R
