Previous issue date: 2002 / Resumo: Trata-se de um estudo exploratório-descritivo, com abordagem qualitativa que procurou desvendar a amamentação do bebê prematuro, tendo como informante a mulher/mãe. Para melhor compreensão do processo de aleitamento materno, realizamos uma revisão da literatura sobre: aspectos histórico-sociais e culturais da amamentação; anatomia da mama e fisiologia da lactação; aspectos do recém-nascido a termo e pré-termo, com o intuito de estabelecer parâmetros do desenvolvimento normal e o que pode ser encontrado com relação ao prematuro; revisamos também noções sobre a maternidade e o significado da prematuridade para os pais. O referencial teórico escolhido foi o cuidado humano e a rede de apoio social tendo como objetivo geral: analisar a vivência do processo de amamentar em mulheres-mães de bebês prematuros internados em uma UTIN de um hospital filantrópico do interior do estado de São Paulo; e objetivos específicos: 1- identificar o significado atribuído pelas mulheres em relação à amamentação de RNPT, comparando-o com sua prática; 2- identificar os sentimentos e as dificuldades que as mulheres-mães vivenciam no processo do aleitamento materno do filho prematuro; 3- descrever as estratégias utilizadas pelas mulheres para a manutenção da lactação, e 4 - identificar a rede de apoio social. Para a coleta de dados utilizamos entrevistas semi-estruturadas gravadas em dois momentos distintos: antes da amamentação ao peito e depois da amamentação ao peito. Utilizamos também a observação participante e a construção do mapa de rede social pessoal. A análise de conteúdo temático foi a estratégia escolhida para a analisar os dados, emergindo três núcleos de sentidos: 1- Desvendando o processo de alimentação do RNPT, através do qual pudemos identificar que as dificuldades e estratégias utilizadas pelas mulheres/mães de RNPTs são muito semelhantes às utilizadas pelas mães de recém-nascidos a termo, inclusive com relação à sua culpabilização diante dos insucessos; 2 - Desvendando a rede de apoio; pudemos identificar as necessidades das mães em ser cuidadas para se tornarem boas cuidadoras, rede esta onde também se insere a equipe de saúde que cuida e se relaciona com o binômio: bebê-mãe; 3 - Exercendo o papel de ser mãe; neste núcleo percebemos que a mulher só se sente mãe quando tem oportunidade de cuidar de seu bebê e oferecer-lhe um produto que só ela é capaz de fornecer - o leite materno. A partir da compreensão desse processo, propostas foram elaboradas com o intuito de minimizar as dificuldades das mulheres-mães, principalmente com relação à capacitação do pessoal que cuida desse binômio: bebê-mãe; e da sua família / Abstract: his work is an exploratory-descriptive study with qualitative approach that aims to unrnask breast-feeding of the premature baby, having as informer the woman/mother. For a better understanding of the process of maternal breast-feeding, we carried out a revision of literature on: social-historic and cultural aspects of breast-feeding; the anatomy of the breast and the physiology of the lactation; aspects of the new-bom on term and pre-term with the intention of establishing parameters of normal development and what can be found in relation to the premature; we have also revised the maternity and the meaning of the prematurity for the parents. The chosen theoretical referential was the human care and the net of social support. Having as general objective: to identify the motherly difficulties in relation to breast-feeding ofpre-term new-born in the Neonatal lCU of a private hospital; and as specific objectives: 1- to compare their speech and practice in relation to PTNB; 2- to describe the strategies used by the mothers for the maintenance of the lactation, and 3- to identify the net of social support. For collection of data we use halfstructured interviews recorded at two distinct moments: before breast-feeding and afire breast-feeding. We have also used participative observation and the construction ofthe map of personal social netoThe analysis of thematic content was the chosen strategy to analyse the data, ftom where it emerged three sensorial nuclei: 1-Unmasking the process of feeding of the PTNB, through which we were able to identify that the difficulties and strategies used by the women/mothers ofPTNB's are very similar to those used by the mothers of NB's on term, also when it comes to their culpability facing failures; 2. Unmasking the support net, we were able to identify the needs of the women/mothers in being well taken care of in order to become good care takers themselves, in such nets we also include the health team which takes care and relates to the binomial: baby/mother; 3. Playing the role ofbeing mother, in this nucleus we notice that the womanlmother only feels like a mother when she has the chance oftaking care ofher baby and by offering a product that only she is capable of supplying the maternal milk. From the understanding of this process, proposals are elaborated with the intention of minimising the difficulties of them women/mothers, mainly regarding staff qualification which takes care of this binomial: baby/mother; and its family / Mestrado / Mestre em Enfermagem
