Previous issue date: 2016-09-01 / Considerando os processos de reforma da Educa??o M?dica nos ?ltimos anos e o debate atual sobre a interioriza??o da medicina, verifica-se a necessidade de estudar as experi?ncias surgidas ap?s as DCN de 2001. Assim, buscou-se analisar a experi?ncia do Curso de Medicina da UFCG-Cajazeiras, bem como: fazer um relato hist?rico do curso; analisar os desafios atuais; reconhecer os fundamentos te?ricos do PPC; descrever a inser??o da Sa?de Coletiva/MFC no curr?culo; analisar se esse PPC atende ?s exig?ncias da APS; descrever como os aspectos referentes ao mundo do trabalho, projeto pedag?gico, abordagem pedag?gica, cen?rios de pr?tica e desenvolvimento docente s?o vivenciados; reconhecer contradi??es entre PPC e condu??o real; e extrair generaliza??es que embasem pol?ticas p?blicas. A pesquisa localiza-se no quadro referencial marxista, utilizando m?todos de procedimento hist?rico, monogr?fico e comparativo, com abordagem qualitativa de car?ter descritivo e explorat?rio, tendo, como t?cnicas de coleta de dados, a Documenta??o Indireta (pesquisas documental e bibliogr?fica) e a Observa??o Direta Intensiva - observa??o participante, inicialmente assistem?tica e, posteriormente, sistem?tica, seguindo instrumento desenvolvido por Lampert; e, enquanto t?cnica de an?lise de dados, o m?todo hermen?utico-dial?tico. A hist?ria do curso revela a tens?o entre baixa capacidade de absor??o da rede de sa?de local e caracter?sticas institucionais da UFCG (corpo docente e PPC). O primeiro aspecto foi p?lo principal da contradi??o nos primeiros anos, parcialmente contornado com redu??o de entradas anuais, viabiliza??o do internato em outras institui??es e instala??o de servi?os hospitalares pr?prios. Somente ent?o o Desenvolvimento Docente ganha principalidade nos desafios atuais. Partindo de problem?tica comum ? Medicina Rural, o PPC apresenta grande ecletismo: flerta com o Materialismo Hist?rico-Dial?tico via Epidemiologia Cr?tica Latino-Americana, que fundamenta sua compreens?o da Medicina, e Metodologia da Problematiza??o, subjacente ? compreens?o do aluno como sujeito e ? diversifica??o dos espa?os de aprendizagem; referenciando-se na Pedagogia das Compet?ncias, dialoga com correntes de cunho racionalista, individualista e neopragmatista; e, finalmente, a P?s-modernidade de Contesta??o de Boaventura de Sousa Santos e o Pensamento
Complexo de Edgar Morin fundamentam uma compreens?o ?tica do Ser Humano e da Ci?ncia, que norteiam os pressupostos de curr?culos baseados no humanismo e da pesquisa como eixo condutor do ensino. O curso est? alinhado ?s diretrizes da ABEM/SBMFC para o ensino na APS. Inserido desde o primeiro ano, com o ensino de abordagens coletivas do processo sa?de-doen?a, a forma??o na APS continua com ?nfase, a partir do quarto per?odo, na abordagem comunit?ria e familiar no territ?rio, dentro do qual se desenvolve a Resid?ncia em MFC e onde, nos anos intermedi?rios seguintes e no internato, treina-se abordagem da cl?nica individual na APS. Nesse percurso s?o trabalhadas metodologias diferenciadas, como viv?ncias, grupos tutoriais para acompanhamento de PTS, semin?rios, etc. O curso apresenta perfil Inovador com Tend?ncia Avan?ada, sendo o Desenvolvimento Docente seu ponto cr?tico, revelando sua contradi??o fundamental: avan?os no plano te?rico, mas que n?o s?o expressos totalmente na pr?tica. Conclui-se que as pol?ticas de interioriza??o do ensino m?dico deve priorizar a an?lise da rede de sa?de, e que mudan?as na Educa??o M?dica depende da a??o pol?tica, apresentando-se uma agenda de propostas. / Taking under consideration the medical education reform processes in recent years and the current debate on the transference of medicine undergraduate courses to the countryside, the need of studying the experiences that have arisen after the 2001 National Curriculum Guidelines is visible. As a result, it was sought to analyze the experience UFCG medicine school located in Cajazeiras, moreover, report the history of the course; evaluate the present challenges; consider the theoretical foundations of the Pedagogical Project of the Course (PPC); describe the insertion of Public Health / Family Medicine in the curriculum; consider whether this PPC meets the requirements of Primary Health Care; describe how the aspects related to the world of work, pedagogical project, pedagogical approach, practice scenarios and teacher development are experienced; recognize contradictions between PPC and real conduction; and excerpt generalizations that may support public policies. This research is based on Marxist frame of reference. Historical, monographic and comparative methods of procedure were used. It has a qualitative approach. Its characteristics are descriptive and exploratory. Indirect documentation (documentary and bibliographic research) and Intensive Direct Observation - participant observation, which, in the beginning was unsystematic and later systematic, following the instrument developed by Lampert, were adopted as data collection techniques. As data analysis technique, the hermeneutic-dialectic method. The story of the course reveals the tension between low absorptive capacity of the local health system and institutional characteristics of UFCG (teaching staff, PPC). The first aspect was the main pole of the contradiction in the early years, which was partially overcome thanks to the reduction of annual entries, possibility of supervised internships in other institutions and UFCG?s own hospital services establishment. Only then, the Teacher Development gained preeminence among the present challenges. Starting from common problems of Rural Medicine, PPC has a great eclecticism: it flirts with the Historical and Dialectical Materialism by Latin American Critical Epidemiology, which bases its understanding of medicine, and Methodology of Problematization, with the understanding of the student as a subject and diversification of learning spaces; referencing on Skills Pedagogy, conforms to
currents of rationalist imprint, individualistic and neopragmatist nature; and, finally, the Postmodernism of Contestation, of Boaventura de Sousa Santos, and the Complex Thought, of Edgar Morin, establish an ethical understanding of the human being and Science, which guides assumptions of curricula based on humanism and research as a conductor axis of teaching. The course is aligned with the guidelines of ABEM / SBMFC for teaching in primary health care, which is inserted since the first year, by teaching of collective approaches of the health-disease process. Training in primary health care emphatically continues in the fourth term when community and family approaches in the territory are studied, developing Residency in Family Medicine and where, during the intermediate following years and the supervised internships, individual clinical approach in Primary Health Care is trained. Throughout the path, different methodologies are worked such as living in communities, tutorial groups for follow Singular Therapeutic Projects, seminars, etc. The course presents Innovative with Advanced Trend profile, and Teaching Development is a critical point, revealing their fundamental contradiction: the advancements in theory are not fully expressed in practice. It is both concluded that the medical educational policies of transference of the undergraduate course to the countryside should prioritize the analysis of the health network, and that changes in medical education depends on political action, presenting an agenda of proposals.
