Previous issue date: 2014 / Trata-se de uma pesquisa qualitativa, do tipo convergente-assistencial, cujo objetivo geral foi construir com enfermeiros de unidades de clínicas médicas e de uma Comissão de Cuidados Paliativos e Controle da Dor, uma proposta de intervenção para a assistência à saúde dos portadores de doenças crônicas alicerçada nos conceitos e princípios dos Cuidados Paliativos da Organização Mundial de Saúde, bem como na Política Nacional de Educação Permanente em Saúde. Para o alcance de tal objetivo foram traçados os seguintes objetivos específicos: identificar se há a inserção dos conceitos e princípios da filosofia dos Cuidados Paliativos da Organização Mundial de Saúde na prática de enfermeiros atuantes em unidades de clínicas médicas e na Comissão de Cuidados Paliativos e Controle da Dor e teorizar com os enfermeiros do cenário do estudo acerca dos conceitos e princípios da filosofia dos Cuidados Paliativos da Organização Mundial de Saúde com vistas a elencar ações de intervenção para a assistência à saúde dos portadores de doenças crônicas. O desenvolvimento deste estudo foi aprovado por Comitê de Ética em Pesquisa. A coleta de dados ocorreu no período de maio a agosto de 2013, foram utilizados dois instrumentos: entrevistas semiestruturadas e grupos de reflexão. Participaram das entrevistas vinte e dois (22) enfermeiros que atuam em um Hospital Público de Ensino da Região Sul do Brasil, sendo vinte (20) das Unidades de Clínicas Médicas e dois da Comissão de Cuidados Paliativos; dos Grupos de Reflexão participaram sete (07) enfermeiros, todos das Unidades de Clínicas Médicas. A análise seguiu o preconizado na Pesquisa Convergente Assistencial, emergido as seguintes categorias: Cuidados Paliativos: o significado e as limitações atribuídas por enfermeiros; Proposta de visita multiprofissional e seus desafios; Educação permanente: um caminho para fortalecer o Cuidado Paliativo. Os resultados revelam que o significado dos Cuidados Paliativos para os participantes do estudo relaciona-se com a finitude, com a necessidade de formação e com a necessidade de definição de qual portador de doença crônica de ser indicado para a paliação. Nos relatos observou-se que os participantes do estudo têm noção do significado e do que é cuidar em Cuidados Paliativos, principalmente, no que tange aos princípios que envolvem sentimentos de cuidado ao próximo, os quais se aproximam da essência da profissão de enfermagem. Porém, para alguns, ainda está presente a concepção de Cuidados Paliativos como um nome mais sutil ao processo de final de vida. Há, portanto, que se repensar o conceito, discuti-lo com os profissionais, superar a visão limitadas da prática, visando uma assistência mais ampla, que contemple o cuidado no processo de adoecer, no enfrentamento das situações que se apresentam no vivenciar da doença crônica. Para fortalecimento do Cuidado Paliativo os participantes indicaram a realização de visitas coletivas e a educação permanente sobre o tema. Neste sentido, apontam a necessidade de romper com a perspectiva apenas da compaixão pelo próximo que está em processo de morte, garantindo-se um olhar para o Cuidado Paliativo alicerçado na filosofia e princípios dos Cuidados Paliativos, possibilitando uma assistência integral e humanizada, com a utilização a tecnologia disponível inclusive para promover uma morte digna. Muito temos que discutir sobre o tema para que a filosofia e os princípios dos Cuidados Paliativos realmente sejam incorporados no cenário do estudo, possibilitando a autonomia do paciente e familiar na reflexão sobre o processo de ser saudável e adoecer com doenças crônicas, um trabalho em equipe, pautado na construção coletiva.<br> / Abstract : It is a qualitative study, of convergent-care type, whose main goal was to build along with nurses from the medical clinics and a Commission on Palliative Care and Pain Management, an intervention proposal to the health care of patients with chronic diseases. It was founded on the concepts and principles of Palliative Care of the World Health Organization and the National Policy on Permanent Education in Health. In order to reach this objective, it was possible to set the following goals. 1) Identifying if there is an inclusion of the concepts and principles of the philosophy on Palliative Care, at the World Health Organization, in the practice of nurses working at units of medical clinics and the Commission of Palliative Care and Pain Management. 2) Theorizing with the nurses of the study scenario about the concepts and principles of the philosophy on Palliative Care of the World Health Organization in order to list intervention actions for the health care of patients with chronic diseases. The development of this research was approved by the Research Ethics Committee. Data collection took place between May and August 2013. It was possible to use two instruments: semi-structured interviews and reflection groups. Twenty two nurses, working in a Public - School Hospital in southern Brazil, participated in the interviews. Twenty nurses were from the medical clinics units, and two nurses from the Commission of Palliative Care. Seven nurses participated in the reflection groups and they were all from the units of medical clinics. The analysis followed the recommendations from the convergent care research, and it emerged the following categories: Palliative Care, the meaning and limitations conferred by nurses; Proposal for a multi-professional visit and its challenges; Continuing education, a path to strengthen the Palliative Care. The results reveal that the meaning of Palliative Care, to the study participants, is related to finitude, the need for training and the need for defining a person with chronic disease to be indicated for palliation. In the reports, it was possible to observe that the study participants had a sense of meaning about Palliative Care, especially, in relation to the principles that involve feelings on caring other people, which approximate to the essence of the nursing profession. However, for some people, the idea of Palliative Care is still present as a more subtle name to the end of the life process. Therefore, it is necessary to rethink the concept, discuss it with the professionals and overcome the limited vision of the practice in order to have a more extensive assistance that may cover the disease process when facing the situations related to the experience of chronic disease. In order to strengthen the Palliative Care, the participants indicated the achievement of collective visits and continuing education on the topic. In this sense, they just point out the need to break with the perspective of compassion for others who are in the dying process. That is to ensure a look at the Palliative Care grounded in the philosophy and principles of the Palliative Care, providing a comprehensive and humane care and using the technology available even to promote a dignified death. It is required to discuss a lot on the topic, so that the philosophy and principles of Palliative Care can be really incorporated into the study setting. It is also necessary to allow the autonomy of the patient and family to think about the process of being healthy and get sick with chronic diseases, and a teamwork guided by the collective construction.
