Previous issue date: 2010-12-28 / The physiotherapy represents actually, in the medical field, therapeutic specialty which uses resources and approaches that include two sorts of medical rationalities: the biomedical and the holistic practice, in the context of health-disease-cure finds itself increasingly under discussion. Both classical physics (Cartesian Newton model) and quantum physics (bioenergetics model) are closely related to physiotherapy practice, this last one is still being investigated. Nowadays when we study the patient s body, we should not study it as machine separated into parts, but observe it as a whole as an integrated system. Because the body tension acquisition results, in a multi factorial formation that urges an analysis of the individual s bio psychosocial environment. Then it emerges the need of investigating far these both biomedical and holistic therapies can contribute to heal the dysfunctions of the neuromuscular skeletal system presented by patients with chronic low back pain. Among these two types of rationality some patients respond better to treatment than others, even when it comes to the same pathology. The discussion and understanding of medical rationalities, as regards the treatment of low back pain are needed to guide better the patients to a more specific physiotherapy. The present study aimed to understand the approaches to the treatment of chronic low back pain under the focus of biomedical and holistic medicine in the context of physiotherapy. We conducted a literary review on online journals in the period from May to December 2010 using the key words: low back pain, chronic pain, physiotherapy, holistic therapies, mind-body therapies and biological models. It was also made in pursuit of thematic in dissertations, theses and books of renowned authors who bring about the theoretical theme. The analysis of which medical rationality and model to be used for the treatment of chronic low back pain this study of literature review, does not even get an outcome goal. This is a complex investigation and thorough review of how a pain such a subjective phenomenon behaves on the individual, this being inserted in a determined bio psychosocial context. The present study provides further discussion of this theme in finding the most effective and efficient treatment of chronic back pain. What we can observe is that in some situations the junctions of the two approaches, biomedical and holistic when applied at the same time have a greater resoluteness in controlling symptoms in patients. From the literature review, we can see that nowadays there is already a paradigm shift in the biomedical knowledge and rationalities that are not exclusively at the forefront within the health-illness-healing process. / A fisioterapia atualmente representa, na área da saúde, uma especialidade terapêutica que se utiliza de recursos e abordagens que abrangem os dois tipos de racionalidades médicas: a biomédica e a holística, no contexto do processo saúde-doença-cura. Tanto a física clássica (modelo Newton-Cartesiano) quanto à física quântica (modelo bioenergético) estão intimamente relacionadas à prática fisioterápica, esta última ainda tem muito investigar. Sabemos atualmente que quando se estuda o corpo de um paciente não se deve estudá-lo como uma máquina separada por partes e sim observá-lo de uma maneira global, como um sistema integrado. Pois a aquisição das tensões corporais resulta de uma formação multifatorial em que urge uma análise na esfera biopsicossocial do indivíduo. Emerge então a necessidade de investigar até que ponto as terapias biomédicas e holísticas, no âmbito da fisioterapia, podem contribuir para solucionar as disfunções do sistema neuromusculoesquelético apresentadas pelo paciente com lombalgia crônica. Dentre esses dois tipos de racionalidades, alguns pacientes respondem melhor a um tratamento do que outros, mesmo quando se trata da mesma patologia. A discussão e compreensão das racionalidades médicas, no que diz respeito ao tratamento das lombalgias, são necessárias para melhor orientar os pacientes a um tratamento fisioterápico mais específico. O presente estudo teve como objetivo compreender as abordagens de tratamento da lombalgia crônica sob o enfoque da medicina biomédica e holística no âmbito da fisioterapia. Realizamos um levantamento literário em periódicos on line no período de maio a dezembro de 2010 utilizando as palavras-chaves: lombalgia, dor crônica, fisioterapia, terapias holísticas, terapias mente-corpo e modelos biológicos. Fizemos também a busca da temática em dissertações, teses e nos livros de autores reconhecidos que trazem embasamento teórico quanto ao tema abordado. A análise de qual racionalidade médica ou modelo deva ser utilizada para o tratamento da lombalgia crônica, nesse estudo de revisão de literatura, não permite ainda se obter um resultado definitivo. Trata-se de uma investigação complexa e minuciosa analisar como a dor, fenômeno tão subjetivo, se comporta no indivíduo inserido em um contexto biopsicossocial determinado. Essa pesquisa permite suscitar mais discussões dentro dessa temática na busca pelo mais efetivo e eficaz tratamento das lombalgias crônicas. O que se pode inferir é que em algumas situações a junção das duas abordagens, biomédica e holística, quando aplicadas concomitantemente possuem uma resolutividade maior no controle dos sintomas apresentado pelos pacientes. A partir da revisão de literatura feita, podemos verificar que atualmente já existe uma quebra de paradigma em que os saberes e as racionalidades biomédicas não se encontram exclusivamente em primeiro plano no processo saúde-doença-cura.
