Para isso, é essencial conhecer o funcionamento do sistema e adequar sua pratica às necessidades levantadas. Objetivos: contribuir com a organização e funcionamento do sistema de referência e contra-referência na linha de cuidado do câncer de mama em uma região de saúde, com foco na integralidade da atenção em 3 municípios do Colegiado de Gestão Regional do Polo Cuesta. Método: utilizamos a metodologia de “pesquisa-ação”. Análises descritivas e estatísticas simples compuseram a parte quantitativa do estudo, enquanto a escolha pela Análise Temática de Bardin configurou a análise dos dados qualitativos. Resultados e discussão: os dados quantitativos nos permitiram observar que, apesar de não haver grande diferença na disposição de serviços de saúde entre os municípios do colegiado, há divergência entre a proporção de exames realizados por cada um deles e de suas respectivas taxas de morbidade e mortalidade por câncer de mama; enquanto os dados qualitativos nos permitem inferir que apesar do bom funcionamento do sistema, não há prática efetiva de contra-referência nem cobrança das normas estabelecidas pela Programação Pactuada e Integrada. Conclusão: apesar de haver vários pontos fortes na atuação desta linha de cuidados, a falta de comunicação entre os serviços é ainda um empecilho para o funcionamento do sistema e necessita ser otimizada para que a rede de assistência se torne realmente efetiva / Introduction: In order to have the effectuation of the principles in the comprehensive services of the SUS (Brazilian Universal Health System), it is necessary to establish communication strategies between dereference and cross-reference services which make up the network of care. For this, it is essential to know how the system works and adapt its practices to the accounted needs. Objectives: To contribute to the organization and operation of the reference and counter-reference in breast cancer care in a health care region, focusing on comprehensive care in three municipalities of the Regional Management Board Pólo Cuesta. Method: We used the methodology of action research. Simple descriptive analyzes and statistics comprised the quantitative part of the study, while the choice of the Bardin’s Thematic Analysis configured the analyses of the qualitative data. Results and Discussion: the quantitative data allowed us to observe that, although there is no big difference in the provision of health services among municipalities of the college, there is divergence between the proportion of examinations performed by each of them and their respective rates of morbidity and mortality from breast cancer, whereas the qualitative data allow us to infer that, despite the good functioning of the system, there is no effective practice of counter-reference or accountability of the standards set by the Integrated Programmed Pactuation (PPI). Conclusion: Although there are several strengths in the performance of this type of care, lack of communication between services is still a hindrance to the functioning of the system and needs to be optimized so that the support network can be truly effective
